As used herein, the term "and/or" includes any and all combinations of one or more of the associated listed items.
Fig. 1 is a schematic structural view of a staple cartridge assembly in the prior art, and fig. 2 is a schematic structural view of a side of a staple cartridge rack of the staple cartridge assembly in the prior art, as shown in fig. 1 and 2, the staple cartridge assembly in the prior art comprises: the nail bin comprises a nail anvil 1 and a nail bin frame 2, wherein a first side wall 12 and a second side wall 13 extending towards the nail bin frame 2 are arranged on two sides of the end portion of the nail anvil body 11, the end portion of the nail bin frame 2 is connected with the first side wall 12 and the second side wall 13 through pins 3, and therefore the nail bin frame 2 rotates around the nail anvil 1 through the pins 3, and a certain included angle is formed between the nail anvil 1 and the nail bin frame 2.
The applicant has found that when the angle is formed between the anvil 1 and the cartridge holder 2, the end of the upper end surfaces of the first and second side walls 12, 13, which end is away from the pin 3, needs to be engaged with the lower end surface of the cartridge holder 2, so as to avoid a gap between the end of the upper end surfaces of the first and second side walls 12, 13, which end is away from the pin 3, and the lower end surface of the cartridge holder 2, and once the gap is formed, the edge of the tissue may be caught in the gap when the cartridge assembly is in the open state to clamp the tissue. If the nail bin assembly needs to be closed at this time, if the tissues clamped into the gap are less, in the forced closing process, the tissues can be cut or damaged by one end of the upper end surfaces of the first side wall 12 and the second side wall 13 far away from the pin 3 and the lower end surface of the nail bin frame 2; if the tissue clamped into the gap is thicker, the cartridge assembly cannot be closed at all, and a doctor who uses the cartridge assembly needs to operate again to clamp the tissue, so that inconvenience is brought to the user. Therefore, in the above-mentioned structure, in order to prevent a gap from occurring between the end of the upper end surface of the first side wall 12 and the second side wall 13 far away from the pin 3 and the lower end surface of the magazine rack 2, the included angle between the anvil 1 and the magazine rack 2 in the open state can only reach about 15 degrees.
Because the included angle between the nail anvil 1 and the nail bin frame 2 in the opening state is small, a doctor using the nail anvil 1 and the nail bin frame 2 under a small angle is not easy to clamp tissues between the nail anvil 1 and the nail bin 23, therefore, the opening angle between the nail anvil 1 and the nail bin frame 2 in the opening state needs to be further enlarged, and after the opening angle is enlarged, a gap must not appear between one end of the upper end surfaces of the first side wall 12 and the second side wall 13, which is far away from the pin 3, and the lower end surface of the nail bin frame 2. Meanwhile, the applicant has also found that, as shown in fig. 1, if the position of the pin 3 for connecting the end portion of the magazine holder 2 and the first and second side walls 12 and 13 is moved leftward, the opening angle of the open state between the anvil 1 and the magazine holder 2 can be enlarged without a gap between one end of the upper end surfaces of the first and second side walls 12 and 13, which is far from the pin 3, and the lower end surface of the magazine holder 2. However, the above scheme is affected by objective factors and cannot be realized.
