Abstract
The invention discloses a composition, which belongs to the technical field of medicines and is prepared from fat particles and acellular dermal matrix. The invention also provides the application of the composition in preparing the implant with the capacity of the soft tissue of the body being deficient or insufficient. Its advantages are: because the collagen scaffold structure formed by the acellular dermal matrix can effectively promote the growth of capillary vessels and fibroblasts of an organism, the nutrition supply of the implanted autologous fat cells is improved, and the survival rate of fat is effectively improved. Meanwhile, a large amount of fibroblasts are generated, so that the compactness and compactness of the filling part are superior to those of pure adipose tissue filling.
Description
Technical Field
The invention relates to the technical field of medicines, in particular to a composition and application thereof.
Background
With the improvement of political, economic and social status and the improvement of living standard of women, the plumpness and the curve beauty of women are more and more emphasized, and the plumpness and the body building of the face, the hip, the breast and other parts are more and more concerned. Adipose tissue transplantation, which is to transplant adipose particle tissues after lipoaspirate into a body to repair tissue depressions and asymmetric limbs caused by diseases, aging, trauma or congenital dysplasia, and is widely applied to operations such as breast enlargement, hip enlargement, filling of face and vagina and the like. However, the survival rate of the granular fat transplantation is low, generally 20% -30%, which causes the problems of multiple transplants, fat liquefaction, necrosis and the like, and seriously restricts the development of the technology.
Acellular Dermal Matrix (ADM) is an acellular tissue material made of animal or human skin that removes all the cellular components of the full epidermis and dermis layers of the skin, but retains the collagen component and tissue infrastructure of the dermis, while retaining the basement membrane component. According to different sources, the acellular dermal matrix can be divided into a variant acellular dermal matrix and a variant acellular dermal matrix, wherein the variant dermal is mainly derived from cadaver skin, and the variant dermal is derived from pigskin, cow leather and the like.
Disclosure of Invention
The invention provides a composition and application thereof.
The invention adopts the following technical scheme:
a composition made from fat particles and an acellular dermal matrix.
Further, the ratio of the fat particles to the acellular dermal matrix is 100 ml: 1-50 g.
Further, the ratio of the fat particles to the acellular dermal matrix is 100 ml: 8-12 g.
Further, the acellular dermal matrix is a xenogenic acellular dermal matrix or a xenogenic acellular dermal matrix.
Further, the fat particles are human autologous fat particles.
The use of the above composition for the preparation of an implant for the loss of volume or lack of volume of soft tissue.
Further, the implant is used for the face, chest, buttocks or vagina.
Further, the implant is administered to the human body by injection.
The invention has the following beneficial effects:
because the collagen scaffold structure formed by the acellular dermal matrix can effectively promote the growth of capillary vessels and fibroblasts of an organism, the nutrition supply of the implanted autologous fat cells is improved, and the survival rate of fat is effectively improved. Meanwhile, a large amount of fibroblasts are generated, so that the compactness and compactness of the filling part are superior to those of pure adipose tissue filling.
Detailed Description
In order to make the objects, technical solutions and advantages of the present application more apparent, the technical solutions of the present application will be described in detail and completely with reference to the specific embodiments of the present application. It should be apparent that the described embodiments are only some of the embodiments of the present application, and not all of the embodiments. All other embodiments, which can be derived by a person skilled in the art from the embodiments given herein without making any creative effort, shall fall within the protection scope of the present application.
Example 1
1. Clinical data
39 patients, all female, aged 34-56 years. Among them, 8 cases of breast augmentation, 5 cases of hip augmentation, 8 cases of vagina and 18 cases of facial filling.
2. Surgical method
2.1 liposuction
The patients' body positions are arranged according to the suction position, and the operative field is disinfected by 0.5 percent iodine tincture and 75 percent alcohol.
The suction part is injected with swelling solution (0.9% physiological saline 1500-. The local anesthetic is injected for 20-30 minutes and then fat aspiration is carried out.
The suction with small range is performed by using a syringe, and the suction device is a disposable 20ml plastic syringe and a single-hole or multi-hole 8-20-gauge needle. A larger range of suction aids in the use of vacuum aspirators.
Suction begins at the deep layer of the suction area and gradually transitions to the shallow layer.
The needle holes left on the surface of the postoperative skin are covered with small gauze, the part of the postoperative skin is wrapped with cotton pads, and the postoperative skin is fixed by elastic clothes and trousers. Oral administration of cefradine within 3 days after operation prevents the infection of the operative area.
2.2 preparation of hybrid implants
The upper layer of granular fat in the syringe was mixed with ADM (ratio 100 ml: 8-12 g) to obtain an implant.
2.3 injection hybrid implant
Local swelling, infiltration and anesthesia are firstly carried out on a receptor area before injection and transplantation, so that tissues in each layer are hydraulically expanded, loosened and non-invasively separated, the injection is carried out while withdrawing a needle when a tunnel is formed in a single-point micro, multi-point, multi-plane and multi-tunnel manner, the breast injection amount is 180 ml/side, and the hip injection amount is 360 ml/side 220-.
The single injection filling transplantation is finished without repeated injection filling transplantation.
3. Effect of the operation
Postoperative complications such as hematoma, infection, necrosis, calcification, induration, cystic lesion, excessive liposuction, fat embolism, pneumothorax and the like do not exist. The breast augmentation does not change the breast tissue structure, does not interfere with breast imaging, and has satisfactory appearance of breasts and buttocks and good hand feeling. The fat survival rate of the patients is generally considered to be higher than that expected before the operation, and the image examination shows that the fat survival rate reaches more than 85 percent.
After the postoperative follow-up visit of 6-12 months, the deformity of the donor area is avoided, the chest circumference is increased by 3-4cm, the hip circumference is increased by 3-5cm, and the fat survival rate is more than 80% as shown by image examination. Lumps and other structural changes did not appear, and the incidence of fat necrosis was less than 1%.
The above description is only an example of the present application and is not intended to limit the present application. Various modifications and changes may occur to those skilled in the art. Any modification, equivalent replacement, improvement, etc. made within the spirit and principle of the present application should be included in the scope of the claims of the present application.
