技术领域
本发明涉及医疗器械领域的一种前端带有封堵球囊的注胶封堵左心耳装置。
背景技术
左心耳位于左心房外侧，为中空结构，内有大量梳状肌，血流缓慢，在心房颤动时极易形成血栓，血栓一旦脱落即可造成栓塞事件，因此左心耳在房颤猝中扮演着重要角色。目前，临床上各种介入封堵手段来实现左心耳的封闭，创伤小,患者易接受,且已被证实安全可行。目前有多种介入封堵左心耳装置已用于临床,其中研究较多的是PLAATO、Watchman、Amplatzer Cardiac Plug和LARIAT等装置。除LARIAT之外,其他装置均通过股静脉及房间隔穿刺途径进入,输送鞘管一般需9～14Fr。
Watchman装置是目前唯一被美国FDA批准的用于左心耳封堵的装置。封堵器镍钛合金骨架形状类似“水母”,外覆聚酯纤维膜,并有倒钩起到固定于心耳内的作用。封堵器有21、24、27、30、33mm等5种尺寸,选择较左心耳开口直径大10％～20％的封堵器,需12Fr输送鞘。借助猪尾巴导管将输送鞘送至心耳深处,封堵器到位后,通过回撤输送鞘释放封堵器。
然而，左心耳封堵器不同封堵装置的形状设计差别很大,能否完全封闭左心耳很大程度上取决于左心耳本身的形态,选择适合心耳形态的封堵装置可提高封堵成功率。而左心耳形态、深度、开口位置和大小,及其与肺静脉的关系均存在较大变异性,不同形态的心耳形成血栓的风险也不同。另外，目前的左心耳封堵器主要以镍钛合金等金属结构为主体，对左心耳结构的顺应性较差，封堵时也容易导致出现左心耳破裂、心包填塞等并发症。
此外，左心耳封堵的手术操作难度比较大，学习曲线较长，对手术操作者的要求高。
发明内容
本发明的目的是为了克服现有技术的不足，提供一种前端带有封堵球囊的注胶封堵左心耳装置，可用医用胶水或者高强度生物凝胶代替金属材质的封堵器来封堵左心耳开口，大大降低了封堵左心耳手术的风险和难度，减少了左心耳封堵手术给患者带来的损伤。
实现上述目的的一种技术方案是：一种前端带有封堵球囊的注胶封堵左心耳装置，包括弯曲弧度可调的外鞘管以及位于所述外鞘管径向内侧的注胶管，所述注胶管在所述外鞘管内具有与所述外鞘管匹配的弯曲弧度，所述注胶管头部有不透射线标志；
所述外鞘管头部为不透射线标志段，所述不透射线标志段的末端连接用于临时封堵左心耳开口的，可收缩和扩张的封堵球囊；所述外鞘管的末端连通双Y阀，所述双Y阀的第一支管连通排气注射器，所述双Y阀的第二支管连通用于控制所述封堵球囊扩张与收缩的压力泵，所述双Y阀的主管的末端连接旋转密封阀；所述注胶管的末端通过所述旋转密封阀连通注胶器。
进一步的，所述外鞘管还包括位于所述封堵球囊后端的可控弯曲段，所述外鞘管的末端设有用于控制所述可控弯曲段弯曲弧度的可控弯曲螺母。
再进一步的，所述外鞘管还包括连接所述可控弯曲段末端的外鞘管本体。
进一步的，所述注胶管的头部设有伸出所述外鞘管头部的注胶管旋转软头。
进一步的，所述外鞘管的内径为6-14Fr，所述注胶管的外径为4～10Fr。
进一步的，所述封堵球囊在扩张状态下为扁椭圆体，其径向上短轴的长度为其轴向长度约2-5倍。
采用了本发明的一种前端带有封堵球囊的注胶封堵左心耳装置的技术方案，包括弯曲弧度可调的外鞘管以及位于所述外鞘管径向内侧的注胶管，所述注胶管在所述外鞘管内具有与所述外鞘管匹配的弯曲弧度，所述注胶管头部有不透射线标志；所述外鞘管头部为不透射线标志段，所述不透射线标志段的末端连接用于临时封堵左心耳开口的，可收缩和扩张的封堵球囊；所述外鞘管的末端连通双Y阀，所述双Y阀的第一支管连通排气注射器，所述双Y阀的第二支管连通用于控制所述封堵球囊扩张与收缩的压力泵，所述双Y阀的主管的末端连接旋转密封阀；所述注胶管的末端连通注胶器。其技术效果是：可用医用胶水、高强度生物凝胶、镍钛合金金属丝团与医用胶水或者高强度生物凝胶的复合物来代替金属材质的封堵器来封堵左心耳开口，大大降低了封堵左心耳手术的风险和难度，减少了左心耳封堵手术给患者带来的损伤。