The specific reason is that the specifications of the staples used in the current anastomats all over the world are basically uniform, the total length of the staple positions on the staple cartridge 23 is fixed, and only three specifications of 30mm, 45mm and 60mm are provided, for safety, the positions of the staples at the right end on the staple cartridge 23 must exceed the left ends of the first side wall 12 and the second side wall 13, and when the cut tissue is sutured through the staples on the staple cartridge 23, the end parts of the sutured positions must exceed the end parts of the clamped and cut tissue, so that the effectiveness of suturing is ensured, and the situation that the end parts of the cut tissue are not sutured fully and can be detached is prevented. This results in a distance of about 1 and a half staple having to be retained between the first and second sidewalls 12, 13 at the inner portion to allow the tissue cut by the knife to be completely stapled. This results in the position of the staple at the right end of the staple cartridge 23 having to exceed the left ends of the first and second side walls 12, 13, and the distance exceeding the left ends of the first and second side walls 12, 13 must be at least a distance of half a staple width, so that the position of the pin 3 cannot be further moved to the left, and the space between the first and second side walls 12, 13 of the anvil 1 has to be provided with a half staple, and a space for the knife to enter needs to be left. In summary, in the existing staple cartridge assembly, the distance between the pin 3 and the left ends of the first side wall 12 and the second side wall 13 is made to be close to the minimum, and on such a premise, the included angle between the anvil 1 and the staple cartridge frame 2 in the open state reaches about 15 degrees. Therefore, this manner of moving the position of the pin 3 for connecting the end portion of the cartridge frame 2 and the first and second side walls 12, 13 leftward cannot be realized due to the structure of the cartridge assembly itself.
To the above situation, in order to effectively enlarge the opening angle between the nail anvil 1 and the nail bin frame 2 in the open state, the applicant innovatively provides a nail bin assembly for the anastomat, fig. 3 is the embodiment of the present invention provides a front view of the nail bin assembly for the anastomat in the closed state, fig. 5 is the embodiment of the present invention provides a schematic view of the nail bin assembly for the anastomat in the open state, fig. 6 is the front view of the nail bin assembly for the anastomat in the open state, as shown in fig. 3 to fig. 6, the nail bin assembly for the anastomat may include: the nail anvil comprises a nail anvil 1 and a nail bin frame 2 with a nail bin 23, wherein the nail anvil 1 comprises a nail anvil body 11 with a forming groove 16, two sides of the end part of the nail anvil body 11 are provided with a first side wall 12 and a second side wall 13 which extend towards the nail bin frame 2, the end part of the nail bin frame 2 is connected with the first side wall 12 and the second side wall 13 through a pin 3, the nail bin frame 2 and/or the nail bin 23 is provided with a first blocking part 21 which extends towards the nail anvil 1, the nail anvil 1 is provided with a first concave part 14 which can contain the first blocking part 21, and the first concave part 14 is positioned at one end part of the first side wall 12 or the second side wall 13, which is far away from the pin 3.
The cartridge assembly for a stapler described above has at least an open state and a closed state, in which the first blocking portion 21 is inserted in the first recess 14; in the opened state, the first stopper 21 is separated from the first recess 14, and the lower end of the first stopper 21 is lower than or equal to the upper end surface of the first sidewall 12. When the cartridge component for the anastomat is completely opened, the maximum included angle A between the anvil 1 and the cartridge rack 2 can reach 20.5 degrees.
As shown in fig. 3, the anvil 1 comprises an anvil body 11 having a forming groove 16, the anvil body 11 extending substantially in a horizontal direction, and the forming groove 16 formed on an upper end surface thereof. The anvil body 11 has a first side wall 12 and a second side wall 13 extending upward toward the cartridge magazine 2 at both sides at the right end thereof, and the first side wall 12 and the second side wall 13 are disposed opposite to each other. In a closed state, the right end of the nail bin frame 2 is positioned between the first side wall 12 and the second side wall 13, and two sides of the right end of the nail bin frame 2 are respectively clung to the first side wall 12 and the second side wall 13, so that the compactness between the nail anvil 1 and the nail bin frame 2 can be improved, and the stability and the reliability of the nail bin assembly during opening and closing are improved. Through holes 5 are formed in the right end portion of the nail bin frame 2, the first side wall 12 and the second side wall 13 in the horizontal direction, pins 3 are inserted into the through holes 5, accordingly, the end portion of the nail bin frame 2 is connected with the first side wall 12 and the second side wall 13, the nail bin frame 2 can rotate around the pins 3, and therefore the nail bin assembly of the anastomat has an opening state and a closing state.
