Abstract
一种隧道器/外壳组件(100,200,300)具有隧道器(110,210,310)和用于在导管的皮下隧穿期间容纳导管(12)的伸长外壳(102,202,302)。在隧穿之后隧道器可从外壳去除以暴露导管前导端以被直接牵引通过隧道,或者包括导管出口侧开口(324)以为了该目的暴露导管前导端。隧道器的钝前导端(112,212,312)具有用于产生皮下隧道(26)的无损伤形状。
Description
本发明涉及医疗设备的领域,尤其涉及用于在将导管植入患者的脉管系统中期间皮下锚固近侧导管部分的用于导管的隧道器。
当导管组件被植入患者的脉管系统中时,导管的远侧部分通过切口插入脉管系统中直到远侧尖端精确地位于预期部位,而近侧部分保留在脉管系统外部。近端部分保留在患者的外部以用于接入输注流体或收回流体的导管,或用于与血液透析装置连接。为了保证导管组件上的应力和应变不导致远侧尖端从它的正确位置移动并且也防止感染,尤其对于长期导管,长度可变的导管通过皮下隧道被放置。这通过使用隧道器或套管针实现。标准隧道器在附连到导管的端部之后牵引导管通过皮下组织。可以通过在其插入脉管系统中之前将隧道器附连到导管远端,或者通过在远侧部分放置到静脉中之后附连到用于隧穿的导管的近端(称为逆行隧穿)执行隧穿。
通常,隧道器是大体不可挠曲的圆筒形杆,带有在靠近导管的静脉入口部位的位置到隧道出口部位之间皮下推进以产生隧道的钝尖端。隧道器的相对或连接端首先可附连到导管端,并且随后在隧穿之后它可从导管端去除。有几种已知的导管/隧道器附连方式,包括在美国专利Nos.4,453,928;4,832,687;5,190,529;5,944,732;6,453,185;和6,872,198;以及在美国专利公报Nos.US 2004/0176739;US2004/0193119;和US 2005/0027282中公开的那些。在美国专利Nos.5,360,407和5,637,102中公开了设备到导管端部的其他连接,其中导管的近端插入带倒钩锁定装置上,借助于锁定外套管保证连接。
在美国公报No.US2005/0027282中,公开了一种可附连到隧道器的连接端的适配器,其中适配器的敞开端允许导管端例如远端部分的插入,其后多个夹紧部分移动到与导管的外表面成夹紧关系并且锁定就位;在隧穿之后,夹紧部分被解锁,释放导管供收回。
希望提供一种隧道器,其容易用于小直径导管,例如PICCs(周边可插入中心导管)或某些居中可插入导管。
也希望提供一种隧道器,其在隧穿之后可从导管去除而不用破坏在隧穿期间与隧道器相邻的导管的端部。
简单地说,本发明提供了一种隧道器组件,其具有隧道器和附连到隧道器的伸长外壳,在隧穿过程期间导管可以位于所述外壳内或者在限定隧道之后导管可以穿过所述外壳但是所述外壳仍然位于隧道中,由此导管不必机械地连接到隧穿装置和从其断开连接。隧道器优选地具有钝的前导尖端,所述前导尖端具有圆整的边和角并且是无损伤的,带有从其朝着外壳前导端向后延伸的伸长斜切表面。在产生隧道之后优选地从外壳的前导端去除隧道器,其后可以从外壳的尾端向前挤压导管直到导管的前导端可以被抓握以进一步被牵引,直到希望被锚固的导管的部分在隧道内就位。

在图中,相同的数字始终表示相同的元件。某些术语在本文中仅仅为了方便而被使用并且不应当被看作对本发明的限制。术语“远侧”和“近侧”分别表示靠近和远离导管在可植入导管组件中的插入尖端的方向。术语“前导端”和“尾端”表示相对于外壳/隧道器组件移动通过皮下隧道的方向的外壳/隧道器组件的端部或导管的端部,无论导管的前导端是否是导管的远端。术语包括具体提及的词语、它的衍生词和相似含义的词语。下面例举的实施例并非旨在穷举或将本发明限制为公开的确切形式。这些实施例被选择和描述以最佳地解释本发明的原理及其应用和实际使用并且允许本领域的其他技术人员最佳地利用本发明。