请参阅图1，本发明的发明人为了能更好地对本发明的技术方案进行理解，下面通过具体地实施例，并结合附图进行详细地说明：
本发明的一种前端带有封堵球囊的注胶封堵左心耳装置包括外鞘管1以及位于外鞘管1径向内侧的注胶管2。外鞘管1的内径为6-14Fr，从外鞘管1的头部至尾端，依次为不透射线标志段11，封堵球囊12、可控弯曲段13和外鞘管本体14。封堵球囊12用于临时封堵左心耳开口，封堵球囊12扩张状态下为扁椭圆体，其径向上短轴的长度为其轴向长度约2-5倍；轴向为与外鞘管1轴向平行的方向，径向为与外鞘管1轴向垂直的方向，封堵球囊12距离外鞘管1的头部约1-5cm，即不透射线标志段11的长度为1～5cm。外鞘管1的不透射线标志段11用于外鞘管1的头部开口端在人体左心耳的定位，较为柔软，内无编织抗折层，不易损伤左心耳组织。外鞘管1的尾端连通双Y阀17，双Y阀17的第一支管171连通用于外鞘管1内排气的排气注射器31，双Y阀17的第二支管172连通压力泵32，用于控制封堵球囊12扩张与收缩。双Y阀17的主管173的末端设有旋转密封阀18，在向人体股静脉内送入导丝、内芯或者注胶管2时起到密封的作用。导丝一端穿过外鞘管1的可控弯曲段13，连接不透射线标志段11，另外一端连接位于外鞘管1末端的可控弯曲螺母16，利用拉伸弯曲原理，通过所述导丝来控制可控弯曲段13的弯曲程度，可精细地调整外鞘管1头部开口端的位置。顺时针旋转可控弯曲螺母16，可使可控弯曲段13的弯曲弧度增大；而逆时针旋转可控弯曲螺母16，可使可控弯曲段13的弯曲段弧度变小。
注胶管2的外径比外鞘管1的内径稍小，一般为4-8Fr，在外鞘管1内保持与外鞘管1匹配的弯曲弧度，注胶管2的头部设有从外鞘管1头部开口端伸出的注胶管旋转软头21，注胶管旋转软头21上有不透射线的标志。注胶管2材质为高分子聚合物，具备一定的柔韧性和抗折性，能保持管腔不变形。注胶管旋转软头21可安全地伸入左心耳内注射凝胶，而不会使左心耳壁受到损伤而穿孔。注胶管2的尾端通过旋转密封阀18与注胶器4紧密连通。
在实施经注胶管2注胶封堵左心耳手术时，操作顺序如下：用房间隔穿刺鞘穿刺股静脉，顺房间隔穿刺鞘将导丝送至左心房，建立股静脉-下腔静脉-右心房-房间隔-左心房的轨道；将房间隔穿刺鞘退出人体股静脉；将内装扩张管的外鞘管1沿着所述轨道送至左心房，所述扩张管从外鞘管1内退出后，经外鞘管1送入猪尾巴导管至左心耳内，调整外鞘管1的可控弯曲螺母16，将外鞘管1的头部开口端送至左心耳内。扩张封堵球囊12，临时堵闭左心耳开口，将所述猪尾管从外鞘管1中退出，向外鞘管1内送入注胶管2，使注胶管2头部的注胶管旋转软头21进入左心耳底部，确认位置准确后，注胶管2连通注胶器4，通过注胶器4和注胶管2向左心耳内注入医用胶水或者生物凝胶，医用胶水或者生物凝胶在左心耳开口迅速溶胀，黏合左心耳腔壁，待医用胶水或生物凝胶充满左心耳，将注胶管2退出外鞘管1。等医用胶水或生物凝胶凝固后，收缩封堵球囊12，退出外鞘管1，完成封堵。
另一种方式：在注入胶水以前，首先通过微导管送入数量不等的镍钛合金丝团，丝团直径在5-30mm之间，然后通过注胶管2注入医用胶水或者高强度生物凝胶，类似于钢筋混凝土结构以增大左心耳内封堵物的结构强度。
