Abstract
一种治疗消化道肿瘤的中成药采用斑蝥,壁虎,蟾酥,水蛭,莪术,半夏,喜树,天花粉,鳖甲,杜仲,八月扎,人参,丹参,青黛,夏枯草,当归,白芍,白术,柴胡,神曲,山楂,枳壳配制成口服成药。该药主要用于治疗原发性肝癌、胃癌、食管癌、肠癌等消化道癌症,并且疗效显著,总有效率为79.4%,对肝的有效率为81.25%。
Description
本发明涉及一种治疗消化道肿瘤中成药及其制备方法。
癌症是当今社会的一大公敌,特别是中晚期癌症,治疗较为困难,本发明具有扶正培本,清热解毒,活血化瘀,益气养血,养阴生津,化痰软坚,理气止痛,消肿散结,滋阴补肾,健脾化湿,利水渗湿等功能,对中晚期癌症可稳定病清,使瘤体缩小,减少痛苦延长生命。
目前国内用中药治疗癌症有过详细的报道,如专利申请CN1097620A和CN1098937A均报道了治疗消化道肿瘤的药物,但本发明所采用的中药配方与其不同并且具有疗效高,药理作用全面等优点。
本发明与专利申请CN1098937A相比,虽有疗效基本相同,但它只适用于肝癌,而本发明则为消化道肿瘤,适应范围广。
本发明治疗消化道肿瘤的中成药由下述中药制剂组成:
斑蝥2g-4g,壁虎20g-40g,蟾酥1g-2g,水蛭20g-30g,莪术20g-60g,半夏20g-60g,喜树10g-80g,天花粉20g-40g,鳖甲30g-120g,杜仲20g-80g,八月扎20g-60g,人参20g-100g,丹参20g-80g,青黛20g-80g,夏枯草20g-100g,当归20g-60g,白芍30g-60g,白术30g-60g,柴胡10g-100g,神曲10g-100g,山楂20g-120g,枳壳10g-60g。
本发明还提供了一种如上所述的治疗消化道肿瘤的中成药的制备方法,包括以下步骤:
(1)将泡制后的喜树,壁虎,水蛭,莪术,半夏,杜仲,八月扎,人参,丹参,夏枯草,当归,白术,丹参,柴胡,山楂,枳壳用清水浸泡两小时后煎煮,煎煮1-2小时后过滤取汁液,如此法煎煮三次;
(2)把泡制后的天花粉、神曲、蟾酥粉碎,过120目筛;
(3)将每100克鳖甲用食醋20g、白糖20g炒20分钟,冷凉后研碎过120目筛;
(4)将斑蝥浸入盛有沙土的锅内,翻炒,炒黄后冷凉研碎,过120目筛;
(5)将步骤(1)中得到的药液与上述步骤(2)、(3)、(4)中制得的中药粉末与青黛粉末混合后,干燥、灭菌,粉碎后过120目筛。
本发明治疗消化道肿瘤的中成药的药理详细描述如下:
中晚期癌症,多下气亏损,则应兼顾扶正固本,其中益气健脾尤为重要,本方将消症,扶正,解毒三法集于一体,重则清热解毒,活血化瘀,理气散结,化痰软坚,清利湿热,滋补肝肾,方中柴胡疏肝气,解郁结,以顺肝性;当归,白芍养肝血,柔肝体,以和肝用,配伍白术,益气健脾,既可使脾健而不受肝乘,又可使脾能化生气血以养肝;鳖甲平肝通利血脉,软坚散结,枳壳理气宽中,消胀除痞;半夏消痞散结,神曲行气消食,健脾开胃;山楂消食化积,破气散瘀,斑蝥攻毒,逐瘀;壁虎祛风,定惊,散结,解毒;蟾酥,解毒,消肿强心止痛;水蛭破血通经,逐瘀消症;喜树,破血化瘀,清热解毒;夏枯草本品辛散结气,苦寒泄热,具有清肝火,明目,散郁结之功;人参为大补元气,补脾益肺,生津止渴,安神增智。主要治疗气欲脱,脉微欲绝,脾气不足,倦怠无力,食欲不振,脘腹胀满,呕吐泻泄,肺气虚损,呼吸短促,气喘,神疲乏力,脉虚自汗,以及热病伤阴,津伤口渴,气虚血亏,心神不安,失眠多梦,惊悸健忘,阳萎早泄,久病虚羸难以康复等一切气血津液不足之候,总之,诸药合用共奏疏肝调脾,软坚消积之功。
