Abstract

技术领域
本发明总体涉及显微外科器械。尤其地,但非限制地,本发明涉及在后段眼科手术中使用的显微外科器械。
背景技术
许多显微外科手术需要精确切割和/或移除各种身体组织。例如,玻璃体视网膜手术经常需要对精细组织如玻璃液、牵引带、膜或视网膜进行切割、移除、切开、分层、凝结或其它的操作。玻璃液或玻璃体包括大量通常附着在视网膜上的微小纤维。因此,对玻璃体的切割、移除或其它操作必须格外小心地完成以防止对视网膜的牵引、视网膜从脉络膜上分离、视网膜撕裂或者最坏的情况——视网膜自身的切割或移除。
在玻璃体视网膜手术中典型地使用显微外科器械,例如玻璃体切除术探针、纤维眼照明器、输注套管、抽吸探针、剪刀、镊子以及激光器。这些设备通常穿过一个或多个外科切口插入平坦部附近的巩膜,其被称为巩膜切开术。这些设备的重复插入和移除会导致玻璃体和其它流体通过巩膜切开术离开眼睛,而增加球体变软、出血、视网膜上的牵引或将细菌带入到眼睛中的可能。因此,一种输注套管被用于将冲洗溶液输注到眼睛中以保持适当的眼压。
套针套管和塞经常与输注套管一起使用。套针套管建立起通过巩膜进入到眼睛的后段的入口。输注套管放置在套针套管内以便为眼睛提供冲洗溶液。套针塞放置在套针套管内以在移除输注套管时防止泄漏。重要的是,在套针套管和输注套管或套针塞之间建立可靠的连接以防止在手术期间不期望的眼压损失。典型地,这种连接通过套针套管的内径和输注套管或套针塞的外径之间的干涉配合而实现。然而,干涉可能使套针套管和输注套管或套针塞出现非弹性变形,并且在套针套管和输注套管或套针塞之间产生静摩擦。该干涉也难于由小的标定仪表和传统的制造公差来控制。此外,产生该干涉的传统方法是卷曲输注套管。然而,这样的卷曲通常在输注套管内产生流动限制,其在一些手术的高真空设置期间是特别地不期望的。因此,需要一种改进的套针套管系统。
发明内容
在一个方面,本发明是一种套针套管系统,其通常包括套针套管和显微外科器械。该套针套管具有座(hub)和联接于所述座的管道。该管道的内表面上布置有突起。显微外科器械可移除地且部分地布置在所述管道内。该器械的外表面上布置有与所述突起配合的凹陷。所述突起和凹陷的配合使得所述器械三点弯曲地放置以将所述器械固定在所述套针套管内。

通过参考附图的图1-2,将最佳地理解本发明的优选实施例及其优点,其中不同的图形中相同或相应的部分采用相同的附图标记。
图1示出了套针套管系统10,通常包括套针套管12和输注套管或套管塞14。为了便于描述,本发明将参考输注套管进行如下描述,应理解的是,其也等价地适用于套针塞或其它显微外科器械。
套针套管12通常包括管道18和位于近端的座16。座16包括用于接纳输注套管14的开口20、用于接触巩膜内表面的环状表面22、以及用于接触巩膜外表面的环状表面24。管道18具有空心孔25、内部表面26、用于接纳输注套管14的开口28、以及用于允许输注套管14穿过进入眼睛后段的开口30。管道18具有布置在座16内的具有第一内径的第一部分以及具有小于其第一内径的第二内径的第二远端部分。内表面26具有凸出的突起32,其优选地布置在座16内。套针套管12优选地由外科手术不锈钢形成。
输注套管14通常包括空心孔34、用于允许冲洗流体进入眼睛后段的开口36、布置在座16内具有第一内径的第一部分,以及具有小于其第一内径的第二内径的第二远端部分。如图2最佳所示,输注套管14在其外表面上具有凹形的凹陷或凹痕36以与套针套管12的突起32相配合。输注套管14优选地由外科手术不锈钢形成。
在使用中,外科医生施行巩膜切开术并插入套针套管12到其中,使得巩膜布置在座16的表面22和24之间;开口30布置在眼睛的后段内,优选眼睛的玻璃体腔内;并且开口20布置在巩膜外。外科医生将输注套管14插入到管道18中使得开口36布置在眼睛的后段内,优选眼睛的玻璃体腔内。套针套管12的突起32与输注套管14的凹陷36相配合。突起32和凹陷36的配合使得输注套管14能以弹性的三点弯曲的方式放置,安全地将输注套管14以合理的保持力紧固在套针套管12内。突起32和凹陷36的配合使得在输注套管14的孔34中不产生流动限制特征。突起32和凹陷36的配合能够采用合理大小的力脱离以允许移除输注套管14。套针塞14或其它显微外科器械可以随后插入到巩膜切口中。
可以相信,本发明的操作和结构从前述描述中明了。然而上述示出或描述的装置和方法是优选的,在不脱离本发明权利要求书所限定的精神和范围的情况下可以进行各种改变和修改。
Claims (8)
1、一种套针套管系统,包括:
套针套管,其具有座和联接于所述座的管道,所述管道的内表面上布置有突起;
显微外科器械,其可移除地且部分地布置在所述管道内,所述显微外科器械的外表面上布置有与所述突起配合的凹陷;
其中所述突起和所述凹陷的配合使得所述显微外科器械三点弯曲地放置,从而将所述显微外科器械固定在所述套针套管内。
2、如权利要求1所述的套针套管系统,其中所述突起具有凸起的几何形状。
3、如权利要求2所述的套针套管系统,其中所述凹陷具有凹形的几何形状。
4、如权利要求1所述的套针套管系统,其中所述显微外科器械是输注套管。
5、如权利要求1所述的套针套管系统,其中所述显微外科器械是套针塞。
6、一种将微外科手术工具固定在套针套管内的方法,包括如下步骤:
将套针套管插入到眼睛的巩膜中;
将显微外科器械插入到所述管道内使得所述显微外科器械的远端布置在眼睛内,其中所述管道的内部几何形状和所述显微外科器械的外部几何形状使得所述显微外科器械三点弯曲地放置,从而将所述显微外科器械固定在所述套针套管内。
7、如权利要求6所述的方法,其中所述显微外科器械是输注套管。
8、如权利要求7所述的方法,其中所述显微外科器械是套针塞。
