Abstract

一种骨水泥通道椎弓根锚钉,所述锚钉为一空心圆柱体,依次包括:前端螺纹段、螺帽段、后端尾套管段;所述前端螺纹段的顶端为尖端部,所述前端螺纹段从前端往尾端的螺纹的螺距逐渐变小;所述前端螺纹段的长度为4.5cm,所述尾套管段的长度为10cm。所述锚钉的材质为金属钛。本发明的中空骨水泥通道椎弓根锚钉,旋入椎弓根内部分的螺钉能牢固稳定持久锚定于椎弓根内,达到增强椎体内骨水泥团块强度、锚定力及稳定性。

Description

一种骨水泥通道椎弓根锚钉
【技术领域】
本发明属于医疗器械耗材技术领域,具体是指一种骨水泥通道椎弓根锚钉。
【背景技术】
随着人类人均寿命延长及人口老龄化,老年性骨质疏松性椎体压缩性骨折发病率逐年增加。据统计,全世界每年约有140万骨质疏松性椎体骨折患者,一般不伴有神经症状。约30%会出现骨质疏松椎体骨折塌陷,而其中13%会表现骨折不愈合,3%会出现塌陷合并迟发神经损伤。针对此类骨折,PVP/PKP是治疗此类骨折安全、微创、疗效肯定的技术。但是,由它所带来的并发症,如骨水泥注入术后出现骨水泥团块松动移位、脱出而导致椎体高度复位丢失、不稳、再疼痛、神经损伤等问题尚无法有效解决。这常见于Kummell病病人,由于属陈旧性骨折,在椎体内形成裂隙空腔、疤痕、硬化、假关节等,从而使骨水泥在椎体内弥散不佳,锚定力严重不足,影响患者中远期疗效。
目前针对此种情况,有人在椎弓根内试图留置骨水泥柱以增强椎体内骨水泥块锚定力及稳定性,防止移位及松动。这种方式先不说能否成功与椎体内骨水泥块牢固结合,即便成功结合,其锚定力、强度及稳定性远远不能满足临床需要(力学稳定性极差)。对于此类骨折病人,常见于老年及高龄患者,自身身体素质条件以及常伴心肺等基础内科疾病,常无法耐受辅助内固定融合等大手术。内固定易导致相关并发症如由于邻近椎体应力集中致再骨折、内固定钉道松动、脱出等问题,因此,给临床治疗上带来很大困扰。如何解决此类问题成为临床骨科医生急需迫切需要解决的课题。
【发明内容】
本发明所要解决的技术问题在于提供一种方便使用的骨水泥通道椎弓根锚钉,达到增强椎体内骨水泥团块强度、锚定力及稳定性。
本发明是这样实现的:
一种骨水泥通道椎弓根锚钉,所述锚钉为一空心圆柱体,所述锚钉为一空心圆柱体,依次包括:前端螺纹段、螺帽段、后端尾套管段;所述前端螺纹段从前端往尾端的螺纹的螺距逐渐变小;所述前端螺纹段的长度为4.5cm,所述尾套管段的长度为10cm。
进一步地,所述锚钉的材质为金属钛。
进一步地,所述锚钉的前端螺纹段的内径为3mm。
进一步地,所述锚钉的前端螺纹段的外径为5.5mm。
进一步地,所述锚钉的前端螺纹段的单边螺深为0.4mm。
进一步地,所述锚钉的前端螺纹段的壁厚为0.8mm。
进一步地,所述前端螺纹段靠近所述螺帽段处的内壁具有一段内螺纹。
本发明的优点在于:本发明的中空骨水泥通道椎弓根锚钉,旋入椎弓根内部分的螺钉能牢固稳定持久锚定于椎弓根内,打入椎体内部分可与椎体内骨水泥块充分牢固结合,如同钢筋混凝土一样,使前面被骨水泥强化椎体与后面椎体附件连成一块。因金属钛质具有较强钢度,从而达到增强椎体内骨水泥团块强度、锚定力及稳定性。有效避免需另外再辅助内固定,对无法耐受大手术患者提供一种解决方法。并可替代目前临床在行PKP/PVP时建立的骨水泥外套管通道系统,不额外增加医疗费用,减轻病人及医保负担,且能明显简化手术操作步骤,从而缩短手术时间。

【具体实施方式】
如图1和图2所示,一种骨水泥通道椎弓根锚钉100,该锚钉100的材质为金属钛制成。锚钉100为一空心圆柱体,依次包括:前端螺纹段101、螺帽段102、后端尾套管段103;前端螺纹段101从前端往尾端的螺纹的螺距逐渐变小;所述前端螺纹段101的长度为4.5cm,所述尾套管段103的长度为10cm。
前端螺纹段101的内径(中空直径)为3mm,外径(包括螺纹)为5.5mm。前端螺纹段101的单边螺深为0.4mm、壁厚(不包括螺纹)为0.8mm。
操作过程:
第一步:先用直径2.5mm穿刺针建立经椎弓根内通道。
第二步:沿穿刺导针经皮旋入骨水泥通道椎弓根锚钉100,旋入至椎体内后1/3至1/2处。
第三步:顺骨水泥通道椎弓根锚钉100用骨钻扩深椎体至椎体前壁(不穿破)。
第四步:将装满骨水泥的推杆顺骨水泥通道椎弓根锚钉100按正常步骤行至椎体内强化至骨水泥渗透快接近椎体后壁停止注射。
第五步:在骨水泥还未凝固之前可根据情况再旋深约5mm-10mm至椎体前1/3左右。
第六步:旋入并锁定内芯(根据术中情况可选用或不用,亦可填充骨水泥替代),最后折断尾套管段103,保留螺帽段102。
本发明的骨水泥通道椎弓根锚钉100不仅可替代在行椎体骨强化时建立的骨水泥通道外套管系统,而且能增强椎体内骨水泥团块锚定力、强度及稳定性(通过锚定于椎体附件椎弓根上),一举两用,特别适合于Kummell病及需行PKP手术病人,其应用前景是非常广阔的。
本发明的中空骨水泥通道椎弓根锚钉,旋入椎弓根内部分的螺钉能牢固稳定持久锚定于椎弓根内,打入椎体内部分可与椎体内骨水泥块充分牢固结合,如同钢筋混凝土一样,使前面被骨水泥强化椎体与后面椎体附件连成一块。因金属钛质具有较强钢度,从而达到增强椎体内骨水泥团块强度、锚定力及稳定性。有效避免需另外再辅助内固定,对无法耐受大手术患者提供一种解决方法。并可替代目前临床在行PKP/PVP时建立的骨水泥外套管通道系统,不额外增加医疗费用,减轻病人及医保负担,且能明显简化手术操作步骤,从而缩短手术时间。

1.一种骨水泥通道椎弓根锚钉,其特征在于:所述锚钉为一空心圆柱体,依次包括:前端螺纹段、螺帽段、后端尾套管段;所述前端螺纹段从前端往尾端的螺纹的螺距逐渐变小;所述前端螺纹段的长度为4.5cm,所述尾套管段的长度为10cm。
