技术领域

背景技术
利用手术窥镜的微创外科手术是理想的，因为它们通常可以提供一个或多个下述的优点：失血减少；手术后患者的不适感减少；恢复和住院的时间缩短；切口更小；以及内部器官更少暴露在可能的污染中。
通常，微创外科手术使用例如腹腔镜的窥镜，当外科手术进行时，窥镜使得患者体内的手术部位实现远程可视化。在腹腔镜手术期间，通过两个或更多个相对小的切口进入患者的腹腔或盆腔，而不是通过典型地在常规外科手术中的一个单独的大切口进入患者的腹腔或盆腔。例如腹腔镜的手术窥镜通常由部分刚性的或相对刚性的棒或杆组成，该棒或杆在其一端具有物镜，而在另一端具有目镜和/或整合的视觉显示器。窥镜也可以连接到远程视觉显示装置或摄像机，以记录手术过程。

发明内容

B鞘套/歧管组件

2.偏转器组件的物理、气动和光学特性
偏转器组件64的尺寸和形状是由几个有时重叠的考虑因素定义和限制的，这些考虑因素包括：(i)规定的物理特性，因为需要以尽可能微创的方式进入手术环境，并且需要与最先进的腹腔镜和其他腹腔镜手术器械和技术相容，这些物理特性是强制性的要求；(ii)规定的气动特性，因为需要在通过腹腔镜镜头的CO2的流动路径和流动速度的方面生成特定的“风切变”效果，这些气动特性是强制性的要求；和(iii)规定的光学特性，因为需要防止干扰通过腹腔镜12的视野和手术区的可视化，这些光学特性是强制性的要求。
3.物理特性
为了微创进入而对尺寸和形状的需求要与最先进的腹腔镜器械和技术相容，这是首要的。这些需求对鞘套14的最小内径和鞘套14的最大外径强加了限制。由于提供的最先进的腹腔镜具有不同的杆直径、长度和镜头形状，鞘套的尺寸和形状为了与其相容而变化。实际上，视野优化组件10包括一套设计成不同尺寸和形状的鞘套14/歧管18组件，以适应不同等级的腹腔镜，使得与所用的那套最先进的腹腔镜相容成为可能。
例如，最先进的腹腔镜包括10mm腹腔镜，5mm腹腔镜，和，在这些尺寸内，0°杆末端，30°杆末端，和45°杆末端。进一步地，在这些等级的腹腔镜内，窥镜和窥镜之间，以及制造商和制造商之间，制造公差通常各不相同。用于给定的腹腔镜等级(例如，10mm或5mm)的给定的鞘套14/歧管18组件理想地要考虑到这些通常的制造和制造商之间的差异，并且理想地将尺寸和形状设成适合在给定的腹腔镜等级中遇到的最大窥镜差异。
为了将鞘套14内的流体流动内腔区域最大化，给定的鞘套14的最小的内径必须紧密符合选择使用的腹腔镜12的特定等级状态的杆的最大外径，鞘套14必须以平滑的，滑动的配合容纳选择使用的腹腔镜12。进一步地，腹腔镜杆的外径和鞘套14内径之间的间隙必须最小化以避免血液和流体从手术区的传输和泄漏。仍进一步地，最小化间隙还可确保腹腔镜12在鞘套14内的自动定心，从而确保通过腹腔镜镜头的如实的和精确的可视化。
例如，对于典型的10mm等级的腹腔镜12，测量值为0.392英寸，鞘套14的内径制造成0.405英寸，从而提供厚度为0.0064英寸的间隙。对于5mm等级的5mm腹腔镜12，测量值为0.196英寸，鞘套14的内径制造成0.218英寸，从而提供厚度为0.011英寸的间隙。 
用于微创进入的鞘套14的最大外径必须考虑套管针的最小内径，最大外径不能超过此最小内径。
例如，典型的10mm套管针，其测量值为0.509英寸，鞘套14的外径制造成0.486英寸，从而提供厚度为0.0115英寸的间隙。典型的5mm套管针，其测量值为0.324英寸，鞘套14的外径制造成0.300英寸，从而提供厚度为0.012英寸的间隙。 
