本发明涉及一种治疗牙周病的中药制剂、制备方法及应用。本发明公开的治疗牙周病的中药制剂是以制首乌、生地、甘草、生鳖甲、制萸肉、菟丝子、穿心莲和地丁草为活性成分与药用辅料制成的医学上可接受的口服制剂。本发明在制剂的制备方法中,对部分药材采取了超临界二氧化碳流体萃取技术萃取有效成分,确保了制剂有效成分的含量。本发明制剂配方新颖合理,治疗牙周病疗效确切,为牙周病患者提供了一种可靠的治疗药物。

技术领域
本发明涉及中药制剂及制备方法,具体涉及一种治疗牙周病的中药制剂、制备方法及应用。
背景技术
牙周病是人类常见多发病,全世界所有国家,均无法避免。我国35岁以上成年人发病率高达98%。至今,在世界范围内仍无理想的特效药品。
中国中医学认为,“肾主骨,骨之余为齿”。当人体肾阴虚损,则骨失濡养,牙齿为之疏豁不固,令齿松动,咀嚼无力;肾虚则胃热,虚火上炎,牙龈红肿疼痛;脾虚,则令人全身功能下降,无力抵御毒邪侵袭,于此,则牙周病频繁发作。
现代医学认为,牙周病病因主要有二个方面,(一)口腔菌群在内外综合因素促动下,产生长消紊乱,失去相对组比的平衡,主要致病需氧菌和厌氧菌迅速大量繁殖,菌斑大量堆积,产生大量有毒代谢物和细菌内毒素,对牙周组织造成感染;(二)人体自身免疫功能低下,失调,使患者自身无法抑制主要致病菌对牙周组织的损伤,出现免疫变态反应和牙周病炎症的各种症状。
发明内容
本发明所要解决的技术问题是结合中医学和现代医学理论提供一种配伍科学合理、标本兼治,疗效显著的牙周病治疗药物。
本发明公开的治疗牙周病的中药制剂是以制首乌、生地、甘草、生鳖甲、制萸肉、菟丝子、穿心莲和地丁草为活性成份与药用辅料制成的医学上可接受的口服制剂,其中活性成份重量百分比配比为制首乌18-25%,生地11-18%,甘草3-7%,生鳖甲11-17%,制萸肉7-15%,菟丝子4-11%,穿心莲4-12%,地丁草8-15%。
本发明所述的口服制剂是指:硬胶囊剂、软胶囊剂、片剂、丸剂、颗粒剂、口服液等。
本发明所要解决的另一技术问题是采用现代提取技术公开上述治疗牙周病的中药制剂的制备方法。
本发明公开的治疗牙周病的口服制剂的制备方法包括下列步骤:
一、穿心莲有效成份萃取物制备
按处方量将穿心莲叶干燥,粉碎,过二十目筛,置萃取釜,密闭;压力18-32Mpa;温度28℃-45℃;CO2流量800ml/h-1250ml/h;用70%-99%乙醇,循环量80ml/h-125ml/h;经2-4h萃取后;减压,取出萃取物,另置;药渣干燥粉碎成220目-280目极粉,另置,待用;
二、生地、生鳖甲、制首乌、菟丝子、制萸肉有效成份萃取物制备
分别按处方量取生地、生鳖甲、制首乌、菟丝子、制萸肉均和,粉碎过40目,置一级萃取釜,压力Mpa8-18;温度52℃-70℃;65%-85%乙醇循环量100ml/h-150ml/h;CO2流量1000ml/h-1500ml/h;经2h-4h萃取后;减压8Mpa/-15Mpa,温度45℃-55℃,流入分离柱,再进入解析釜,减压至正常压后,取出萃取物,另置;药渣,收集待用;
三、制剂活性成份制备
按处方量将甘草,地丁草粉碎20目,加入步骤二的药渣,置装有冷凝器的反应锅内,分别加入6倍量水,4倍量水,2.5倍量水煎煮三次;收集冷凝器挥发性油水混合物,弃渣,和匀三次煎汁,过滤,滤液内加入油水混合物,搅和;减压浓缩至比重1-1.25稠膏,加入步骤一萃取物和步骤二萃取物,混合均匀,低温干燥,粉碎成220目-280目极细粉;极细粉内加入步骤一制得的穿心莲极细粉,搅拌均匀即得;
四、胶囊剂、片剂、颗粒剂、丸剂的制备
取上述步骤三制得的活性成份药粉与药用辅料配伍,用常规方法制得胶囊剂、片剂、颗粒剂、丸剂等;
