Anwendung finden die oben beschriebenen Systeme zur Verödung von Weichteiltumoren in unterschiedlichen Organen, insbesondere in Leber, Prostata und Gehirn.
Bei dem bisher üblichen minimal-invasiven Vorgehen erfolgt als erstes die Punktion des Tumors durch die Haut hindurch unter lokaler Betäubung und bildgebender Kontrolle mittels Röntgen-Computertomographie oder Magnetresonanz-Tomographie. Sodann wird in einem weiteren Arbeitsschritt der flexible Hüllkatheter im Punktionskanal mittels Schleusensystem positioniert und anschließend der eigentliche Laserapplikator eingeführt.
Als nachteilig bei dem beschriebenen Vorgehen hat sich jedoch herausgestellt, daß bei stark durchbluteten Organen wie der Leber oder der Niere ein Großteil der eingebrachten Energie durch den Blutstrom wieder abtransportiert wird und somit verloren geht, so daß gerade bei sehr großen Tumoren häufig mehrere synchrone 5 oder asynchrone Laserapplikationen erforderlich sind.

In einem bevorzugten Ausführungsbeispiel wird durch die Verwendung von Quarzglas bzw. Saphir als transparentem Hohlzylinder im Vergleich zu den bestehenden Systemen ein um den Faktor vier verbesserter Abtransport überschüssiger Wärme aus dem Gewebe ermöglicht. Weiterhin wurde erkannt, daß durch den hohen Schmelzpunkt von Glas die thermische Zerstörschwelle deutlich heraufgesetzt wird, so daß die Laserverödung sicherer durchgeführt werden kann. Um dem Anwender eine Orientierung zu ermöglichen und eine reproduzierbare Position des Punktionsschaftes im Körpergewebe zu gewährleisten, ist der Stabilisierungsmantel vorzugsweise mit äquidistanten Markierungen versehen.
In einer Variante wird dem Fluid in bekannter Weise eine streuende Substanz zugesetzt, um die Abstrahleigenschaften des weiter unten beschriebenen Diffusorelementes zu verbessern. In einer speziellen Ausführungsform kann sogar auf das Diffusorelement ganz verzichtet werden und die aus dem Lichtwellenleiter prograd austretende Strahlung durch die im Fluid gelöste lichtstreuende Substanz homogen verteilt werden. In einer weiteren Variante wird dem Fluid ein Ultraschallkontrastmittel zugesetzt, um die Lokalisation des gesamten durchströmten Schaftes im Körpergewebe bei Verwendung des bildgebenden Ultraschalls zu erleichtern. In einer weiteren Ausführungsform wird dem Fluid ein 0 Magnetresonanz-Kontrastmittel zugesetzt, um die Lokalisation bei Verwendung im vorzugsweise offenen Magnetresonanz-Tomographen zu erleichtern.
In einer weiteren Ausführungsvariante wird anstelle der Temperatur die sogenannte Autofluoreszenz des Gewebes bei Anregung im Wellenlängenbereich 200 bis 700 nm als Vitalparameter für die Beurteilung der vollständigen thermischen GewebeVerödung genutzt. Das verstärkte oder abgeschwächte Auftreten bestimmter Fluoreszenzbanden (z.B. NADH-Fluoreszenz) ist ein Maß für den vitalen Gewebezustand und kann zur Steuerung bzw. Regelung der Laserausgangsleistung verwendet werden.