斑蝥的水醇或丙酮提取物均能抑制Hela细胞和人的食管癌,贲门癌,胃癌,胃窦癌乳腺癌,纤维瘤,何杰金氏瘤,肝癌,肺癌及脾肉瘤细胞的代谢。斑蝥素或斑蝥提取物对一些实验动物移植性肿瘤有一定抑制作用。如对小鼠腹水型肝癌,网织细胞肉瘤(L2),肉瘤180均有显著抑制作用,斑蝥素的抗癌机理可能在于抑制了肿瘤细胞蛋白质和核酸的合成。
喜树:喜树碱对白血病L1210,DNA细胞,Hela细胞和其他肿瘤细胞均有一定抑制作用。喜树碱可使白血病L1210,L5178,Y.K1946,L615和腹水型肝癌小鼠的生存时间还可明显抑制多和实体肿瘤,如小鼠Lews,肺癌,黑色素瘤B16,脑瘤B22,艾氏腹水癌实体型及大鼠W256瘤肉瘤,吉田肉瘤等,而且喜树碱对小鼠白血病L1210和P388的各种耐药株(耐亚硝脲氮芥,阿糖胞苷,6-巯基嘌呤,长春新碱等,亦均有抗肿瘤作用。他的作用原理主要作用于细胞的DNA前期(G1期)和后期(G2期))。亦有作用,但不如S期细胞明显,并可延缓G2期进入核分裂(M期),较高浓度可抑制核的有丝分裂。喜树碱可首先抑制癌细胞的DNA多聚酶,继而影响DNA的生物合成,亦可直接破环DNA或首先与DNA结合,使DNA易受内切酶的攻击,进而使RNA和蛋白质合成受阻,杀伤癌细胞。
水蛭:水蛭体外实验,对小鼠肝癌生长有抑制作用。
壁虎:体外实验本品水溶液可抑制人体肝癌细胞膜的吸收,近代多用治疗多种癌肿,尤其是治疗食管癌。
蟾酥:蟾酥对小鼠肉瘤180和兔B.P瘤都有较满意的抑制效果。对白血病细胞抑制也较强。对小鼠精原细胞腹水癌细胞,肝癌细胞无抑制作用,但可延长其生存期。蟾酥对恶性神经胶质瘤细胞及中国金色仓鼠恶性转化细胞系有较明显的抑瘤作用。
人参:人参中的多种皂甙,人参多糖及人参挥发油具有抗肿瘤作用。人参皂甙对大鼠MOyyis肿瘤细胞经25代续代培养后,发现MOyyis细胞发生了形态的改变。镜检与正常的肝细胞相似,红参中的参皂甙能使癌细胞再分化,诱导逆转减癌细胞,人参皂甙-Rh2具有明显抑制人体早幼粒白血病HL-60癌株作用。人参皂甙对小鼠肉瘤S180有抑制作用,人参可明显减慢癌前病变或早期癌的发展速度,保护癌旁肝组织,尤其是线粒体,内质网等亚微结构,减少癌前病变的发生,从而明显延长癌鼠生存期。人参茎叶皂甙,花皂甙和人参多糖对S180也有明显的抑制作用。人参多糖还能抑制小鼠艾氏腹水癌细胞的增增,延长S180小鼠存活时间,并可增加肉瘤S180小鼠脏PFC.SPEC的血清抗SRBC抗体和脾细胞的抗体分泌量,以及增加肉瘤S180小鼠免疫器官重量。就中医而言,人参味甘性微温,大补元气,元气为人体最根本之气,元气衰微则导致体虚欲脱,脉微欲绝之证《本草经疏》中日:“能回阳气于垂绝,去虚鸦于俄倾”为治虚劳内伤第一要药,故凡大失血,大汗,大吐泻,以及一切疾病导致的元气虚极欲脱之证,单用本品即效。