五、口服液制备
按处方量将穿心莲粉碎20目,取10倍量乙醇(70%-99%,10℃-25℃)密闭浸泡24h分离醇浸液,回流法回收乙醇,余液另置;药渣另置,待用;
按处方量将甘草、地丁草及生地、生鳖甲、制首乌、菟丝子、制萸肉烘干粉碎20目,加入穿心莲药渣,投入带有冷凝装置的反应锅分别加入8倍量水,6倍量水,2倍量水,三次煎沸,每次1.5h收取冷凝器的挥发性油水混合物,弃渣混合三次煎汁,过滤,将穿心莲醇提取物加入滤液内,滤液减压浓缩,至60℃浓度为比重1.06-1.10,冷却至10-25℃,加入挥发性油水混合物再加矫味剂和防腐剂适量,即得。
本发明所要解决的技术问题是公开上述中药制剂在制备治疗牙周病药物中的应用。
本发明制剂可用于由牙周病引起的牙龈红肿,出血、化脓、牙齿松浮、疼痛,无法咀嚼及发烧,头痛等症的治疗。
治疗剂量为每日3次,每次3片,每片含原药6.0-8.0mg,2-7天为一疗程。
“齿者,骨之所终,髓之所养,肾实主之,故肾衰则齿豁,精盛,则齿坚。《成都口腔医院.屈强》,(《四川中医》11卷第二期P.46)。目前,治疗牙周病以清热类药为多。清热类药“不但有抑杀细菌,病毒,钩端螺旋体,原虫,真菌等作用,还有中和病毒……,改善机体反应性”;其主要有以下几个方面作用:“1、抗病原微生物;2、抗细菌毒素;3、解热4、抗炎5、对免疫功能影响;清热药还可增强垂体——肾上腺皮质功能”。(《中国中医药发展五十年》1999年出版,孟庆云.P119-120)。
本发明制剂针对传统清热剂在治疗牙周病中的不足,在同一制剂中,注重清热、消炎、凉血、解毒的同时,兼顾了滋养肝肾,补益脾胃。在提高抑杀致病菌的同时,提高了人体对致病菌感染的免疫力,抑制变态反应。对人体整体虚实全面平衡,短期控制,改善牙周病症状。即:肾主骨,骨之余为齿,肾阴虚损,则骨失濡养,牙齿为之疏豁松动,咀嚼无力;肾亏虚则胃热,虚火上炎,则牙龈红肿、疼痛;脾虚,则正消邪长,全身功能降低,牙周病频繁发作。首乌,生地专入肝肾,补养真阴,救孤阳元烈之象;菟丝子是脾、肝、肾三经补益要药,虚者可补,实者可利,寒者可温,湿者可燥,燥者可润,脾健肝常,肾气当固,肾阴当守,此三味,为君药。生鳖甲,制萸肉,潜敛真阳,止痛,除热,消烦,壮筋骨,是以为臣。地丁草,穿心莲,凉血消肿,清热解毒,此二味为佐。甘草,补脾益气,缓急止痛,清热,解毒,调和诸药,此为使者。诸药协力,则滋肾阴,养肝血,健脾土,清泄虚实邪火,利肝、凉血,标本兼治,相得益彰,使牙周病诸症,迎刃而解。
传统的中药复方制剂的制备方法,采用水提法和醇提法,时间长(6天左右),温度高(100℃),使有效成分中对热不稳定的物质在提取过程易被氧化,分解,聚合,及异构化,而且提取物杂质较多。有许多分子量大的有效成分,因沸点高,挥发度小,采用水提法和醇提法无法得到,既造成能耗大,又使有效成分损失,总的得率偏低,药材资源浪费,影响制剂应有疗效。
本发明制备方法中,对部分药材采取了超临界二氧化碳流体萃取技术,萃取温度低(25℃-45℃左右),时间短(2-5小时),能耗省,无有机溶剂残留,质量稳定。由于超临界二氧化碳流体对部分脂溶性成分具有较强溶解力,高渗透性和高传质速率,无萃取过程中有效成分损失,且萃取过程中始终处于溶解度非饱和状态,避免有效成分发生分解、氧化、聚合、异构化等不良现象发生,能最大程度地保持各组分的原有特性,萃取物香气自然。同时在高压作用下,对许多分子大,沸点高,挥发度小的有效物质,超临界二氧化碳流体能更为有效而完整地抽提出来,使得率平均高于传统提取法的3.5-4倍,如植物多糖和植物内酯等,确保制剂应有的效能,提高了药品的疗效。
