Abstract
A surgical implement for removing the cement from a cavity in a bone which has been previously used as a seating for an implant for an artificial limb joint, and which requires to be replaced, has a cylindrical shaft (1) with a cutting screw (3) at its end, terminating in a point, and has a T shaped handgrip (5) at the top, with conical portion (4) somewhat lower down to act as a stop. The design avoids the need to have excessive engagement forces acting on the bone material itself which could be detrimental to the bonding already established for the implant.
Description
Die Erfindung betrifft ein chirurgisches Instrument zum Extrahieren von Knochenzement aus dem Hohlraum eines Röhrenknochens.
Muss ein Knochenimplantat, das als Gelenkersatz mit Hilfe von Knochenzement im Knochen befestigt worden ist, aus irgendwelchen Gründen wieder entfernt werden, so bereitet das Entfernen und Ausschlagen des in der Markhöhle des Knochens verankerten Zementköchers sehr oft grosse Schwierigkeiten. Nach einer bekannten Methode ist es notwendig, in dem seitlichen Mantel des Knochens in Längsrichtung ein Stück weit ein Fenster anzubringen, um den Knochenzement in Richtung auf das offene Ende des Knochens hin ausschlagen zu können. Diese Methode besitzt vor allem den Nachteil, dass sie einen erheblichen Eingriff in die Substanz des Knochens erfordert, wodurch sich seine Festigkeit und seine Eignung für eine erneute Aufnahme eines Implantates erheblich verschlechtern.
Der Erfindung liegt die Aufgabe zugrunde, ein Instrument zum Extrahieren von Knochenzement zu schaffen, mit dem dieser aus der Markhöhle entfernt werden kann, ohne dass ein ins Gewicht fallender Eingriff in die Knochensubstanz notwendig wird. Gelöst wird diese Aufgabe durch einen stangenförmigen Körper. der an seinem einen Ende einen Gewindeschneider trägt, der seinerseits zur Körpermitte hin in ein Schraubengewinde übergeht, während das andere Ende des Körpers mit einem Handgriff versehen ist, und durch einen im Abstand vom gewindetragenden Ende des Körpers angeordneten Anschlag. Das erfindungsgemässe Instrument schneidet in eine zuvor in dem Zementköcher eingebrachte Bohrung ein Gewinde und verankert sich mit diesem selbst im Boden des Köchers.
Mit Hilfe des Anschlages kann der Zement dann aus der Markhöhle ausgeschlagen werden, ohne dass der Knochen wie bisher aufgemeisselt werden muss.
Als Werkstoff für das Instrument eignen sich vor allem die für chirurgische Instrumente und/oder Knochenimplantate üblichen Metalle und Metallegierungen, z. B. also Titan, Titanlegierungen, rostfreier Stahl oder die bekannten Co-Cr Ni-Mo -Legierungen.
Im folgenden wird die Erfindung anhand eines Ausführungsbeispiels im Zusammenhang mit der Zeichnung näher erläutert.
Fig. 1 zeigt eine Ansicht des Instruments, während
Fig. 2 in grösserem Massstab, ebenfalls in einer Ansicht, das mit einem Gewindeschneider versehene Ende des Instrumentes wiedergibt.
Der stabförmige Instrumentenkörper 1 trägt an seinem einen Ende, das gegenüber dem übrigen Körper 1 - der sei nerseits in seinem Querschnitt beliebig ausgeführt sein kann einen geringeren Durchmesser aufweist, einen Gewindeschneider 2; an diesen schliesst sich zur Körpermitte hin ein
Schraubengewinde 3 an. Die Länge dieses Gewindes 3 wird so bemessen, dass sie mindestens der Zementdicke entspricht, die erfahrungsgemäss am unteren Ende eines Köchers vorhanden ist.
