Chirurgisches Nähgerät entsprechend dem Oberbegriff des Patentanspruchs 1.
Bei dem im Oberbegriff des Patentanspruchs 1 angegebenen chirurgischen Nähgerät (DE-AS
176) wird das je nach Nahttyp verwendete, auswechselbare Klammermagazin zusammen mit dem
Stößel als Einheit hergestellt und als Einheit im Nähgerät ausgewechselt, um eine Übereinstimmung
von Klammermagazin und Stößel in jedem Fall zu gewährleisten. Die Zuordnung der den verwendeten
Klammern entsprechenden Matrize erfolgt mittels einer Farbkodierung. Ein derartiges Nähgerät, stellt somitci
ne zwangsmäßige Zuordnung von Stößel und Klammer- so
magazin sicher. Die Zuordnung einer dem Klammermagazin entsprechende Matrize, aufgrund der Farbkodierung,
kann jedoch aufgrund von schlechter Beleuchtung bzw. mangelnder Sorgfalt oder aufgrund von Verunreinigungen
fehlerhaft sein. Eine derartige fehlerhafte Zuordnung der Matrize führt zwangsläufig zu einer
mangelhaft ausgebildeten Naht, da die Klammern nicht in der vorgegebenen Weise umgebogen werden. Eine
derartig fehlerhafte Naht führt zu Komplikationen während der Operation oder nach der Operation w
gegebenenfalls zum Tod des Patienten.
Aus dem DE-GM 18 35 500 ist ein chirurgisches Nähgerät mit je nach Nahttyp auswechselbaren
Klammermagazinen bekannt Es ist jedoch keine Vorkehrung getroffen, eine Verwechslung bzw. richtige
Zuordnung von Stößel, Klammermagazin und Matrize auszuschließen. Chirurgische Nähgeräte, wie sie in den
DE-AS 20 51 991 und 22 21 643 beschrieben sind, sind nur für einen Nahttyp geeignet.
Darüber hinaus wurden bereits verschiedene chirurgische Nihgerite der einschlägigen Art entwickelt Diese
Geräte sind zum Vernähen (Verschluß) der Stümpfe von
Hohlorganen bestimmt Unterschiedliche Organe und diverse Operationsverfahren beim Vernähen der
Stümpfe von Hohlorganen erfordern die Anwendung unterschiedlicher Nahttypen. So wird z. B. zum Vernähen
von Darmstümpfen eine linienförmige Naht bzw. zum Vernähen eines Luftröhrenzweiges eine pfahlzaunförmige
oder schräge Naht angewandt Wenn eine besonders dauerhafte Naht erforderlich ist, ist es
zweckmäßig, eine kreuzförmige Naht anzuwenden. Die Nähte unterscheiden sich auch in bezug auf Länge und
Dicke der zusammenzunähenden Gewebe. Beim Vernähen eines Magenstumpfes beträgt die Nahtlänge etwa
120 mm, während beim Vernähen des Stumpfes eines Darms, eines Luftröhrenzweigs, eines Gefäßblocks usw.
die Nahtlänge etwa 40 mm beträgt
Es ist ein chirurgisches Nähgerät bekannt, das üblicherweise beim Vernähen von Organen und
Geweben, insbesondere von Stümpfen von Hohlorganen, mit Metallklammern zur Anwendung kommt
(US-PS 30 79 606).
Dieses umfaßt ein Anschlaggehäuse mit einer Wechselmatrize, die Ausnehmungen zum Umbiegen der
Klammern aufweist einen Antriebshalterschlitten mit einem Wechselmagazin, einen Stößel, einen Antrieb für
den Halterschlitten, einen Antrieb für den Stößel. Das
Anschlaggehäuse ist in Form eines im Querschnitt U-förmigen, länglichen Teils mit Führungen und einem
hakenförmigen Teil, an dessen Innenseite die abnehmbare Matrize mit den Ausnehmungen zum Umbiegen
der Klammern befestigt ist, ausgeführt In den Führungen des Anschlaggehäuses ist der Halterschlitten
mit dem Wechselmagazin beweglich angeordnet Der Halterschlitten weist ebenfalls Führungen auf. Das
Klammermagazin besitzt Klammertvjten, deren Anordnung
der der Ausnehmungen auf der Matrize entspricht In jeder Klammernut befindet sich «ine Metallklammer.