本发明还结合使用固体流浸膏超细粉碎(细度大于250目),改进了人的吸收能力,提高了药物的生物利用度。
《牛黄解毒片》是治疗牙周病有针对性的品种,在临床应用中覆盖面积很大,是公认的清热消炎良剂。
本发明以《牛黄解毒片》为对照组,对本发明制剂进行有关临床验证:
试验组:本发明制剂,规格700mg/片,剂型:薄膜异型片。
对照组:牛黄解毒片,600mg/片
给药剂量:700mg*2粒/次,3次/日
给药时间:7天;排除服用其他任何药品;完全一致。
临床症状:1、牙龈红肿;2、牙龈疼痛;3、牙龈出血;4、牙齿松动;5、咬唇疼痛。
牙周病指数:牙龈指数(G1);龈沟出血指数(SB1);牙周袋深度(PD);牙齿松动度(MD)。
试验结果:
1、给药前、后,实验组G1、SB1、PD、MD均有高度显著性(改善)差异(P<0.01);对照组G1、SB1有显著(改善)差异(P<0.01),而PD、MD均无明显差异(P>0.05)。

试验结果显示,本发明制剂在2-7天内能有效控制牙周病,疗效明显好于牛黄解毒片。
本发明治疗牙周病的中药制剂在清热、消炎、凉血、解毒的同时,兼顾了滋养肝肾,补益脾胃,在提高抑杀致病菌的同时,提高了人体对致病菌感染的免疫力,标本兼治,疗效显著。为牙周病的治疗提供了一种疗效确切、服用方便的新型药物。
具体实施方式:
实施例1、制剂活性成分制备
(1)穿心莲叶干燥,粉碎,过二十目,置萃取釜,密闭;压力19.8Mpa;温度35℃;CO2流量820ml/h;用78%乙醇,循环量82ml/h;经4h后;减压,取出萃取物。另置;药渣干燥粉碎成220目极细粉,另置,待用。
(2)生地、生鳖甲、制首乌、菟丝子、制萸肉均和,粉碎过40目,置一级萃取釜,压力10Mpa;温度55℃;78%乙醇循环量108ml/h;CO2流量1180ml/h;经3h后;减压10.8Mpa,温度48℃,流入分离柱,再进入解析釜,减压至正常压后,取出萃取物,另置;药渣,收集待用;
(3)甘草,地丁草粉碎20目,加入步骤(2)中的药渣,置装有冷凝器的反应锅分别加入6倍量水,4倍量水,2.5倍量水分别煎煮三次;收集冷凝器挥发性油水混合物,弃渣,和匀三次煎汁,过滤,滤液内加入油水混合物,搅和。减压浓缩至比重1.25稠膏,加入步骤(1)中的穿心莲萃取物,步骤(2)中生地、生鳖甲、制首乌、菟丝子、制萸肉混合萃取物,低温干燥,粉碎成220目极细粉;极细粉内加入步骤(1)穿心莲极细粉,搅拌均匀。
实施例2、糖衣片制备
配方:总原药材量为6300g
制首乌20%、生地15%、甘草5%、生鳖甲15%、制萸肉15%、菟丝子11%、穿心莲8%、地丁草11%
辅料适量,制成700mg/片,1000片
取上述配方,按实施例1方法制得制剂活性成分,压片,热糖粉为衣,制成片重700mg片剂。
实施例3、胶囊制备
配方:总原药材量为6300g
制首乌22%、生地15%、甘草5%、生鳖甲13%、制萸肉13%、菟丝子13%、穿心莲10%、地丁草9%
辅料适量,制成350mg/粒,共2000粒
取上述配方,按实施例1方法制得制剂活性成分,灌装入胶壳中即得。
实施例4、糖衣丸制备
配方:总原药材量为7000g,共制8000粒
制首乌20%、生地14%、甘草8%、生鳖甲14%、制萸肉14%、菟丝子10%、穿心莲10%、地丁草10%
辅料适量
(1)穿心莲叶干燥,粉碎,过二十目,置萃取釜,密闭;压力19.8Mpa;温度35℃;CO2流量820ml/h;用78%乙醇,循环量82ml/h;经4h后,减压,取出萃取物。另置,药渣干燥粉碎成220目极细粉,另置,待用。
