Abstract

Verfahren zum automatisierten Bestimmen von hämodynamischen Parametern in einem Blutgefäß an invasiv aufgezeichneten Pulswellen durch eine Analyse dieser Pulswellen und/oder von aus der Gesamtheit ausgewählten Pulswellen, mindestens jedoch einer Pulswelle, durch ein Aufzeichnen eines zeitlichen Druckverlaufs in einem Blutgefäß an nur einem einzigen Messpunkt in dem Blutgefäß, wobei der dabei aufgezeichnete Druckverlauf nachfolgend einer automatisierten Signalverarbeitung zugeführt wird, wobei an einem vorher bis zu dem vorgesehenen Messpunkt innerhalb des Blutgefäßes eingeführten Druckmesskatheter (1) mit einem Druckmesssensor (2) folgende Verfahrensschritte ausgeführt werden: - Aufzeichnen und Speichern des zeitlichen Druckverlaufs und Erfassen mindestens eines Pulswellensignals (PW) an dem Messpunkt, - Extrahieren eines Pulswellenechoanteils (PE) aus dem mindestens einen Pulswellensignal (PW) und Ermitteln eines Zeitversatzes (T) zwischen einem Pulswellengrundsignalanteil (PG) und dem Pulswellenechoanteil (PE) im Pulswellensignal (PW), - Umrechnen des Zeitversatzes (T) in eine Pulswellengeschwindigkeit (PWV) mittels eines gegebenen Referenzabstandes zwischen einer Echostelle des Blutgefäßsystems und der Position des Druckmesssensors (2) im Blutgefäß und/oder - Bestimmung von insbesondere einer Pulswellengeschwindigkeit (PWV) ohne Berücksichtigung eines Referenzabstandes über ein gegebenes Druck/Fluss- Modell und ein Druck/Volumen-Modell des Blutgefäßsystems.Method for the automated determination of hemodynamic parameters in a blood vessel from invasively recorded pulse waves by analyzing these pulse waves and/or pulse waves selected from the totality, but at least one pulse wave, by recording a temporal pressure curve in a blood vessel at only a single measuring point in the blood vessel, wherein the pressure curve recorded in this way is subsequently fed to an automated signal processing, wherein the following method steps are carried out on a pressure measuring catheter (1) with a pressure measuring sensor (2) previously introduced up to the intended measuring point within the blood vessel: - recording and storing the temporal pressure curve and detecting at least one pulse wave signal (PW) at the measuring point, - extracting a pulse wave echo component (PE) from the at least one pulse wave signal (PW) and determining a time offset (T) between a pulse wave base signal component (PG) and the pulse wave echo component (PE) in the pulse wave signal (PW), - converting the time offset (T) into a Pulse wave velocity (PWV) by means of a given reference distance between an echo point of the blood vessel system and the position of the pressure measuring sensor (2) in the blood vessel and/or - determination of in particular a pulse wave velocity (PWV) without taking a reference distance into account via a given pressure/flow model and a pressure/volume model of the blood vessel system.