理想的是，在所使用的腹腔镜器械和技术的给定的特定尺寸和形状的限制下，尽可能地最大化外径。这是由于，鞘套14的内径和外径共同限定了鞘套的壁厚度Sw。壁厚度Sw以及鞘套14的长度，相应地，限定了可用于通过鞘套14传输CO2和流体的最大区域。用于供应CO2的流体流动内腔或多个内腔的区域，相应地，限定了通过偏转器组件64引导的CO2的最大流量。流量应该在选用的吹入器的最小给定输出时足够用以供应足以防止起雾的穿过腹腔镜12的镜头的无水CO2。还有会影响CO2对镜头除雾效果的是无水CO2的水含量。给定相同的流量，无水CO2中存在的水越少，则组件的除雾能力越强。进一步地，理想的流量应该也是足以在手术期间使烟和手术碎片偏转，远离腹腔镜的视野，从而由偏转器组件64引导的无水CO2既可以除雾也可以使碎片偏转。
与常规吹入器一起使用的医疗级CO2通常为纯度99％，即，不超过1％的气体不是CO2，并且这样的医疗级无水CO2一般具有以体积记为百万分之25的最大含水量。通常情况下，最先进的吹入器回路传递无水CO2的最大流量约为每小时20升。通常情况下，吹入器回路会感应到回路内的压力并且在感应到的压力在15mmHg或高于15mmHg时关闭循环，在感应到的压力低于15mmHg时开启循环。
给定了上述鞘套尺寸，并给定了典型医疗级无水CO2的供应，至少每分钟约1.0升的流量对于实现这个目标是很重要的。给定了上述尺寸和典型医疗级无水CO2的供应，每分钟小于0.8升的流量则不足以防止腹腔镜镜头上的明显的湿气积聚。
在一个代表性的实施例中，对于具有0.405英寸的内径和0.486英寸的外径，和11.25英寸的长度的鞘套14(其适于供典型的10mm腹腔镜通过，且其自身通过常规的套管针)(例如，Sw＝0.081英寸)，鞘套壁内的可用总面积为0.056平方英寸。基于上述壁内(内，外，和径向的)所需的结构性支持，供内腔传输流体的可用总面积是0.027平方英寸。
在一个代表性的实施例中，所有内腔的区域由5个内腔34-42占据，两个用于 传输CO2(34和36)，一个用于无菌流体(38)，和两个用于被动式排气通风(40和42)。
每个内腔的区域可以通过选择内腔的几何形状以最大化。在一个代表性的实施例中，内腔的几何形状一般为三角形或具有圆角的饼状。将鞘套14内的内腔分隔开的径向壁的尺寸设成使内腔之间的间隔最小化。
在一个代表性的实施例中，CO2的运输是通过两个内腔34和36完成的，内腔34和36以大约175度围绕鞘套14的外圆周延伸并且包括0.013平方英寸的流动面积。无菌流体的运输是通过一个内腔38完成的，这个内腔包括0.003平方英寸的流动面积。排气通风是通过两个内腔40和42完成的，这两个内腔包括0.011平方英寸的流动面积。排气内腔40和42的远端开口理想地与鞘套的远端间隔开，以防止血液和流体的摄取。
4.气动特性
如用图解法在图6中示出的，偏转器组件64必须在腹腔镜镜头上方伸出规定的横向距离，从而限定了偏转宽度X，足以将轴向流动通过鞘套14的内腔(例如，沿着腹腔镜杆的轴线)的CO2的方向变为非轴向地，横过腹腔镜镜头的横向路径(例如，相对于腹腔镜杆的轴线成一定角度)。尽管如此，偏转宽度X的距离不应该延伸到那个会妨碍腹腔镜镜头的视野的点。这是偏转器组件64气动特性与光学特性重叠的一个例子。进一步的光学特性将在下文作更详细的描述。
偏转器组件64还必须轴向伸出以超过鞘套14的远端边界一规定的轴向距离，从而限定了空气通道距离Y，足以维持CO2以足够接近(邻近)腹腔镜镜头的一距离沿着由偏转宽度X划定界限的路径流动，以获得所期望的剪切流的效果，但是不会形成急速的流动弯曲，该弯曲可能会导致期望的CO2流速降低。
