Abstract

Technical Field
The invention belongs to the field of traditional Chinese medicine preparations, and particularly relates to a traditional Chinese medicine composition and tincture for treating intractable neurodermatitis and a preparation method thereof.
Background
Neurodermatitis, also called as chronic simple lichen, is a chronic functional disorder of skin characterized by paroxysmal pruritus and lichen, and is very painful in severe cases with flaky and thick papules and moss-like changes. The symptoms are mild and severe, and the disease is easy to relapse after being cured.

Kuh-seng, bitter in taste and cold in nature, is externally used for killing parasites and relieving itching, and is used for treating eczema, mange, leprosy and other symptoms; kurarinone has anti-HBV and anti-HCV effects; the alkali has effects of inhibiting proliferation of liver cancer cell strain and killing it directly; the ethyl acetate extract of radix Sophorae Flavescentis has effect in inhibiting animal and plant pathogenic bacteria; the matrine has antibacterial, virus inhibiting and interferon inducing effects;
rhubarb, radix et rhizoma Rhei, bitter in flavor and cold in nature, has the effects of removing food retention, clearing damp-heat, purging fire, cooling blood, removing blood stasis and detoxifying, and can be used for treating pyocutaneous disease and pyogenic infections by external application.
Borneol, being pungent, bitter and cool in nature and flavor, is externally used for sore and swelling disease without healing after ulceration, and has the functions of clearing away heat and toxic material, preventing corrosion and promoting granulation.
Salicylic acid has the functions of sterilization, inflammation diminishing and cutin removing, and is a common component of chemical external drugs.
Menthol, which is used as an irritant in medicine, acts on the skin or mucous membrane to cool and relieve itching.

1 clinical data
1.1 general data
On the premise of informed consent, 90 cases of intractable neurodermatitis patients, 51 cases of men and 39 cases of women, which meet the Tunne standard and exclusion standard in the XX hospital during the 9 th month to the 12 th month in 2019; the ages were 20-76 years, with the average (38.26 s 4.17) year. The Chinese herbal medicines are numbered in turn according to the sequence of the people's houses and are randomly divided into three groups of 30 cases. The general data of each group, namely sex, age, course of disease and difference of score of major symptoms, have no statistical significance P >0.05) and are comparable.
1.2 methods
The first treatment group: the traditional Chinese medicine formula tincture (the traditional Chinese medicine composition tincture for treating intractable neurodermatitis) is used for treatment, and is externally applied to an affected part for 4 weeks, wherein the traditional Chinese medicine formula tincture comprises the following raw material medicines, by mass, 50g of radix euphorbiae lantu, 40g of radix sophorae flavescentis, 40g of rheum officinale, 12g of cantharis whole worm, 25g of borneol, 50g of salicylic acid, 4g of menthol and 1200ml of 75% ethanol.

The control group is treated by Tacrolimus ointment (Astellas Toyama Co., Ltd. Toyama Plant, Chinese medicine standard J20100016), applied to affected part once in the morning and at night for 4 weeks, and simultaneously self-enhanced disease management is carried out, skin protection is well carried out, and solarization is avoided.
Three groups of patients were cured and followed for 3 months and observed for relapse rates.
3 analysis of therapeutic effects
3.1 Observation index
The main symptoms and signs including pruritus, pimple and moss before treatment, during treatment and after stopping the drug for 2 weeks are observed and recorded according to 0-3 grade scoring method from none, light, medium and heavy. 0 min to 4 min. ② the cure rate, the total effective rate and the adverse reaction of each group of patients after stopping taking the medicine for 2 weeks are compared. The pruritus, the pimple and the moss disappear completely;
the pruritus, the pimple and the moss basically disappear, and the integral drop index is more than equal 60 percent;
the improvement is that the itch, the pimple and the moss basically disappear and relieve, and the integral reduction index is more than or equal to 30 percent and less than 60 percent;
the skin itch, pimple and moss basically disappear, are not relieved or even aggravated, and the integral drop index is less than 30 percent.
3.3 statistical methods