Fig. 7 is a schematic structural view of a magazine frame in a staple cartridge assembly for an anastomat according to an embodiment of the present invention, and as shown in fig. 7, the magazine frame 2 may have a first blocking portion 21 extending downward toward the anvil 1. In another possible embodiment, the cartridge 23 may have a first blocking portion 21 extending toward the anvil 1. In other possible embodiments, both cartridge rack 2 and cartridge 23 may together form a first stop 21 extending downwards in the direction towards anvil 1.
As shown in fig. 5 and 6, the anvil 1 has a first recess 14 capable of accommodating the first blocking portion 21, and when the first blocking portion 21 is accommodated in the first recess 14, the first blocking portion and the second blocking portion are located at one end of the first side wall 12 or the second side wall 13 far away from the pin 3, i.e. at the left end of the first side wall 12 or the second side wall 13. The lower end surface of the nail bin frame 2 is used for installing a nail bin 23 corresponding to the forming groove 16, and the distance between one end of the nail bin 23 and the forming groove close to the pin 3 and the pin 3 is smaller than the distance between one end of the first side wall 12 and the second side wall 13 far away from the pin 3 and the pin 3.
In a possible embodiment, the first recess 14 can be provided at the lower end of the inner side wall of the first side wall 12, at the upper end face of the anvil body 11, or at the position where the anvil body 11 is connected to the first side wall 12. The depth of the first recess 14 may be determined according to the height of the first stopper 21 extending downward. The first recess 14 may be deepest and may be a slot extending downwardly through the anvil body 11 or the first side wall 12, so that the height of the first stop 21 extending downwardly may be maximized. The larger the height of the first blocking part 21 extending downwards is, the larger the included angle between the nail anvil 1 and the nail bin frame 2 is on the premise that no gap is formed between one end of the upper end surface of the first side wall 12 and the second side wall 13 far away from the pin 3 and the lower end surface of the nail bin frame 2 when the nail bin assembly is in the open state. Of course, when the cartridge module is in the closed state, and the first blocking portion 21 is inserted into the first recessed portion 14, the lower end of the first blocking portion 21 does not exceed the lower end surface of the anvil body 11 as much as possible, otherwise, the radial maximum size of the cartridge module at this position may be increased, which is not favorable for the cartridge module to extend into the human body.
In a possible embodiment, fig. 4 is a bottom view of a cartridge assembly for a stapler in a closed state according to an embodiment of the present invention, and as shown in fig. 4 and 7, the cartridge frame 2 may have a second stop portion 22 extending toward the anvil 1. In another possible embodiment, the staple cartridge 23 can have a second stop 22 extending towards the anvil 1. In other possible embodiments, both cartridge rack 2 and cartridge 23 may together form a second stop 22 extending downwards in the direction towards anvil 1. The anvil body 11 has a second recess 15 therein which can receive the second stop 22. When the second blocking portion 22 is received in the second recess 15, both are located at an end of the first side wall 12 or the second side wall 13 remote from the pin 3. The first stopper 21 and the second stopper 22 are located on both sides of the magazine rack 2 or the magazine 23, respectively. The first 14 and second 15 recesses are located on either side of the anvil 1.
Similarly, a second recess 15 can be provided at the lower end of the inner side wall of the second side wall 13 or at the upper end surface of the anvil body 11 or at the position where the anvil body 11 is connected to the second side wall 13. The depth of the second recess 15 may be determined according to the height of the second stopper 22 extending downward. The second recess 15 may be deepest by a slot extending downwardly through the anvil body 11 or the second side wall 13, so that the height of the second stop 22 extending downwardly is maximized.
As shown in FIG. 1, the right end of the nail anvil 1 and/or the nail cartridge holder 2 may be connected with a first rotating head mechanism 6 and a second rotating head mechanism 7 which are oppositely arranged, and the first rotating head mechanism 6 and the second rotating head mechanism 7 respectively control the nail cartridge assembly to rotate left or right.