偏转宽度X和通道距离Y共同限定了偏转器组件的气动特性。在期望的最小流速下，气动特性生成了流动路径，该流动路径以所期望的流动速度不断地横过腹腔镜镜头输送CO2，简称为“风切变”。CO2“风切变”横过腹腔镜镜头的气动特性防止在手术期间起雾，理想地还使得烟和手术碎片偏转，离开腹腔镜镜头的视野。
由偏转宽度X和通道距离Y限定的气动特性共同产生了出口角度AEXIT，在腹腔镜镜头的平面和偏转器组件64的终端边缘之间测量出口角度AEXIT。出口角度AEXIT必须小于最大为45度的角度，否则CO2的流动路径不足以既横过又邻近腹腔镜镜头。为了维持期望的出口角度AEXIT，通道距离Y应该至少与鞘套的壁厚度Sw相等，并且不应该超过鞘套的壁厚度Sw的1.5倍。偏转宽度X应该至少等于通道宽度Y的两倍，但是不能延伸到腹腔镜镜头的视野内。
5.光学特性
偏转器组件64的光学特性被选择为(i)不阻挡或减少由腹腔镜12所提供的手术区的照明图像；(ii)不降低由手术区上的腹腔镜12所提供的照明强度；和(iii)防止腹腔镜12的镜头上的照明光线的反射。
如上所述，最大的偏转宽度X考虑了理想的光学特性中的一项；也就是说，偏转宽度X不应该妨碍腹腔镜镜头的视野。
为了防止照明减弱，偏转器组件64理想地由具有高透光性能(例如，透明度)的材料制成，从而不会干扰光线通过光缆30到达手术区上的通道，和被反射的图像传输到腹腔镜12的摄像机电缆32的通道。
进一步地，偏转器组件64的材料和表面光洁度必须引起最小程度的光反射。在一个代表性的实施例中，偏转器组件64由拜尔模克隆(Bayer Makrolen)Rx1805制成，拜尔模克隆Rx1805具有限定为SPI/SPE A-3的表面光洁度。
6.方向
如前所述，CO2传输是通过两个内腔34和36完成的，内腔34和36以大约175度围绕鞘套14的外圆周延伸。对于0°的杆末端(见图5A)，偏转器组件64相对于腹腔镜镜头的方位是不那么重要的。然而，对于成角度的杆(例如，30°的杆末端或45°的杆末端)(见图5B)，偏转器组件64相对于腹腔镜镜头的方位是很重要的。
如图5B所示，典型的腹腔镜12的成角度末端具有高端66和低端68。镜头在高端66和低端68之间倾斜成规定的角度。在具有成角度的末端的腹腔镜12内，照明电缆30(将光线传送到手术区上)设置在成角度的末端的高端66，摄像机电缆32(将反射光线传回摄像机)设置在成角度的末端的低端68。为了在成角度的末端上提供理想的风切变效果，偏转器组件64相对于倾斜的腹腔镜镜头定向是很重要的，由此引导CO2流从末端的低端68朝末端的高端66横过镜头的倾斜平面。在这样的布置下，除雾和碎片偏转路径靠近摄像机电缆32而产生，从而有效地构成了手术室团队的眼睛。这样的布置，理想的出口角度AEXIT引导CO2的流动路径足以横过和邻近腹腔镜镜头的倾斜平面，以达到最优的除雾和碎片偏转。
F.无菌流体冲洗 
如前所述，如果需要的话，导管组16也可以包括，与快速更换连接件22连接的，一根尺寸和形状设成用于连接无菌流体的来源72的导管70，如盐溶液或无菌水(如图1A和2A所示)。优选地，无菌流体包括溶解的“表面活性剂”，该表面活性剂通过减少油和水分子之间的接触面的表面张力而稳定油和水的混合物(如脂肪)。
歧管18上的快速更换连接器20(见图3A/3B和4A/4B)还可以包括整体地连接无菌流体导管70的开口，以引导无菌流体通过鞘套14内分隔的内腔38到达鞘套14的远端。偏转器组件64引导无菌流体横过腹腔镜镜头。