在图1-3中,显示了隧道器/外壳组件100的第一实施例,包括具有前导端104和尾端106的圆筒形外壳102和在其前导端104附着到外壳102的隧道器110。隧道器110包括钝的、无损伤尖端112和连接部分114,该隧道器110通过所述连接部分(例如通过压配合到外壳前导端104中)被固定到外壳102。隧道器110限定从钝的、圆整的尖端112向后朝着外壳前导端延伸的伸长斜切表面116,这种组合为它提供分离患者的皮下组织以限定隧道的能力。
在图1中,也显示了导管组件10具有导管12、接口14、一对延伸管组件16(与导管12的相应内腔关联)和导管远端18。导管12被显示为它的长度的大部分位于外壳102内,导管远端18邻近在外壳前导端104的隧道器110。优选地,导管组件还包括组织向内生长套囊30,其便于组织向内生长到隧道内以增强导管的锚固。导管组件10可以在已经放置到外壳102内的情况下被运输,尽管它不需要附着到隧道器组件100;如果需要的话,导管可以借助于在外壳近端的夹具固定到外壳,在执业医生进行隧穿之前容易允许从隧道器组件去除导管。形成于患者中用于锚固导管的典型皮下隧道在长度上为大约5cm到大约10cm。典型导管长度可以为大约60cm;隧道器/外壳组件100例如可以为大约15cm到20cm。该隧道器尤其对例如4-6F的小直径导管有用。
在图1中,隧道器组件100被显示为形成U形。该形状通过减小总长度便于运输、装卸和储存,并且也用于在运输、装卸和储存期间以非卷曲形状将导管保持在其内。制造外壳的材料优选地足够可挠曲以允许执业医生容易地使外壳形成为线性构造,以穿过隧道。
在图3中显示了在患者20中从入口切口22皮下隧穿到出口切口24(隧道26在其间延伸)之后、其内有导管12的隧道器/外壳组件100。隧道器110已经完全穿过(并且由此产生和限定)隧道26,并且外壳102基本被牵引通过隧道26,又随着它牵引导管前导端18。备选地,可以在其内没有导管12的情况下利用隧道器/外壳组件100以产生隧道,其后可以通过它的前导端18使导管12插入和通过外壳102,而外壳保留在隧道内;在隧穿之后当隧道器110例如通过从外壳端折断或切断从外壳前导端104断开连接时,导管前导端18可以由执业医生抓握。在导管组件10被隧穿并且导管12的锚固部分28在隧道26内就位(在长度上为大约5cm到10cm)之后,锚固部分28优选地包括常规的组织向内生长套囊30。如果导管前导端18是导管的远端,导管现在可以在它的前导端18被切成所需长度并且准备好通过静脉切开术插入患者的脉管系统中,例如通过使用都是常规的易撕导引鞘、扩张器和导丝(未显示)。
图4-7示出了本发明的隧道器/外壳组件200的第二实施例,外壳202具有前导端204和尾端206,并且隧道器210具有钝的尖端212、连接部分214和延伸到钝的尖端212的伸长斜切表面216。图5尤其显示了钝的尖端212包括圆整的边和一对圆整的角218,由此是无损伤的,这在本发明的所有实施例中是优选的。隧道器210的连接部分被显示为包括外螺纹220,使得它可以通过旋入外壳前导端204中连接到外壳202;隧道器210通过简单地从其拧下而类似地容易从外壳202的前导端断开连接和去除。可选地,连接部分可以改为包括一个或多个倒钩(未显示)而不是螺纹。组件200的外壳202可以由与第一实施例的外壳相同的材料制造,并且被显示为具有大体弯曲性质,其可以便于被推过皮下组织以产生隧道。