Im Abstand vom Gewinde 3 sitzt, bereits auf dem dikkeren Teil des Körpers 1, ein Anschlag 4, der mit dem Stab 1 fest verbunden, z. B. verschweisst, ist. Am freien Ende des dickeren Teils ist, ebenfalls durch Schweissen, ein Handgriff 5 befestigt. Dieser kann jedoch auch, z. B. über einen Vierkant, abnehmbar mit dem Stab 1 verbunden sein.
Die Verwendung des erfindungsgemässen Instrumentes, das Teil eines daneben einen Körner, einen Bohrer sowie einen Schlitzhammer umfassenden Bestecks zum Extrahieren von Knochenzement ist, geschieht beispielsweise folgendermassen. Nach dem Entfernen des Implantatschaftes wird der Zement soweit, wie ohne Eingriff in die Knochensubstanz möglich, entfernt und der Köcher möglichst vollständig ausgekratzt. Mit Hilfe des Körners wird dann der Köcherboden angerissen, anschliessend wird unter Kühlung mit Hilfe eines Bohrers im wesentlichen in Achsrichtung des Knochens ein Kanal durch den Köcherboden gebohrt. Durch diesen Kanal wird dann das erfindungsgemässe Instrument hindurchgeführt, wobei es in die Kanalwand ein Gewinde schneidet, in dem sich sein Gewindeteil 3 fest verankert.
Mit Hilfe des Schlitzhammers, dessen Schlitz an die Dicke und Form des Schaftes 1 im Bereich des Anschlages 5 angepasst ist, wird der Köcher schliesslich aus dem Knochen ausgeschlagen.

If a bone implant, which has been fixed in the bone as a joint replacement with the help of bone cement, has to be removed again for whatever reason, the removal and knocking out of the cement socket anchored in the medullary cavity of the bone very often causes great difficulties. According to a known method, it is necessary to make a window in the lateral mantle of the bone in the longitudinal direction in order to be able to knock out the bone cement in the direction of the open end of the bone. Above all, this method has the disadvantage that it requires considerable intervention in the substance of the bone, as a result of which its strength and its suitability for a new insertion of an implant are considerably impaired.
The invention is based on the object of creating an instrument for extracting bone cement with which it can be removed from the medullary cavity without the need for any significant intervention in the bone substance. This task is solved by a rod-shaped body. which carries a thread cutter at one end, which in turn merges into a screw thread towards the center of the body, while the other end of the body is provided with a handle, and by a stop arranged at a distance from the thread-bearing end of the body. The instrument according to the invention cuts a thread into a bore previously made in the cement quiver and anchors itself with this in the bottom of the quiver.
With the help of the stop, the cement can then be knocked out of the medullary cavity without the bone having to be chiseled as before.
The metals and metal alloys customary for surgical instruments and / or bone implants, e.g. B. So titanium, titanium alloys, stainless steel or the well-known Co-Cr Ni-Mo alloys.
The invention is explained in more detail below using an exemplary embodiment in conjunction with the drawing.

Screw thread 3. The length of this thread 3 is dimensioned so that it corresponds at least to the cement thickness which, according to experience, is present at the lower end of a quiver.
At a distance from the thread 3 sits, already on the thicker part of the body 1, a stop 4, which is firmly connected to the rod 1, for. B. is welded. A handle 5 is attached to the free end of the thicker part, also by welding. However, this can also, for. B. be detachably connected to the rod 1 via a square.
The use of the instrument according to the invention, which is part of a set of cutlery, which also includes a center punch, a drill and a slotted hammer, for extracting bone cement, is done, for example, as follows. After removing the implant shaft, the cement is removed as far as possible without interfering with the bone substance and the quiver is scraped out as completely as possible. The bottom of the quiver is then torn with the aid of the center punch, and a channel is then drilled through the bottom of the quiver while cooling with the aid of a drill, essentially in the axial direction of the bone. The instrument according to the invention is then passed through this channel, cutting a thread into the channel wall in which its threaded part 3 is firmly anchored.
With the aid of the slotted hammer, the slot of which is adapted to the thickness and shape of the shaft 1 in the area of the stop 5, the quiver is finally knocked out of the bone.