如图1A/2A所示，无菌流体导管70，如果存在，理想地包括管道抽吸装置(in-line pumping device)72。管道抽吸装置72的尺寸和形状设成可按手术 室台上个人的需求操作以通过歧管18将多股无菌流体通过内腔输送到达鞘套14的远端。管道抽吸装置72和来源可以是整合在一起的，并包括，例如填充有无菌流体的20cc注射器，并且注射器通过导管卢尔锁(luer-lock)连接到盐溶液导管。可替代地，管道抽吸装置72和来源可以是分开的，并包括，例如，无菌流体的袋子，导管组16的盐溶液导管上为尖物连接从而以传统方式开启与该袋子的连通；和管道挤压球形物或类似物，以从袋中泵出一股无菌流体到快速更换连接件22。
在这种布置中，偏转器组件64的尺寸和形状也设成可以引导一股无菌流体以期望的路径横过腹腔镜镜头。多股无菌流体用于冲洗掉镜头端部的可能最终积聚的碎片，从而清洁镜头。然后，由导管组16内的挤压泵74通过偏转器组件64供应的多股空气(见图1A/2A)用于清除镜头上的残留流体滴并使其离开偏转器组件64，以维持由偏转器组件64建立的不断地横过腹腔镜镜头的CO2的期望的流动路径和流动速度，以维持可接受的视野。
在一个示例性的实施例中(见图5A和5B)，偏转器组件64沿着多条独立的分散通道76(所示的为3个)引导多股的无菌流体或空气。分散通道76以扇动的形式分配多股无菌流体或空气横过腹腔镜12的镜头。在一个示例性的实施例中，分散通道76在与CO2的路径大致成90度的路径上排出多股无菌流体或空气。无菌流体路径相对于CO2路径横过镜头的这个方位，对于有效的镜头清洁是最理想的，既适用于0°的杆末端，也适用于成角度的末端(例如，30°的杆末端和45°的杆末端)。
Ⅱ.视野优化组件的使用
视野优化组件非常适于用作一次性使用的一次性腹腔镜附件装置，以方便微创手术期间的(由于无水CO2流的)术中除雾和碎片偏转和(由于一股无菌流体，优选地包括“表面活性剂”的)腹腔镜12的镜头的清洁，同时还维持手术部位的可视化。
图7-34绘示了一个代表性的方法，包括由有资格的技术人员/手术室人员进行的使用无菌技术的视野优化组件的准备和使用。
该过程可以被纳入随同包装一起的书面使用说明书。说明书也可以是单独提供的，例如，具体体现为单独的指导手册，或视频或音频磁带，CD或DVD。使用说明书也可以通过互联网网页获得。
说明书可以指导手术室准备工作，剥开外袋，外袋内有视野优化组件的部件(如图7所示)，以及移除无菌区上的无菌物。将鞘套14/歧管18组件小心地移除，以防止对鞘套14的壁或它的远端的损坏，并且也将导管组16和通风装置24在进行必要的连接之前保持在无菌区上。
在安装时(参见图8和图9)，鞘套14(具有歧管18，其在制造时整体地连接 到鞘套14，称为鞘套组件)可以组装到相应的腹腔镜12上。在这个代表性的实例中，假设手术室团队计划使用0度的腹腔镜12(见图8和9)和至少一个成角度的腹腔镜12(见图10和11)，例如，30度和/或45度的腹腔镜12。因此，在安装时，用于选择使用的每个腹腔镜12的鞘套组件将会预先组装到相应的腹腔镜12上。
如图8和10所示，当抵着一个手或指尖轻轻地按压鞘套组件的末端时，腹腔镜12可以向下插入鞘套14中。鞘套14的尺寸和形状设成使得腹腔镜12将顺利滑动通过鞘套14。继续插入，直到腹腔镜12的镜头和远端边缘抵着鞘套14的远端端部处的停止件而落座。腹腔镜12将抵着停止件26在鞘套14内“降至最低点(buttom out)”，从而确保镜头和偏转器组件64正确的轴向对准。
如果腹腔镜12是成角度的(如图10所示)，相应的鞘套组件也将会包括对准叉状导向件78。窥镜的灯柱坐落在对准叉状导向件78内，从而确保成角度的镜头和偏转器组件64之间正确的旋转对准。