As shown in FIG. 1, when the cartridge module is in the closed state, the body portions of the anvil 1 and the cartridge magazine 2 are substantially flush with each other, and the size of the forming gap between the left ends of the anvil and the cartridge magazine is substantially fixed by the influence of the tissue. The overlapped parts of the first side wall 12 and the second side wall 13 at the two sides of the anvil 1 and the two sides of the nail bin frame 2 basically have no crack, the first blocking part 21 is inserted into the first concave part 14 to be stored, and the second blocking part 22 is inserted into the second concave part 15 to be stored, so that the compactness is ensured between the upper end surface of the right end of the anvil body 11 and the lower end surface of the right end of the nail bin frame 2.
As shown in FIGS. 5 and 6, when the cartridge module is opened, the cartridge holder 2 rotates about the pin 3, so that the first blocking portion 21 is disengaged from the first recess 14 and the second blocking portion 22 is disengaged from the second recess 15. Along with the increasing of the included angle between the magazine rack 2 and the anvil 1, the intersection point 4 of the included angle formed between the lower end surface of the magazine rack 2 and the upper end surfaces of the first side wall 12 and the second side wall 13 of the anvil 1 moves from the left end to the right end of the upper end surfaces of the first side wall 12 and the second side wall 13. In the process, as the cartridge frame 2 is provided with the first blocking part 21 and the second blocking part 22 extending towards the direction of the anvil 1, the positions of the lower ends of the first blocking part 21 and the second blocking part 22 are continuously raised, finally, the lower end of the first blocking part 21 is lower than or equal to the upper end surface of the first side wall 12, and the lower end of the second blocking part 22 is lower than or equal to the upper end surface of the second side wall 13. Thus, even if a gap is formed between the end of the upper end surface of the first side wall 12 and the second side wall 13 far away from the pin 3 and the lower end surface of the cartridge holder 2, the gap is formed where the first blocking portion 21 and the second blocking portion 22 are basically engaged with the end of the upper end surface of the first side wall 12 and the second side wall 13 far away from the pin 3, and thus a blocking effect is achieved. At this time, the intersection point 4 of the included angle formed between the lower end surface of the nail bin frame 2 and the upper end surfaces of the first side wall 12 and the second side wall 13 of the anvil 1 is positioned between the first blocking portion 21 and the pin 3 or between the second blocking portion 22 and the pin 3.
When the cartridge assembly clamps tissue, the tissue is blocked by the first blocking portion 21 and the second blocking portion 22 and cannot enter the gap, so that the problem that the edge of the tissue can be clamped into the gap is avoided. Meanwhile, the included angle between the nail bin frame 2 and the nail anvil 1 can be increased to the extent that the lower end of the first blocking portion 21 is equal to the upper end surface of the first side wall 12, and the lower end of the second blocking portion 22 is equal to the upper end surface of the second side wall 13, so that the included angle between the nail bin frame 2 and the nail anvil 1 can be effectively increased, and due to the influence of the specific specifications of the nail bin 23 and the forming groove 16 in the prior art, the thicknesses of the nail bin frame 2 and the nail anvil 1 are restricted, and must be matched with the nail bin 23 and the forming groove 16 in the prior art, so that when the nail bin assembly for the anastomat in the application is completely opened, the maximum included angle A between the nail anvil 1 and the nail bin frame 2 can reach 20.5 degrees.
There is also provided in the present application a stapler including a cartridge assembly for a stapler as claimed in any one of the above. The nail bin assembly and the anastomat in the application can enable the opening angle between the nail anvil 1 and the nail bin frame 2 to be increased under the opening state on the premise of not increasing the cost and adding any other parts, the anastomat is more convenient to use, and meanwhile the problem that the edge of tissue can be possibly clamped into a gap between one end of the upper end face of the first side wall 12 and the second side wall 13, which is far away from the pin 3, and the lower end face of the nail bin frame 2 can be avoided.