另外,根据现代药理研究,柴胡具有镇静,镇痛,解热等作用,柴胡皂甙a和d具有抗渗出和抗肉芽肿的作用,有抑制癌细胞的作用。当归水煎液能显著促进血红蛋白及红细胞的生成,有抗贫血作用,有抗炎,镇痛及明显的抗血栓形成作用,能防止肝糖元降低,对肝有保护作用,有提高巨噬细胞及网状内皮细胞吞噬能力,提高非特异性免疫功能作用,西德研究人员从当归中分离出一种多糖(AR-1)具有重要的抗肿瘤活性。根据荷瘤(IMC癌腹水型)小鼠的延命率来计算和评价,AR-1用量在1.6-100mg/kg时,可有效地抗S180腹水癌,IMC癌Meth-A纤维肉瘤及MM-16癌的生长,同时能使3/5的小鼠生存期超过60天。白芍所含芍药甙有较好的解痉作用,对胃肠平滑肌表现抑制,亦有一定的镇痛,扩张血管及抑菌的作用,它能促进淋巴母细胞转化,抑制肿瘤生长,对细胞免疫及体液免疫均有一定的促进作用。白术有强壮作用,能促进体重增长,体力增加,能保护肝脏,防止四氯化碳所致肝糖元减少,显著延长凝血酶元时间,有坑凝血作用。体外实验表明,白术挥发油中对食管癌细胞有明显抑制作用。20mg/ml时,可使大部分癌细胞脱落,残存的小片细胞的核固缩,核仁模糊不清,胞质多空泡。白术挥发油对小鼠肉瘤180,艾氏腹水癌及淋巴肉瘤腹水型均有抑制作用。鳖甲可抑制结缔组织增生,消散结块,故有软坚散结作用。现代药理证实,鳖甲用美蓝法试验对肝癌,胃癌,急性淋巴性白细胞的抑制作用,用细胞平板过滤有效。实验还证明,鳖甲能抑制人体肝癌,胃癌和呼吸。半夏各种制剂均有镇吐作用,并有抗肿瘤之功。神曲内含酵母菌,维生素B复含体,酶类,麦角固醇,蛋白质,脂肪等,有促进消化,增进食欲的作用。山楂能增加胃中消化酶的分泌,促进消化,并含有脂肪酶,能加强胃脂肪醇,蛋白酶的活性,有较强的助消化作用,山楂粉剂有降低实验性高脂血症的胆固醇与β脂蛋白的作用;山楂中的黄酮类或分壮荆素化合物,苦杏仁,甙可延长荷瘤动物的寿命,山楂提取液有抑制艾氏腹水癌细胞的作用,山楂核水煎液对子宫颈癌JTC-26抑制率高达70%。
本发明治疗消化道肿瘤的中成药由下述基本制备工艺:
1.煎煮:将泡制后的喜树,壁虎,水蛭,莪术,半夏,杜仲,八月扎,人参,丹参,夏枯草,当归,白术,丹参,柴胡,山楂,枳壳用清水浸泡,浸泡两小时后,将所泡药物和浸出液一同倾入锅内,加适量清水。一般第一次煎煮水液高出药料5厘米左右,先以小火加热,待药料充分膨胀,即以武火煮沸,再转用文火煎熬,保持微沸,煎沸1-2小时后,过滤取汁。残渣续加清水再熬,第二次加水量一般淹没药即可。如法煎煮3次,煮至药料已透,无硬心煎液气味淡薄为度,取出煎汁,将残渣加以压榨,榨出液过滤,与数次煎煮滤液合并,静置沉淀,反复以纱布过滤,去沉淀物,便取得药汁清液。
2.碾碎:将炮制后的天花粉,神曲,蟾酥碾碎,过120目筛备用。
3.醋制鳖甲:每100克鳖甲用醋20g,白糖20g炒20分钟,冷去后研碎过120目筛备用。
4.斑蝥的制作,将沙土置锅内加热,喷入5克酒,10克醋,待沙土温度达90℃时,加斑蝥,轻轻翻沙土20分钟,取出斑蝥,除去头,足,翅,然后碾碎,过120目筛备用。
5.烘干:将过滤后的药液与2.3.4.及青黛制成的药粉混匀制糊状,用180 W电烤箱温度80℃,烤成块状,然后粉碎,过120目筛,在120℃消毒30分钟,最后分袋包装,每袋10g。
实施例1
药物组成:斑蝥3g,壁虎30g,蟾酥1.5g,水蛭25g,莪术30g,半夏30g,喜树20g,天花粉30g,鳖甲60g,杜仲50g,八月扎30g,人参50g,丹参30g,青黛30g,夏枯草40g,当归30g,白芍40g,白术40g,柴胡40g,神曲40 g,山楂50g,枳壳30g。