腹腔镜12(现在完全插入到鞘套14中)歧管18在手的支撑下，由手术室准备团队的成员在期望的方向上旋转鞘套组件上的锁圈28，期望的方向例如为顺时针(见图9和11)，由锁圈28上的箭头指示出，直到感触到牢固的停止(例如，大约旋转1/3后)。锁圈28上的对准标记和歧管18上的对准标记的定位有助于直观地确认腹腔镜12相对于鞘套14被固定而不能轴向移动。
吹入器设置在无菌区外。一旦患者在无菌区上用布帘遮盖，预计将吹入器的输出导管的端部(源于无菌区外的吹入器)带入到无菌区。还预计将用于腹腔镜12的光缆30和摄像机电缆32带入到无菌区。
如图12和13大致所示，手术室团队制造切口以获得进入体内的腹腔镜操作部位的入口，例如，通过腹部壁进入腹腔。通过切口插入具有旋塞阀的第一套管针(其可以是光学套管针的形式)。或者，根据医生的偏好，可以在只有皮肤切口的情况下推动第一套管针通过腹部壁。一旦第一套管针到达它的合适位置，从第一套管针移除填塞器(套管针的锋利的内部插入件)。
将无菌区上的导管组16的吹入器管道连接到无菌区上的吹入器回路的输出导管。将无菌区上的导管组16的第一分支46连接到第一套管针的旋塞阀(见图13)，第一分支46源于Y型连接器44。打开旋塞阀，并开启吹入器。吹入回路的CO2输出通过第一套管针使腹部膨胀。
在这期间(见图8和10)，当吹入器通过第一分支46供应CO2时，无菌区上的导管组16的第二分支48，以及整体地与它附接的快速更换连接件22可以在无菌区上保持自由的、未连接的状态，第二分支48也源自Y型连接器44。快速更换连接件22内的单通道止回阀用于阻止CO2流通过第二分支48，即使 当吹入器通过第一分支46供应CO2时。目前，吹入器回路的整个CO2压力通过第一分支46传递到第一套管针。
将选用的第一腹腔镜12在合适的时间交给手术室台处的人员，选用的第一腹腔镜12已由手术室准备团队如已述的那样预先插入到鞘套14内。在无菌区上，手术室台处的人员将光缆30和摄像机电缆32连接到腹腔镜12(见图14)。在无菌区上，手术室台处的人员现在将导管组16的快速更换连接件22连接到歧管18的快速更换连接器20(见图15)。单通道止回阀打开，吹入器回路的小部分的输出由第二分支48途经歧管18送入到鞘套14内。
然后，将腹腔镜/鞘套组件作为整合的单元通过第一套管针放置以获得腹腔的初始视野(见图16)。由于偏转器组件64的技术性质，CO2在镜头上方流过，消除雾气并使碎片偏转离开。如果存在填充有无菌流体(优选地具有“表面活性剂”)的并连接到导管卢尔锁的泵(例如，20cc注射器)，可以由手术室台处得人员操作这个泵以将无菌流体冲洗过鞘套14的偏转器组件64。偏转器组件64引导流体以大致偏离CO2路径90度的路径冲过镜头。一旦如此做，导管组16上的球形物可以多次泵压，引入多股空气以清除镜头上的流体滴并使其离开末端偏转器，以维持被引导的CO2流不断地横过腹腔镜镜头。
一旦获得满意的视野，附加的带有旋塞阀的辅助套管针，例如三个到四个，或更多，也通过切口放置以提供其他器械的入口(见图17)。具有视野优化组件的套管针通风装置24理想地放置在多个辅助套管针中的一个的旋塞阀内，并且旋塞阀打开(见图18)。
如图19所示，手术室团队的成员优选地将主吹入管道(导管组16的Y型连接器44的第一分支46导管)与第一套管针脱开，而到无菌区上其他合适的套管针的旋塞阀上(除了与通风装置24连接的套管针)。这另一套管针随后用作主吹入套管针，与第一套管针分离，该第一套管针现在作为主可视化套管针。这样，由吹入套管针提供用于在手术期间的主CO2吹入，该吹入套管针也不是可视化套管针。尽管吹入器周期性的循环，通风装置24提供的吹入压力的受控的泄漏在气腹内生成压力梯度，以帮助维持从偏转器组件64横过镜头的大致连续的CO2。鞘套14内的内腔40和42(如前所述)也可以作为附加的被动通风口，以将吹入压力通过歧管18泄漏出去。
