Adapter für Endoprothesen
Die Neuerung betrifft einen Adapter für Endoprothesen-, insbesondere einen Steckadapter nicht nur bei Hüftgelenkrevisionen, sondern auch für den Austausch eines Keramiksteckkopfes vom Metallkonus eines implantierten Schaftes und anschließenden Neuaufbau und einer stufenlosen Korrektur-Einstellung des CCD- Winkels und des Halslängenausgleiches, wobei ein Adapter mit exzentrischem Innenkonus zur Vergrößerung des gesamten Auslenkwinkels (ROM) gegenüber der Pfanne, an seiner, dem exzentrischen Innenkonus gegenüberliegenden Stelle mit der größten Wandstärke, eine Ausnehmung besitzt.
Die Verwendung von Adaptern für Endoprothesen, als Bindeglied zwischen dem Schaft einer implantierten Hüftendoprothese und der Steckkugel, ist weitgehend bekannt.
Um die größtmöglichste Auslenkung der Hüfte gegenüber der Hüftpfanne zu ermöglichen, bekannt als ROM - Range of Motion-, sind bereits Adapter bekannt, die an den Stellen der Berührung mit der ortsfesten Pfanne eingeengt oder schmaler gehalten.
Eine weitere Lösung zur Vergrößerung des Auslenkwinkels (ROM) ist eine Einschnürung oder Durchmesserverkleinerung direkt unterhalb des Schaftes von Hüftendoprothesen.
So ist beispielsweise durch das deutsche Gebrauchsmuster G 299 08 391 ein Gelenkprothesen- System bekannt, bei dem auf einem konusförmigen Schaft einer implantierten Hüftendoprothese ein länglicher Adapter mit einem Innen- und Außenkonus verbringbar ist, wobei auf dem Außenkonus eine Gelenkkugel mit entsprechendem Innenkonus gebracht werden kann. Der Adapter hat im Bereich außerhalb der Steckkugel eine den Außendurchmesser verkleinernde Einschnürung um bereits hierdurch eine größere Auslenkung (ROM) der Hüfte zu erzielen. Allerdings handelt es hier um einen geraden Adapter dessen Innenkonus zum Außenkonus keinerlei Exzentrizität aufweist.
Auch bei Adaptern mit Exzentrizität zwischen Innenkonus zum Außenkonus, insbesondere wenn die Mittelachsen des Innenkonus zum Außenkonus nicht parallel verlaufen, ist der Auslenkungswinkel (ROM) begrenzt. Dieser Winkel beträgt im allgemeinen zwischen 126° und 134°.
Die Neuerung hat sich daher die Aufgabe gestellt, den Auslenkwinkel bei Adaptern mit Exzentrizität zwischen Innenkonus zum Außenkonus zu
vergrößern, ohne die Stabilität und Festigkeit der Wandstärke eines Adapters zu beeinträchtigen.
Zur Lösung dieser gestellten Aufgabe wird vorgeschlagen, den Adapter, an der dem exzentrischen Innenkonus gegenüberliegenden Stelle mit der größten Wandstärke und außerhalb des Überdeckungsbereiches der Steckkugel, mit einer Ausnehmung, nicht über den vollen Umfang, zu versehen. Dabei verläuft diese Ausnehmung nach fast dreiviertel des Umfanges wieder auswärts auf den Außendurchmesser des Adapters.
Weitere Vorteile ergeben sich aus der nachfolgenden Beschreibung und werden anhand von Zeichnungen näher erläutert. Es zeigt:
Fig. 1 Ein Kugelgelenk mit dem Winkel der größten Auslenkung, Fig. 2 einen Adapter mit einer Ausnehmung am Umfang.
Fig. 1 zeigt beispielweise nur die Grundbestandteile einer Hüftendoprothese, mit Pfanne 1, Gelenkkugel 2 und einem Schaft 3 einer Endoprothese, wobei dies auch ein Teil eines Adapters sein kann. Dargestellt ist die größtmögliche Auslenkung nach links, wobei sich Schaft 3 und Pfanne 1 am Punkt 4 berühren. Die größtmögliche Auslenkung nach rechts ist durch die Linie 5 dargestellt, so dass sich ein größter Auslenkungswinkel ROM ergibt. Der Schaft 3 ist hierbei bereits gegenüber der Kugel 2 stark im Durchmesser verringert worden.
Für den Fall der Notwendigkeit der Verwendung eines Adapters mit Exzentrizität zwischen Innenkonus zum Außenkonus, zur freien Einstellung des CCD- Winkels und bei gleichzeitigem Halslängenausgleich, wird ein Adapter 10 gemäß Fig. 2 benutzt. Dieser Adapter 10 hat einen Außenkonus 11 und einen Innenkonus 12, wobei dessen Mittelachse 13 zur Mittelachse 14 des Adapters geneigt ist. Sitzt später der Innenkonus auf dem Schaft einer implantierten Endoprothese, so kann der Chirurg in freier Einstellung die Gelenkkugel 20 in alle Stellungen zwischen Valgus- Varus und Retrotorsion und Antetorsion bringen.
Der Adapter 10 hat nun an seinem Außenumfang, vorzugsweise an der Stelle größter Wandstärke zum Innenkonus 12 eine Ausnehmung 15, welche wie dargestellt, als eine Art Hohlkehle gestaltet ist. Diese Ausnehmung 15 verjüngt sich nach nahezu dreiviertel des Umfanges wieder auf den sonstigen Außendurchmesser. Diese Ausnehmung 15 kann aber auch derartig gestaltet sein, dass sie gleichmäßig und parallel bis nahezu hälftig des Umfanges verläuft und dann erst gerundet auf den Außendurchmessers zurück geht. Mit einem solchermaßen gestalteten Adapter 10 läßt sich der Auslenkwinkel (ROM) um einige Grade vergrößern.

Claims (6)

1. Adapter für Verbindung einer Gelenkkugel mit einem Zapfen und umlaufender Einengung im Bereich des Außendurchmessers, dadurch gekennzeichnet, dass der Adapter (10) eine Exzentrizität zwischen Außenkonus (119 und Innenkonus (12) besitzt und eine nicht über den gesamten Umfang verlaufende Ausnehmung (15) besitzt.
2. Adapter nach Anspruch 1, dadurch gekennzeichnet, dass die Ausnehmung (15) außerhalb des durch die Gelenkkugel (20) überdeckten Bereich angeordnet ist.
3. Adapter nach Anspruch 1, dadurch gekennzeichnet, dass sich die Ausnehmung (15) an der Stelle größter Wandstärke zum Innenkonus (12) befindet.
4. Adapter nach Anspruch 1, dadurch gekennzeichnet, dass sich die Ausnehmung (15) an der Stelle größter Wandstärke zum Innenkonus (12) befindet und von dort dreiviertel des Außenumfanges des Adapter (10) einnimmt.
5. Adapter nach Anspruch 1, dadurch gekennzeichnet, dass die Ausnehmung (15) zentrisch zulaufend sich nach dreiviertel Außenumfang auf den Außendurchmesser verläuft.
6. Adapter nach Anspruch 1, dadurch gekennzeichnet, dass die Ausnehmung (15) parallel in gleicher Tiefe bis dreiviertel des Außenumfanges des Adapter (10) verläuft.