In den Führungen des Halterschlittens ist der Lamellen aufweisende Stößel beweglich angeordnet Die Anordnung
der Lamellen am Stößel stimmt mit der der Klammernuten im Magazin überein. Der Antrieb für
den Halterschlitten ist e,n Schraubenantrieb, der für den Stößel ein Hebelantrieb ist
Das Gerät arbeitet wie folgt Der hakenförmige Teil des Gerätes wird unter das zu vernähende Organ
herangeführt Durch Drehen der Schraubenmutter des Antriebs für den Halterschlitten wird dieser zusammen
mit dem Klammermagazin und dem Stößel an die Matrize herangeführt Dabei wird das zu vernähende
Gewebe komprimiert so daß die Blutzirkulation unterbrochen wird, und es wird zwischen der Ebene der
Matrize, auf der sich die offenen Klammernuten befinden, durch das Magazin ein sogenannter Durchnähspalt
eingestellt Durch Betätigen eines Hebels (Stößelantrieb) wird der Stößel ebenfalls in Richtung auf
die Matrize vorgeschoben, wobei die Stößellamellen in die Klammernuten eindringen und die Klammern aus
diesen ausstoßen. Nachdem die Metallklammern mit ihren freien Enden das zusammengedrückte Gewebe
durchgestochen haben, treffen diese auf die Ausnehmungen der Matrize auf und werden durch diese
»B«-förmig umgebogen, so daß die Klammern das Gewebe fest verbinden.
Auch mit diesem Gerät kann nur ein Nahttyp, z. B. eine linienförmige Naht, angelegt werden. Um alle
Nahttypen erzeugen zu können, müssen entsprechend viele Geräte vorhanden sein, Das bringt aber eine
beträchtliche Erhöhung der Operationsausrüstung mit sich.
Die Aufgabe der Erfindung besteht darin, ein s chirurgisches Nähgerät zum Anlegen einer durch
Klammern gebildeten Naht gernäß dem Oberbegriff des Patentanspruchs 1 so weiterzubilden, daß in jedem Fall
eine fehlerhaft!; Zuordnung van Matrize zu Klammermagazin
und Stößel ausgeschlossen ist ι ο
Diese Aufgabe wird erfindungsgemäß durch die im Patentanspruch 1 gekennzeichnete Erfindung gelöst
Eine vorteilhafte Ausgestaltung der Erfindung ergibt
sich aus dem Patentanspruch 2.
Durch die zwangsmäßige Zuordnung der einzelnen Elemente ist in jedem Falle sichergestellt, daß das
Nähgerät nur dann funktionsfähig ist, wenn für einen
bestimmten Nahttyp der entsprechende zugehörige Stößel bzw. die entsprechende zugehörige Matrize
verwendet wird, wodurch eine fehlerhafte Naht 100%ig
ausgeschlossen wird. Das Gerät ist preiswert "jnd bequem zu handhaben.
Nachstehend wird die Erfindung anhand von Ausführungsbeispielen unter Hinweis auf die Zeichnungen
näher erläutert Es zeigt 2s

Der chirurgische Nähapparat zi<r Anlegung einer
Klammernaht umfaßt ein Anschlaggehäuse 1 (Fig. 1) mit einer Wechselmatrize 2, auf der sich Ausnehmungen
3 (Fig.3, Fig.4, Fig.6, Fig.7, Fig.8, Fig.9) zum
Umbiegen von Klammern 4 (Fig.4) befinden, ferner
einen Antriebshalterschlitten 5 (Fig. 1) mit einem so Wechselmagazin 6, einen Stößel 7, einen Antrieb 8 für
den Halterschlitten und einen Antrieb 9 für den Stößel.