制备方法,具体步骤同上述基本制备工艺。
实施例2
药物组成:斑蝥2g,壁虎20g,蟾酥1g,水蛭20g,莪术20g,半夏20g,喜树10g,天花粉20g,鳖甲80g,杜仲20g,八月扎20g,人参20g,丹参50g,青黛50g,夏枯草20g,当归40g,白芍30g,白术30g,柴胡60g,神曲60g,山楂60g,枳壳60g。
制备方法,具体步骤同上述基本制备工艺。
综观全方,具有扶正培本,清热解毒,活血化瘀,益气养血,养阴生津,化痰软坚,理气止痛,消肿散结,滋阴补肾,健脾化湿等功能,对中晚期癌症可稳定病情,使瘤体缩小,减少痛苦,延长生命,对体弱还适于化疗及放疗者较为适宜,并可与化疗,放疗联合使用,可减少副作用,提高疗效。共治疗消化道肿瘤560例,肝癌180例,胃癌128例,食管癌136例,结肠癌60例,直肠癌56例,总有效率为79.4%,总显效率为582.%,临床治愈率(肿瘤消失)占5%。对肝癌的有效率为81.25%,显效率为59.3%,且具有费用低,制作简单,毒副作用少等特点,此方适用于原发性肝癌,胃癌,食管癌,结肠癌等消化道肿瘤。

斑蝥2g-4g,壁虎20g-40g,蟾酥1g-2g,水蛭20g-30g,莪术20g-60g,半夏20g-60g,喜树10g-80g,天花粉20g-40g,鳖甲30g-120g,杜仲20g-80g,八月扎20g-60g,人参20g-100g,丹参20g-80g,青黛20g-80g,夏枯草20g-100g,当归20g-60g,白芍30g-60g,白术30g-60g,柴胡10g-100g,神曲10g-100g,山楂20g-120g,枳壳10g-60g。
2.按照权利要求1所述的治疗消化道肿瘤的中成药,其特征在于它由下列用量的中药制成:
斑蝥3g,壁虎30g,蟾酥1.5g,水蛭25g,莪术30g,半夏30g,喜树20g,天花粉30g,鳖甲60g,杜仲50g,八月扎30g,人参50g,丹参30g,青黛30g,夏枯草40g,当归30g,白芍40g,白术40g,柴胡40g,神曲40 g,山楂50g,枳壳30g。
3.按照权利要求1所述的治疗消化道肿瘤的中成药,其特征在于它由下列用量的中药制成:
斑蝥2g,壁虎20g,蟾酥1g,水蛭20g,莪术20g,半夏20g,喜树10g,天花粉20g,鳖甲80g,杜仲20g,八月扎20g,人参20g,丹参50g,青黛50g,夏枯草20g,当归40g,白芍30g,白术30g,柴胡60g,神曲60g,山楂60g,枳壳60g。
4.一种如权利要求1至3中任意一项所述的治疗消化道肿瘤的中成药的制备方法,包括以下步骤:
(1)将泡制后的喜树,壁虎,水蛭,莪术,半夏,杜仲,八月扎,人参,丹参,夏枯草,当归,白术,丹参,柴胡,山楂,枳壳用清水浸泡两小时后煎煮,煎煮1-2小时后过滤取汁液,如此法煎煮三次;
(2)把泡制后的天花粉、神曲、蟾酥粉碎,过120目筛;
(3)将每100克鳖甲用食醋20g、白糖20g炒20分钟,冷凉后研碎过120目筛;
(4)将斑蝥浸入盛有沙土的锅内,翻炒,炒黄后冷凉研碎,过120目筛;
(5)将步骤(1)中得到的药液与上述步骤(2)、(3)、(4)中制得的中药粉末与青黛粉末混合后,干燥、灭菌,粉碎后过120目筛。