在图8中显示了外壳隧道器组件300的第三实施例。与图1-3的实施例相同,隧道器310包括适于被压配合到外壳302的前导端304中的连接部分314。隧道器310也包括钝的、圆整的无损伤尖端312和从其向后朝着外壳前导端304延伸的锥形表面316。在隧道器310中限定了与延伸到外壳302的中空通道328的中心通道326连通的侧向开口324。侧向开口324为隧道器/外壳组件300提供了一种选择,即允许执业医生将导管12的尾端推入外壳和隧道器中,直到导管前导端14进入通道326并且在侧向开口324穿出隧道器/外壳组件300,使得前导端18可以由执业医生抓握和直接牵引通过外壳和隧道,其后隧道器/外壳组件然后可以从导管和隧道被去除,并且被丢弃。
本发明的隧道器可以由塑料例如聚丙烯或尼龙6/ABS混合物模制而成。本发明的外壳可以由塑料例如聚乙烯挤出。外壳优选地长于隧道的预期长度,并且优选地大于10cm,更优选地大于15cm。外壳的增加长度导致在隧穿期间导管自身保持在外壳内的更大保证。通过执业医生简单地修剪在其近端的过多长度,在隧穿之前外壳的总长度可以被减小。
本领域的技术人员将会理解可以在不脱离本发明的宽泛创新概念的情况下对上述实施例进行变化。所以应当理解本发明并不限于公开的特定实施例,而是旨在涵盖在附带权利要求所限定的本发明的范围内的修改。
Claims (15)
1.一种用于脉管导管的皮下隧穿的隧穿装置,包括:
具有前导端和尾端的圆筒形伸长外壳,以及在所述外壳的前导端可去除地附着到所述外壳并且从所述外壳的前导端延伸的隧道器,该隧道器具有钝的无损伤前导尖端和朝着所述外壳的前导端延伸的伸长斜切表面。
2.根据权利要求1所述的隧穿装置,其中所述隧道器包括延伸到所述外壳的前导端中的连接部分。
3.根据权利要求2所述的隧穿装置,其中所述连接部分通过压配合被保持在所述外壳的前导端内。
4.根据权利要求2所述的隧穿装置,其中所述连接部分通过被旋入所述外壳的前导端中而被旋入和保持在所述外壳的前导端内。
5.根据权利要求2所述的隧穿装置,其中所述隧道器包括通过所述连接部分并且延伸到侧向开口的通道。
6.根据权利要求1所述的隧穿装置,其中隧道器/外壳组件由大体挠性材料形成U形。
7.根据权利要求1所述的隧穿装置,其中隧道器/外壳组件在长度上至少为10cm。
8.根据权利要求1所述的隧穿装置,其中隧道器/外壳组件在长度上至少为15cm。
9.一种导管组件和用于导管组件的导管的皮下隧穿的隧穿装置的组件,包括:
具有导管的导管组件,带有朝着导管尾端延伸的导管前导端,和
具有前导端和尾端的圆筒形伸长外壳,和在所述外壳的前导端可去除地附着到所述外壳并且从所述外壳的前导端延伸的隧道器,所述隧道器具有钝的无损伤前导尖端和朝着所述外壳的前导端延伸的伸长斜切表面,
所述导管被布置在所述外壳内,使得所述导管前导端邻近所述隧道器并且所述导管尾端从所述外壳的尾端向外延伸,并且所述外壳的内径被选择为使得所述导管通过轻微的摩擦配合被保持在所述伸长外壳内,从而在通过所述隧道器产生皮下隧道期间所述导管保留在所述外壳内,其后所述隧道器/外壳组件和所述导管被牵引通过所述隧道,并且所述隧道器/外壳组件能够在所述导管前导端从所述导管去除。
10.根据权利要求9所述的组件,其中所述隧道器包括延伸到所述外壳的前导端中的连接部分。
11.根据权利要求10所述的组件,其中所述连接部分通过压配合保持在所述外壳的前导端内。
12.根据权利要求10所述的组件,其中所述连接部分通过旋入所述外壳的前导端中而被旋入并保持在所述外壳的前导端内。
13.根据权利要求10所述的组件,其中所述隧道器包括通过所述连接部分并且延伸到侧向开口的通道。
14.根据权利要求9所述的组件,其中隧道器/外壳组件的长度至少为10cm。
15.根据权利要求9所述的组件,其中隧道器/外壳组件的长度至少为15cm。