3.4 therapeutic results
All 90 patients in the group are successfully tested and treated, and no obvious adverse reaction occurs in three groups of patients. 3.4.1 the cure rate and total effective rate of three groups of patients after stopping taking the medicine for 2 weeks are compared (see Table 1)
The results show that the cure rates of the first treatment group, the second treatment group and the control group are respectively 66.67%, 70.00% and 50.00%, the total effective rates are respectively 100%, 100% and 83.33%, and the clinical curative effects of the first treatment group and the second treatment group are better than those of the control group. However, the difference between the first treatment group and the second treatment group was not statistically significant. The clinical efficacy of the first treatment group and the second treatment group was superior to that of the control group, but the difference between the A treatment group and the second treatment group was not statistically significant.

3.4.2 comparison of healing time of rash, disappearance time of symptom and recurrence rate of follow-up 3 months for each group of patients (see Table 2)
The healing time of the rash, the symptom disappearance time and the recurrence rate of the patients cured in the first treatment group and the second treatment group are respectively lower than those of the control group, the differences have statistical significance (P is less than 0.05), and the differences between the first treatment group and the second treatment group by the soil indexes have no statistical significance. The three groups of relapsing patients still have effective treatment by simply using traditional Chinese medicine external washing, while the traditional Chinese medicine treatment curative effect of the first treatment group and the second treatment group of relapsing patients is obviously better than that of a control group, but the re-grouping contrast study cannot be carried out because the total sample amount of the relapsing patients is less.

At present, western medicines still have a plurality of defects in the aspect of antifungal treatment, and researches show that the traditional Chinese medicine has unique curative effects in the aspect of intractable neurodermatitis, no matter the traditional Chinese medicine is a single medicine or an effective component of the traditional Chinese medicine, or a compound preparation. The treatment method comprises external treatment, internal treatment and combination treatment, and the simple external treatment is suitable for patients with initial onset and limited focus.
The data show that the traditional Chinese medicine formula tincture of the first treatment group (the traditional Chinese medicine composition tincture for treating intractable neurodermatitis comprises the following bulk drugs of, by mass, 50g of radix euphorbiae lantu, 40g of radix sophorae flavescentis, 40g of rheum officinale, 12g of whole cantharis, 25g of borneol, 50g of salicylic acid, 4g of menthol and 1200ml of 75% ethanol) and the traditional Chinese medicine formula tincture of the second treatment group (the traditional Chinese medicine composition tincture for treating intractable neurodermatitis comprises, by mass, 50g of radix euphorbiae lantu, 40g of radix sophorae flavescentis, 40g of rheum officinale, 12g of whole cantharis, 25g of borneol, 50g of salicylic acid, 4g of menthol, 1g of ivy, 1g of fructus kochiae, 1g of salvia miltiorrhiza, 1g of angelica, 1g of erigeron breviscapus, 1g of bergamot, 1g of aloe, 1g of rosemary and 1200ml of 75% ethanol) are fully combined according to the pharmacodynamics characteristics of each component, the antibacterial radix euphorbiae lantu, the lightyellow sophora root which is bitter in taste and cold in nature and can kill parasites and relieve itching, and the rhubarb which has the effects of purging intense heat, cooling blood, removing stasis and detoxifying are adopted, the cytotoxicity and the bubbling characteristics of cantharidin are utilized, the cutin removing and inflammation diminishing characteristics of salicylic acid are combined, thick scabs of intractable neurodermatitis can be fully removed, the concentration of cantharidin is well controlled, the irritation degree is low, and the menthol which has the effects of cooling and relieving itching is used as an auxiliary material, so that the affected part feels cool and comfortable after being applied. The used medicine has good effects of inhibiting and removing skin lichenification, and has the effects of relieving itching, diminishing inflammation, removing blood stasis, detoxifying and the like, and the medicines are used together to directly reach the pathological change part, thereby fully playing the curative effect.