Das Anschlaggehäuse 1 wird durch einen im Querschnitt im wesentlichen U-förmigen länglichen Teil
10 mit Führungen 11 (gestrichelt angedeutet) und einen
hakenförmigen Teil 12 gebildet, an dessen Innenseite die Matrize 2 angeordnet ist. Zum Einbau der Wechselmatrize
2 in den Apparat weist sie eine T-Nut 13 (F i g. 6,7,
8, 9) und die Innenseite des hakenförmigen Teils 12 einen T-Vorsprung 14 auf (Fig.2). Am Endteil 15
(F i g. 1) des Anschlaggehäuses ist ein Vorsprung 16 zum Zusammenwirken mit dem Antrieb 8 für den Halterschlitten
ausgeführt. Zur bequemeren Handhabung des Apparates ist das Anschlaggehäuse 1 mit einem Griff 17
versehen. «
Der Antriebshalterscb'Jtten 5 ist in Form eines
verlängerten Stabes 18 mit einem Schaft 19 an einem Ende und einem Halter 20 für das Wechselklammermagazin
6 am anderen Ende ausgebildet
Zum Arretieren des Magazins 6 weist der Halter 20 Vorsprünge 21 (F i g. 2) und das Magazin 6 (F i g. 6,7,8,
9) Profilnuten 22 auf, Außerdem sind am Magazin 6 Durchgangsnuten 23 für die Klammern vorgesehen,
deren Anordnung der der Ausnehmungen 3 auf der Matrize entspricht Am Antriebshalterschlitten 5 sind
zur Zusammenarbeit mit dem Anschlaggehäuse 1 (Fig. 1) Vorsprünge 24 ausgeführt Der Schaft 19 des
Halterschlittens ist hohl und mit Außen- und Innengewinde ausgeführt
Der Stößel 7 stellt ebenfalls einen länglichen Stab 25 dar, an dessen einem Ende eine nicht dargestellte T-Nut
und an dessen anderem Ende ein Kopf 26 mit Lamellen 27 ausgeführt ist Die Konfiguration und Anordnung der
Lamellen 27 des Stößels 7 stimmen mit denen der Klammernuten 23 überein (F i g. 6,7,8,9).
Der Stößel 7 (F i g. 1) ist abnehmbar ausgeführt Dazu
ist der Stößel 7 mit Hilfe einer T-Nut mit einem T-Vorsprung 40 des Antriebs 9 für ^n Stößel 7 lösbar
gekoppelt Der Antrieb 8 für den Halter schlitten 5 wird durch eine Schraubenmutter 28 gebildet, auf der eine
kreisförmige Eindrehung 29 ausgeführt ist Die Schraubenmutter 28 bildet mit dem Außengewinde des
Schaftes 19 ein Schraubenpaar und steht mit ihrer kreisförmigen Eindrehung 29 mit dem Vorsprung 16 des
Anschlaggehäuses 1 in beweglichem Eingriff.
Der Antrieb 9 für den Stößel 7 wird ebenfalls durch eine Schraube 30 mit einer kreisförmigen Eindrehung 31
gebildet Die Schraube 30 bildet mit dem Innengewinde des Schaftes 19 des Halterschlittens 5 ein Schraubenpaar
und steht mit ihrer kreisförmigen Eindrehung 31 mit der nicht dargestellten Gabel des Stößels 7 in
beweglichem Eingriff.
Auf der Wechselmatrize 2 (Fig.6, 7, 8, 9) ist eine
stirnseitige Führungsplatte 32 ausgeführt, die senkrecht zur Oberfläche 33 mit den Ausnehmungen angeordnet
ist und in Richtung auf das Magazin hervorragt Die stirnseitige Führungsplatte 32 hat ein Profil 34, dessen
Form einem bestimmten Nahttyp entspricht (Fig.5).