(2)生地、生鳖甲、制首乌、菟丝子、制萸肉均和,粉碎过40目,置一级萃取釜,压力10Mpa;温度55℃;78%乙醇循环量108ml/h;CO2流量1180ml/h;经3h后;减压10.8Mpa,温度48℃,流入分离柱,再进入解析釜,减压至正常后,取出萃取物,另置;药渣,收集待用;
(3)甘草,地丁草粉碎20目,加入(2)中药渣,置装有冷凝器的反应锅分别加入6倍量水,4倍量水,2.5倍量水分别煎煮三次;收集冷凝器挥发性油水混合物,弃渣,和匀收集三次煎汁,过滤,滤液内加入油水混合物,搅和。减压浓缩至1.25稠膏,加入(1)中穿心莲萃取物,(2)中生地、生鳖甲、制首乌、菟丝子、制萸肉混合萃取物,低温干燥,粉碎成220目极细粉;并和入(1)中穿心莲极细粉,机制丸,每8粒重700mg之丸剂。
实施例5、口服液制备
配方 总原药材量为2850Kg,制成1000瓶
制首乌24%、生地18%、甘草5%、生鳖甲12%、制萸肉12%、菟丝子10%、穿心莲5%、地丁草14%
辅料适量,制成500ml/瓶。
按处方量将穿心莲粉碎20目,取10倍量乙醇(78%,25℃)浸泡密闭浸泡24h分离醇浸液,回流法回收乙醇,余液另置;药渣另置,待用;
按处方量得甘草、地丁草及生地、生鳖甲、制首乌、菟丝子、制萸肉烘干粉碎20目,加入穿心莲药渣,投入带有冷凝装置的反应锅分别加入8倍量水,6倍量水,2倍量水,三次煎沸,每次1.5h收取冷凝器的挥发性油水混合物,弃渣混合三次煎汁,过滤,将穿心莲醇提取物加入滤液内,滤液减压浓缩,至60℃浓度为1.06,冷却至25℃,加入挥发性油水混合物再加矫味剂和防腐剂适量,装瓶即得。
实施例6、干糖浆(颗粒)剂制备
配方:总原药材量72Kg,制1000袋
制首乌24%、生地12%、甘草5%、生鳖甲14%、制萸肉14%、菟丝子11%、穿心莲8%、地丁草12%
辅料适量,制成15g/袋,颗粒剂。
(1)穿心莲叶干燥,粉碎,过二十目,置50L萃取釜,密闭;压力19.8Mpa;温度35℃;CO2流量820ml/h;用78%乙醇,循环量82ml/h;经4h后;减压,取出萃取物。另置;药渣干燥粉碎成220目-280目极粉,另置,待用。
(2)生地、生鳖甲、制首乌、菟丝子、制萸肉均和,粉碎过40目,置一级萃取釜,压力,流入分离柱,再进入解析釜,压力10Mpa;温度55℃;78%乙醇循环量108ml/h;CO2流量1180ml/h;经3h后;减压10.8Mpa,温度48℃,减压至正常压后,取出萃取物,另置;药渣,收集待用;
(3)甘草,地丁草粉碎20目,加入(2)中药渣,置装有冷凝器的反应锅分别加入6倍量水,4倍量水,2.5倍量水分别煎煮三次;收集冷凝器挥发性油水混合物,弃渣,收集三次煎汁,过滤,滤液内加入油水混合物,搅和。减压浓缩至1.25稠膏,加入(1)和(2)中萃取物,低温干燥,粉碎成220目极细粉;极细粉内加入穿心莲极细粉,搅拌均匀;制颗粒,烘干,装15g/袋,即得。
实施例7、软胶囊制备
配方:总原药材31.5Kg,制2000粒
制首乌20%、生地14%、甘草5%、生鳖甲14%、制萸肉14%、菟丝子9%、穿心莲10%、地丁草14%
辅料适量,制成2500mg/粒,软胶囊剂。
(1)穿心莲叶干燥,粉碎,过二十目,置萃取釜,密闭;压力19.8Mpa;温度35℃;CO2流量820ml/h;用78%乙醇,循环量82ml/h;经4h后;减压,取出萃取物。另置;药渣干燥粉碎成220目极细粉,另置,待用。
(2)生地、生鳖甲、制首乌、菟丝子、制萸肉均和,粉碎过40目,置一级萃取釜,压力压力10Mpa;温度55℃;78%乙醇循环量108ml/h;CO2流量1180ml/h;经3h后;减压10.8Mpa,温度48℃,流入分离柱,再进入解析釜,减压至正常压后,取出萃取物,另置;药渣,收集待用;
