Beschreibung
Die Erfindung betrifft eine Vorrichtung nach dem Oberbegriff des Anspruch 1.
In der modernen Neurochirurgie möchte man durch möglichst geringe Öffnungen in den Hohlraum des
Kopfes eindringen, um zum Beispie! Biopsien zu nehmen oder Tumore zu entfernen. Dies möchte man
außerdem auch noch unter einem Sichtsystem, wie z.B. der Kernspintomographie durchführen.
Herkömmliche stereotaktische Stellvorrichtungen für derartige Interventionen sind aufwendig und
beziehen ihren Referenzpunkt nicht am Kopf, sondern am Operationstisch. Der Kopf des Patienten
kann sich jedoch in Bezug auf diesen Operationstisch bewegen. Hier wurden komplizierte
Haltevorrichtungen erstellt. All diese Dinge sind sehr groß und teuer. Die Erfindung hat zum Ziel, eine
kleine Lösung, die eventuell auch als Kurzzeiiimpiantat konzipiert sein kann, darzulegen. Die Erfindung
wird in den folgenden Figuren erläutert:

In Fig. 1 ist die Neurotrokarvorrichtung 1 gezeigt. Sie besteht aus der Befestigungsplatte 2 und der
Nadelhalterung 3, die mittels eines Kugelgelenkes 15 in der Befestigungsplatte sitzt. Die
Nadelhalterung weist eine lnstrumenteneinführröhre vor, die vorzugsweise als kreisrunde Bohrung
ausgestaltet ist. Es kann sich auch jede andere Geometrie anbieten. Festgestellt wird das Kugelgelenk
15 zur Befesiigungspiatte 2 mittels einer Klemmvorrichtung 5, die durch eine Rändelschraube 14
bedient werden kann. Ein Instrument kann selbst mittels einer separaten Fixiervorrichtung 4 in der
Nadelhalterung gehalten werden.
Die Vorrichtung ist aus derartigen Materialien konzipiert, daß sie im Kemspintomographen einsetzbar
ist, nicht deflektiert und auch keine wesentlichen Bildverzerrungen, sogenannte Bildartefakte setzt. Als
Materialien können hier Titanlegierungen angewendet werden, wie sie in der Patentschrift
DE 195 31 117.5-35 dargestellt sind. Ebenso kann das Material aus Kunststoff sein, aus Keramik oder
aus verchromten Messing- oder Aluminiumlegierungen bestehen. Die Nadelhalterung 3 kann aus dem
zur Befesfigungspiatte stehendem Lot 13 um den Winkel &agr; herausgedreht werden. Nach Durchbohren
der Schädeidecke wird die Vorrichtung mittels eines Schneidgewindes 6, was sich unterhalb der
Befestigungsplatte 2 befindet, in die Schädeldecke gedreht und sitzt fest an dem Schädei.

Fig. 3 und 4 zeigen, wie eine Nadel oder ein instrument in die Vorrichtung eingeführt werden kann. Die
Nadel 7 ist durch die Fixiervorrichtung 4 in einem festen Winkel &agr; in die Vorrichtung 1 eingefügt Die
Einstechtiefe wird über die Federvorrichtung 4 eingestellt
Nach einer ersten Anwendung kann die Nadel entfernt werden, der Neurotrokar bleibt an Ort und
Stelle und dient als Führungs- und Positionierhilfe für weitere Instrumente. Bei Bedarf kann er auch
steril verschlossen werden.
Im Fall der Fig. 3 ist eine neuronale Aspiraiionsbiopsienade! gezeigt mit einer Nadelspitze, die als
Rundschliff konzipiert ist. Die Nadel verfügt über einen Führungsdraht 10 sowie einem
Schlauchverbinder, der z.B. als Luer-Lock-Anschluß ausgeführt sein kann 8 und einem
Führungsdrahtfingergriff 9 zum Bedienen des Führungsdrahtes.

Claims (5)
1. Vorrichtung zur Einführung von medizinischem Besteck in den neuronalen Bereich des Kopfes,
dadurch gekennzeichnet, daß diese z.B. mitteis eines Schneidgewindes (6) in den Schädelknochen fest eingedreht werden kann und ein Instrument (4) in einem definierten
Raumwinke! eingeführt werden kann.
2. Vorrichtung nach Anspruch 1,
dadurch gekennzeichnet, daß mittels einer Klemmvorrichtung (5) der Winke! &agr;, der sich ergibt
aus der Senkrechten (13) zur Befestigungspiatte und der Achse der rohrförmigen Einfuhrröhre
(12), einstellbar ist
3. Vorrichtung nach einem der oberen Ansprüche,
dadurch gekennzeichnet, daß die eingeführten Instrumente in einer definierten Position mittels
einer Klemmvorrichtung (14) fixiert werden können.
4. Vorrichtung nach einem der oberen Ansprüche,
dadurch gekennzeichnet, daß sie steril verschließbar ist und für einige Zeit im Patienten
verbleiben kann und eine einmal eingestellte Position später wieder erreicht werden kann.
5. Vorrichtung nach einem der oberen Ansprüche,
dadurch gekennzeichnet, daß diese aus einem biokompatiblen Material gefertigt ist, welches
im Kernspintomographen einsetzbar ist und keine störenden Bildverzerrungen, sogenannte
Artefakte aufweist, wie Keramik, titanhaltige Legierungen, Kunststoff oder verchromte Messingoder
Aluminiumlegierungen.