Eine der Stirnflächen 35 (F i g. 6,7,8,9) des Magazins 6
und eine der Stirnflächen 36 des Stößels 7 sind identisch mit dem Profil der stirnseitigen Führungsplatte 32 für
einen bestimmten Nahttyp ausgebildet Das Profil der stirnseitigen Führungsplatte kann durch Nuten 37 und
das der Stirnfläche des Magazins und Stößels durch gleiche Vorsprünge 38 gebildet werden.
Zur Begrenzung des Gewebes gegen Bewegen hinter die Nahtzone während des Durchnähens ist am Apparat
ein Zusatzteil in Form einer Begrenzungsleiste 39 (Fig. 1) zwischen dem hakenförmigen Teil 12 und der
Schraubenmutter 28 hin und her verschiebbar angeordnet
Die Handhabung des chirurgischen Nähapparates ist wie folgt Sie wird dabei an einem konkreten Beispiel
eines Darmstumpfnähapparates dargelegt
Nach in der modernen Chirurgie akzeptierten Verfahren wird ei" Darmstumpf mit einer linienförmigen
Naht vernäht Für die linienförmige Naht werden entsprechend Matrize 2 (F i g. 6), Wechselmägäzin 6 und
Wechselstößel 7 ausgewählt. Durch Dtehen der Schraubenmutter 28 (Fig. 1) des Antriebs für den
Halterschlitten 5 entgegen dem Uhrzeigersinn wird der Halterschlitten 5 in r'je Ausgangsstellung zurückgeführt
und aus dem Anschlaggehäuse 1 herausgenommen.
Der Stößel 7 wird derart eingestellt, daß seine T-Nut (nicht dargestellt) die kreisförmige Eindrehung 31 der
Schraube des Antriebs 9 für den Stößel umfaßt und sein
Kopf 26 in den Halter 20 hineinragt Dabei muß die profilierte Stirnfläche 36 (Fig.6) in bezug auf den
Vorsprung 24 in die entgegengesetzte Richtung (Fig.!)
gerichtet sein.
Der Antriebshalterschlitten 5 wird zusammen mit dem Stößel 7 derart in den im Querschnitt U-förmigen
Teil 10 des Anschlaggehäuses 1 eingelegt, daß der Stößel 7 auf dem Boden des Teils 10 aufliegt Danach
wird die Schraubenmutter 28 des Antriebs für den Halterschlitten um eine Umdrehung gedreht so daß Hie
Vorsprünge 24 in die Führungsnuten 11 des Anschlaggehäuses
1 hineinragen. Auf diese Weise wird ein bewegliches, zwischen dem hakenförmigen Teil 12 und
dem Endteil 15 des Anschlaggehäuses 1 hin und her verschiebbares Gelenk gebildet is
Das Wechselmagazin 6 (Fig.2) wird derart in den
Halterschlitten eingesetzt, daß die Vorsprünge 21 in die Nuten 22 des Magazins 6 eingreifen. Dabei wird es so
weit wie möglich an den Vorsprüngen 21 entlang vorgeschoben. Die Matrize 2 wird schließlich so am
hakenförmigen Teil 12 (F i g. 1) des Anschlaggehäuses 1 montiert, daß der T-Vorsprung 14 (Fig.2) des
Anschlaggehäuses 1 in die Nut 13 (F i g. 6) der Matrize 2 (F i g. 2) hineinragt, wobei die stirnseitige Führungsplatte
zur Seite des Magazins 6 hin gerichtet ist
Der hakenförmige Teil 12 des Anschlaggehäuses 1 wird nun unter das zu vernähende Organ (im
vorliegenden konkreten Fall unter den Darm) herangeführt Die Begrenzungsleiste 39 (Fig. 1) wird in
Richtung auf den Teil 12 derart vorgeschoben, daß das zu vernähende Organ zwischen diesem und der Matrize
2, Magazin 6 und Begrenzungsleiste 39 eingeschlossen ist
Durch Drehen der Schraubenmutter 28 des Antriebs für den Halterschlitten 5 wird dieser gemeinsam mit
dem Magazin 6 und dem Stößel 7 an die Matrize 2 herangeführt, wobei er das Gewebe zur Bildung des
Durchnähspalts zusammendrückt. Danach wird der Stößel 7 durch Drehen der Schraube 30 seines Antriebs
9 im Uhrzeigersinn in Richtung auf die Matrize vorgeschoben, wobei seine Lamellen 27 in die
Klammernuten 23 des Magazins 6 eingreifen und die Klammern aus den Nuten des Magazins ausstoßen. Die
Klammern durchstechen mit ihren freien Enden das Gewebe, die danach auf die Ausnehmungen 3 der
Matrize 2 auftreffen und an diesen B-förmig umgebogen werden, so daß sie eine Naht bilden.
