Description
Foreliggende oppfinnelse vedrører en blanding mellom agomelatin, eller /V-[2-(7-metoksy-l-naftyl)etyl]acetamid, med formel (I):
eller dets hydrater, krystallinske former og addisjonssalter med en farmasøytisk akseptabel syre eller base, og et tymoregulerende middel valgt fra litium, karbamazepin, valproat og lamotrigin for å oppnå farmasøytiske sammensetninger for anvendelse i behandlingen av alvorlig depressiv lidelse, syklotymisk lidelse og dystymisk lidelse.
Agomelatin, eller /v-[2-(7-metoksy-l-naftyl)etyl]acetamid, har det dobbelte karak-tertrekk av å være, på den ene side, en agonist av reseptorer i det melatoninergis-ke system og, på den andre side, en antagonist av 5-HT2c-reseptoren. Disse egenskaper gir den aktivitet i sentralnervesystemet og, mer spesielt, i behandlingen av alvorlig depressiv lidelse, vinterdepresjon, søvnforstyrrelser, kardiovaskulære patologier, patologier i fordøyelsessystemet, insomni og tretthet på grunn av jetlag, appetittforstyrrelser og obesitet.
Agomelatin, dets fremstilling og dets anvendelse i terapeutika har blitt beskrevet i europeisk patentbeskrivelse EP 0 447 285.
Det er nå funnet at agomelatin, eller /V-[2-(7-metoksy-l-naftyl)etyl]acetamid eller dets hydrater, krystallinske former og addisjonssalter med en farmasøytisk akseptabel syre eller base, anvendt i assosiasjon med et tymoregulerende middel i form av litium, karbamazepin, valproat og lamotrigin har verdifulle egenskaper som tillater dets anvendelse i behandlingen av alvorlig depressiv lidelse, syklotymisk lidelse og dystymisk lidelse.
Lidelser i sentralnervesystemet, slik som sinnsstemningslidelser, påvirker et stort antall mennesker i alle aldere. De benevnes slik fordi konsekvensen av vedvarende positive eller negative effekter av tilstrekkelig alvorlig intensitet frembringer mistil-passet atferd.
Alvorlig depressiv lidelse er en akutt og alvorlig sinnsstemningslidelse kjennetegnet ved tristhet, pessimisme, ideer om selvmord, en ideomotorisk nedgang og forskjellige somatiske besvær. Ekstremt funksjonshemmende kan det føre til opphør av all funksjonell eller sosial aktivitet. I tillegg til det deprimerte individs egen lidelse kommer familiens lidelse.
Syklotymisk lidelse er kjennetegnet ved den repetitive forekomst av positive (eks-pansjonistisk sinnsstemning) og negative (depressiv sinnsstemning) tymiske variasjoner som er mindre alvorlige enn de ved bipolar lidelse, med mindre funksjonell svekkelse; diagnosevanskeligheten er imidlertid reell, og noen elementer fører til troen på at syklotymisk lidelse er forløperen til bipolar lidelse. Assosiasjonen av et antidepressiv og en tymoregulator tillater at tymiske variasjoner kontrolleres og forhindrer dem fra å utvikle seg til en karakteristisk bipolar lidelse.
Til sist er dystymisk lidelse en kronisk og intens sinnsstemningslidelse som er kjennetegnet ved lange perioder med dysforisk sinnsstemning og funksjonell svekkelse. Akkurat som for yrkesutmattelsessyndrom kan de andre symptomer på dystymi inkludere de følgende følelser: mangel på tilpasning, fortvilelse, irritabilitet eller overdrevent sinne, skyldfølelse, tap av interesse eller av generell glede, sosial til-baketrekking, kronisk tretthet, nedgang i aktivitet eller produktivitet og dårlig kon-sentrasjon. Dystymi er en snikende mental lidelse. I motsetning til de invalidiseren-de funksjonelle symptomer assosiert med sykdommer slik som alvorlig depressiv lidelse, lider individer grepet av dystymi generelt av moderat sosial og yrkesmessig dysfunksjon. For eksempel, til tross for typiske forstyrrelser i sin generelle mellom-menneskelige funksjon, ses dystymiske individer ofte å arbeide iherdig i sitt yrke og opprettholde en fasade av normalitet.
Selv om et stort antall effektive molekyler eksiterer i dette feltet - tymoanaleptika og tymiske regulatorer kan nevnes mer spesielt - tillater ingen av disse forskjellige patologiske tilstander å bli behandlet med fullstendig tilfredsstillelse, og flere av dem har signifikante bivirkninger. Følgelig pågår utviklingen av nye alternative behandlinger og fortsetter å være en nødvendighet.
Det er nå overraskende oppdaget at agomelatin anvendt i blanding med et tymoregulerende middel i form av litium, karbamazepin, valproat og lamotrigin har egenskaper fullstendig egnet for behandlingen av alvorlig depressiv lidelse, syklotymisk lidelse og dystymisk lidelse. Slike tymoregulatorer, generelt beskrevet for sine anti- maniske egenskaper som tillater dem å virke på den tymiske ekspansivitet i maniske tilstander, anvendes for tiden i behandlingen av bipolare lidelser.