本发明的一种前端带有封堵球囊的注胶封堵左心耳装置，采用医用水凝胶封堵左心耳开口，通过临床验证，医用水凝胶生物相容性很高，并在心力衰竭治疗中得到应用，并得到FDA批准临床使用。与目前临床开展的现有技术相比，本发明的一种前端带有封堵球囊的注胶封堵左心耳装置，结构简单，外鞘管1的外径明显缩小，体表创伤更小。同时使水凝胶的形状可适应左心耳的形状，可做到无死角封堵，更不需预制封堵器的型号和形状，操作简便，可靠性高，操作完成后，体内不会残留金属异物，或者仅残留少量镍钛合金丝，整个过程不易出现心包填塞等并发症。
本发明的一种前端带有封堵球囊的注胶封堵左心耳装置可使用各种类型的生物凝胶来封堵左心耳开口，比较适合的是医用水凝胶，这是一种高分子网络体系，性质柔软，能保持一定的形状，能吸收大量的水，注入左心耳后，可迅速填满左心耳中的空隙。医用水凝胶在与血液、体液及人体组织相接触时表现出良好的生物相容性，既不影响生命体的代谢过程，又可以使代谢产物通过水凝胶排出。医用水凝胶比其它任何合成生物材料都接近活体组织，在性质上类似于细胞外基质部分，吸水后可减少对周围组织的摩擦和机械作用，显著改善材料的生物学性能。
通过本发明的一种前端带有封堵球囊的注胶封堵左心耳装置封堵左心耳开口时所采用的医用水凝胶必须是可注射的、溶胀速度快、结构强度高、生物相容性好、降解周期较长的医用水凝胶，如聚乙烯醇类医用水凝胶、纳米复合医用水凝胶、藻酸盐多孔医用水凝胶、多孔聚丙烯酰胺医用水凝胶等等，也可使用目前临床使用的封堵血管瘘的医用胶水，如Onyx胶水等。
通过本发明的一种前端带有封堵球囊的注胶封堵左心耳装置封堵左心耳开口时，由静脉系统穿刺房间隔途径，将外鞘管1送至左心耳，用外鞘管1自带的封堵球囊12封闭左心耳开口，经过外鞘管1直接向左心耳内注入生物凝胶；或者经外鞘管1送入注胶管2，将注胶管2送入左心的底部，注入生物凝胶封堵左心耳。外鞘管1顶端的封堵球囊12可临时封堵左心耳开口，可以起到限制生物凝胶外溢的作用。其次，也可对生物凝胶进行塑形，使之光滑平整，容易内皮化，不易形成血栓。
整套经导管注胶封堵左心耳装置组件包括远端带封堵球囊12的外鞘管1、注胶管2以及引导内芯。辅助器械包括注胶器、压力泵、排气注射器。手术过程中还将用到房间隔穿刺鞘、各类导丝、猪尾巴导管等其他物品。
本发明的一种前端带有封堵球囊的注胶封堵左心耳装置，大大降低了封堵左心耳手术的风险和难度，缩短学习曲线。

所述外鞘管头部为不透射线标志段，所述不透射线标志段的末端连接用于临时封堵左心耳开口的，可收缩和扩张的封堵球囊；所述外鞘管的末端连通双Y阀，所述双Y阀的第一支管连通排气注射器，所述双Y阀的第二支管连通用于控制所述封堵球囊扩张与收缩的压力泵，所述双Y阀的主管的末端连接旋转密封阀；所述注胶管的末端通过所述旋转密封阀连通注胶器。
2.根据权利要求1所述的一种前端带有封堵球囊的注胶封堵左心耳装置，其特征在于：所述外鞘管还包括位于所述封堵球囊后端的可控弯曲段，所述外鞘管的末端设有用于控制所述可控弯曲段弯曲弧度的可控弯曲螺母。
3.根据权利要求2所述的一种前端带有封堵球囊的注胶封堵左心耳装置，其特征在于：所述外鞘管还包括连接所述可控弯曲段末端的外鞘管本体。
4.根据权利要求1所述的一种前端带有封堵球囊的注胶封堵左心耳装置，其特征在于：所述注胶管的头部设有伸出所述外鞘管头部的注胶管旋转软头。
5.根据权利要求1所述的一种前端带有封堵球囊的注胶封堵左心耳装置，其特征在于：所述外鞘管的内径为6-14Fr，所述注胶管的外径为4～8Fr。
6.根据权利要求1所述的一种前端带有封堵球囊的注胶封堵左心耳装置，其特征在于：所述封堵球囊在扩张状态下为扁椭圆体，其径向上短轴的长度为其轴向长度的2-5倍。