手术继续进行。偏转器组件64提供微创手术期间的术中除雾和腹腔镜镜头清洁，同时维持手术部位的可视化。如所期望的那样，如果需要通过冲洗镜头以增加可视化，可以使用无菌流体冲洗机制。如果这样做，导管组16上的球形物应该多次泵压，以清除镜头的流体滴并使其离开偏转器组件64，以维持CO2帘幕般横过镜头。
在手术期间，手术室团队可以决定，例如，一部分程序使用不同角度的窥 镜能获得更好的可视化。导管组16的连接件和歧管18的连接器的快速更换性质，大大方便了将腹腔镜12以对手术程序最小程度的打断而更换成另一个，并且不会对无菌区造成危害。
为了将一个腹腔镜12更换成另一个，手术室团队的一个成员将腹腔镜/鞘套组件这个整合单元从可视化套管针中抽出(见图20)。手术室团队的一个成员将腹腔镜12与光缆30和摄像机电缆32脱开(见图21)。手术室团队的一个成员将快速更换连接件22与快速更换连接器20脱开，将腹腔镜/鞘套组件从导管组16释放(见图22)。脱开的腹腔镜/鞘套组件作为整合单元交给手术室团队的一个成员，例如，手术助理护士(见图23)。此时没有理由将鞘套14从匹配的腹腔镜12上移除。这可以在之后完成，在手术完全做完后。
包括将被使用的第二腹腔镜12的腹腔镜/鞘套组件，已经组装成一个整合单元，如前所述。将这一预组装的单元交给手术室团队的一个成员(见图24)。手术室团队的一个成员将第二腹腔镜12连接到光缆30和摄像机电缆32(见图25)。手术室团队的一个成员将导管组16的快速更换连接件22连接到快速更换连接器20，以使得腹腔镜/鞘套组件与导管组16处于流动连通(见图26)，完成快速更换。第二腹腔镜/鞘套组件被插入到可视化套管针内(见图27)。
与每个鞘套14相关联的歧管使得快速连接特征起作用。可以将无菌区上的导管组16迅速地脱开，但不必，并且理想地没有，将其更换为另一导管组16。在给定的手术程序期间，相同的导管组16用于每个所使用的腹腔镜/鞘套组件(可以将配有附加鞘套的不需要的导管组16简单地抛弃)。
手术继续进行，使用第二腹腔镜/鞘套组件。 
可以在手术进行中以上述方式完成额外的腹腔镜的快速更换。
一旦手术完成后，将所有的器械，包括使用的腹腔镜/鞘套组件从可视化套管针中移除(见图28)。手术室团队的一个成员将腹腔镜12与光缆30和摄像机电缆32脱开(见图29)。手术室团队的一个成员将快速更换连接件22从快速更换连接器20上脱开，将腹腔镜/鞘套组件从导管组16释放。将腹腔镜/鞘套组件交给手术室团队的一个成员(见图31)，并与之前使用过的腹腔镜/鞘套组件一起放置(见图32)。
将入口部位闭合。将吹入器关闭。导管组16从吹入回路上脱开。将歧管18上的锁圈松开，并且将腹腔镜从鞘套抽出以备重复使用(见图33)。将鞘套和导管组16处理掉(见图34)。
有些套管针被称为“光学套管针”，套管针内的填塞器内具有内腔。如果将腹腔镜12的镜头首先放入光学套管针的中心以引导第一套管针插入，那么鞘套14不能在腹腔镜12上，因为该组合件不能适于通过填塞器的内腔。在这种情况下，使用不带鞘套14的腹腔镜12以放置第一套管针。随后，腹腔镜12 插入鞘套14，并且以如前所述的方式使得导管组16产生连接。在填塞器已从套管针移除的情况下，腹腔镜/鞘套组件以前述方式放入第一套管针。