Das vernähte Organ wird danach in der Ebene des Apparates beschnitten. Durch Drehen der Schraubenmutter
28 des Antriebs für den Halterschlitten 5 entgegen dem Uhrzeigersinn wird dieser zusammen mit
dem Stößel 7 und Magazin 6 in die Ausgangsstellung zurückgeführt Das Organ wird freigegeben und
dadurch ist der Vorgang beendet
Bei dem Apparat ist eine zwangsläufige Übereinstimmung von Wechselmatrize. Magazin und Stößel
gegeben, wodurch eventuelle Fehler des medizinischen Personals beim Anbringen dieser Wechselbauteile an
dem Apparat ausgeschlossen sind. Dies ist entscheidend, weil in dem Fall, in dem einer der Bauteile (Matrize,
Stößel, Magazin) für einen Nahttyp, z. B. für eine linienförmige Naht, mit Wechselbauteilen eines anderen
Nahttyps, z. B. für eine pfahlzaunförmige Naht verwendet wird, dies mindestens einen Bruch des Stößels
hervorruf.', und darüber hinaus die Folge hat, daß die Klammern falsch gebogen werden, wodurch die Naht
nicht hält mit der möglichen Konsequenz, daß der Patient stirbt.
Bei dem vorgeschlagenen Apparat sind diese Fehler völlig ausgeschlossen, da in dem Fall, in dem wenigstens
eines der Wechselbauteile aus Versehen verwechselt wird, der Apparat von Anfang an nicht arbeitet, d. h. es
nicht möglich ist das Gewebe zusammenzudrücken, da das Profil der stirnseitigen Führungsplatte 32, das nicht
mit dem der Stirnfläche des Magazins zusammenpaßt sich nicht dem Antriebshalterschlitten der Matrize
nähern läßt.
Claims (2)
1. Chirurgisches Nlhgerlt zum Anlegen einer durch Klammern gebildeten Naht mit einer an einer
Stützbacke auswechselbar angeordneten Matrize s mit einer Fliehe mit Ausnehmungen, einem an einem
der Stützbacke gegenüberliegend, verschiebbar angeordneten Schlitten auswechselbar angeordneten
Klammermagazin und einem an einem Stößelantrieb auswechselbar angeordneten, dem Klammermagazin
unter zwangsweiser Zuordnung entsprechenden Stößel, dadurch gekennzeichnet, daß die Matrize (2) eine stirnseitige, senkrecht zur
Fliehe (33) mit den Ausnehmungen (3) verlaufende Führungsplatte (32) mit einem Führungsprofil (34) is
aufweist, daß das Klammermagazin (6) an seiner der Führungsplatte (32) zugewandten Stirnseite (35) ein
mit dem Führungsprofil (34) zusammenwirkendes ProHl aufweist, daß der Stößel (7) an seiner der
Führungsplatte (32) zugewandten Stirnseite (36) ein »
mit dem Führungsprofil (34) zusammenwirkendes ProFil aufweist, und daß die zusammenwirkenden
Profile von Führungsplatte (32), Klammermagazin (6) und Stößel (7) für jeden Nahttyp derart
ausgebildet sind, daß Lage und Abstandsbeziehungen der Führungsnuten die zwangsweise Zuordnung
ermöglichen.
2. Chirurgisches Nähgerät nach Anspruch 1,
dadurch gekennzeichnet, daß die profilierte Oberfläche der Führungsplatte (32) der Matrize (2) durch zu