Det er nå oppdaget at disse tymoregulerende midler innehar den karakteristiske egenskap at de forsterker effektene av agomelatin både i feltet for slike depressive sinnsstemningslidelser og maniske lidelser som nevnt ovenfor.
Denne uforutsigbare effekt tillater anvendelsen av blandingen ifølge oppfinnelsen å bli overveid i behandlingen av sinnsstemningslidelser og, mer spesielt, alvorlig depressiv lidelse, syklotymisk lidelse og dystymisk lidelse.
En slik blanding mellom agomelatin eller dets hydrater, krystallinske former og addisjonssalter med en farmasøytisk akseptabel syre eller base, og litium, karbamazepin, valproat og lamotrigin kan benyttes for å oppnå farmasøytiske sammensetninger tiltenkt for behandlingen av alvorlig depressiv lidelse, syklotymisk lidelse og dystymisk lidelse.
Oppfinnelsen vedrører også slike farmasøytiske sammensetninger inneholdende en blanding mellom agomelatin eller dets hydrater, krystallinske former og addisjonssalter med en farmasøytisk akseptabel syre eller base, og litium, karbamazepin, valproat og lamotrigin i kombinasjon med én eller flere farmasøytisk akseptable eksipienser for anvendelse ved behandling av alvorlig depressiv lidelse, syklotymisk lidelse og dystymisk lidelse.
Blant de aktuelle farmasøytiske sammensetninger kan det nevnes, mer spesielt, de som er egnet for oral, parenteral eller nasal administrasjon, tabletter eller drageer, sublingvale tabletter, gelatinkapsler, lozengere, suppositorier, kremer, salver, der-male geler etc.
I tillegg til agomelatin og den nevnte tymoregulerende forbindelse omfatter de far-masøytiske sammensetninger én eller flere eksipienser eller bærere valgt fra fortynningsmidler, smøremidler, bindemidler, desintegrerende midler, absorpsjons-midler, fargestoffer, søtemidler etc.
Eksempelvis kan det nevnes:
som fortynningsmidler: laktose, dekstrose, sukrose, mannitol, sorbitol, cellu-lose, glyserol, som smøremidler: silika, talk, stearinsyre og dens magnesium- og kalsiums-alter, polyetylenglykol,
som bindemidler: aluminium- og magnesiumsilikat, stivelse, gelatin, tragant, metylcellulose, natrium karboksymetylcellulose og polyvinylpyrrolidon,
som desintegrerende midler: agar, alginsyre og dens natriumsalt, brusende blandinger.
Den nyttige dosering varierer i henhold til pasientens kjønn, alder og vekt, adminis-trasjonsruten, lidelsens natur og alle assosierte behandlinger og strekker seg fra 1 mg til 50 mg agomelatin per 24 timer og er mer foretrukket 25 mg per dag. Do-sen av det aktuelle tymoregulerende middel vil være mindre enn den anvendt når det administreres alene.
Farmasøytisk sammensetning:
Formulering for fremstillingen av 1000 tabletter hver inneholdende 25 mg aktiv ingrediens: N-[2-(7-metoksy-l-naftyl)etyl]acetamid .................................................... 25 g Laktose-monohydrat ............................................................................... 62 g Magnesiumstearat ................................................................................. 1,3 g Povidon .................................................................................................. 9 g Vannfri kolloidal silika ............................................................................ 0,3 g Cellulosenatriumglykolat ......................................................................... 30 g Stearinsyre .......................................................................................... 2,6 g Klinisk studie: Det utførte kliniske studiet anvender internasjonal nomenklatur, i hovedsak DSM-IV, og også validerte måleverktøy, slik som Hamiltons skala for depresjon, Youngs maniskala, den globale kliniske inntrykksskala, som alle er instrumenter anbefalt av retningslinjene som gjelder. Sammenligning av agomelatin-valproat-blandingen versus tymoregulator-placebo under beste metodebetingelser tillater oss å konklu-dere med at blandingen er overlegen.
Claims (3)
1. Blanding mellom agomelatin, eller /v-[2-(7-metoksy-l-naftyl)etyl] acetamid eller dets hydrater, krystallinske former og addisjonssalter med en farmasøytisk akseptabel syre eller base, og et tymoregulerende middel valgt fra gruppen bestå-ende av forbindelsene litium, karbamazepin, valproat og lamotrigin for anvendelse ved behandling av alvorlig depressiv lidelse, syklotymisk lidelse eller dystymisk lidelse.
2. Blanding ifølge krav 1 hvor det tymoregulerende middel er valproat.
3. Farmasøytisk sammensetning omfattende som aktiv bestanddel agomelatin eller dets hydrater, krystallinske former og addisjonssalter med en farmasøytisk akseptabel syre eller base, i blanding med et tymoregulerende middel valgt fra litium, karbamazepin, valproat og/eller lamotrigin, alene eller i kombinasjon med én eller flere farmasøytisk akseptable eksipienser for anvendelse ved behandling av alvorlig depressiv lidelse, syklotymisk lidelse eller dystymisk lidelse.