(3)甘草,地丁草粉碎20目,加入(2)中药渣,置装有冷凝器的反应锅分别加入6倍量水,4倍量水,2.5倍量水分别煎煮三次;收集冷凝器挥发性油水混合物,弃渣,收集三次煎汁,过滤,滤液内加入油水混合物,搅和。减压浓缩至1.25稠膏,加入(1)和(2)萃取物,低温干燥,粉碎成220目极细粉;加入挥发性油水混合物,加入精制药用油搅均;加入混悬稳定剂,配制成混悬状药液;
(4)按常规明胶,甘油,负离子水,药用色素搅拌均匀,使溶解度一致的混合胶液;
(5)将胶液和混悬状药液按比例配制成油胶液包裹得软丸,每丸内含物重700mg,状软丸置40℃烘干,用无水溶剂洗涤,即得。
Claims (4)
1、一种治疗牙周病的中药制剂,其特征在于该制剂是按重量百分比由原料药制首乌18-25%,生地11-18%,甘草3-7%,生鳖甲11-17%,制萸肉7-15%,菟丝子4一11%,穿心莲4-12%,地丁草8-15%与药用辅料制成的口服制剂。
2、一种如权利要求1所述的治疗牙周病的中药制剂,其特征在于其中所述的口服制剂是指硬胶囊剂、软胶囊剂、片剂、丸剂、颗粒剂和口服液。
3、一种如权利要求1所述的治疗牙周病的中药制剂的制备方法,其特征在于该方法包括下列步骤:
一、穿心莲有效成份萃取物制备
按组方用量将穿心莲叶干燥,粉碎,过二十目筛,置萃取釜,密闭;压力18-32Mpa;温度28℃-45℃;CO2流量800ml/h-1250ml/h;用70%-99%乙醇,循环量80ml/h-125ml/h;经2-4h萃取后;减压,取出萃取物,另置;药渣干燥粉碎成220目-280目极细粉,另置,待用;
二、生地、生鳖甲、制首乌、菟丝子、制萸肉有效成份萃取物制备
分别按组方用量取生地、生鳖甲、制首乌、菟丝子、制萸肉均和,粉碎过40目,置一级萃取釜,压力Mpa8-18;温度52℃-70℃;65%-85%乙醇循环量100ml/h-150ml/h;CO2流量1000ml/h-1500ml/h;经2h-4h萃取后;减压8Mpa/-15Mpa,温度45℃-55℃,流入分离柱,再进入解析釜,减压至正常压后,取出萃取物,另置;药渣,收集待用;
三、制剂活性成份制备
按组方用量将甘草,地丁草粉碎20目,加入步骤二的药渣,置装有冷凝器的反应锅内,分别加入6倍量水,4倍量水,2.5倍量水煎煮三次;收集冷凝器挥发性油水混合物,弃渣,和匀三次煎汁,过滤,滤液内加入油水混合物,搅和;减压浓缩至比重为1-1.25稠膏,加入步骤一萃取物和步骤二萃取物,混合均匀,低温干燥,粉碎成220目-280目极细粉;极细粉内加入步骤一制得的穿心莲极细粉,搅拌均匀即得;
四、胶囊剂、片剂、颗粒剂、丸剂的制备
取上述步骤三制得的活性成份药粉与药用辅料配伍,用常规方法制得胶囊剂、片剂、颗粒剂或丸剂;
五、口服液制备
按组方用量将穿心莲粉碎20目,取10倍量70%-99%乙醇,10℃-25℃密闭浸泡24h分离醇浸液,回流法回收乙醇,余液另置;药渣另置,待用;
按组方用量将甘草、地丁草及生地、生鳖甲、制首乌、菟丝子、制萸肉烘干粉碎20目,加入穿心莲药渣,投入带有冷凝装置的反应锅分别加入8倍量水,6倍量水,2倍量水,三次煎沸,每次1.5h收取冷凝器的挥发性油水混合物,弃渣混合三次煎汁,过滤,将穿心莲醇提取物加入滤液内,滤液减压浓缩,至60℃浓度为1.06-1.10,冷却至10-25℃,加入挥发性油水混合物再加矫味剂和防腐剂适量,即得。
4、一种如权利要求1所述的中药制剂在制备治疗牙周病药物中的应用。
