- Eleven-year-old Jake Chambers (Tom Taylor) experiences visions involving a mysterious warlock called the Man in Black (Matthew McConaughey) who seeks to destroy a Tower and bring ruin to the world, and a Gunslinger who opposes him. Jake's vision involves a facility on top of a plateau, which contains many children playing and interacting, under the observation of creatures who wear human skin. They are recognized by a cut in the skin behind their ear. At a designated time, an alarm blares and some of the children walk towards a pyramid like a modern structure. Here, the Man in Black puts the children in captivity and harnesses their psychic energy or their "shine" to launch an attack on the Tower. As the tower crumbles, dark forces are unleashed upon the living world.
Jake's mother Laurie (Katheryn Winnick), stepfather Lon (Nicholas Pauling), and psychiatrists dismiss these as dreams resulting from the trauma of his father's death the previous year. Meanwhile, several cities around the world have been rattled by a series of earthquakes in recent weeks. Jake keeps sketches of his dreams on a wall in his room as he tries to piece together the mystery. Jake tells the psychiatrists that quakes are occurring on the planet as the tower is under attack.
Jake has another vision where the Man in Black slaughters many gunslingers by burning them with the fire from hell. A Gunslinger named Roland meets his father who says that they are not lost till the tower still stands. The Man in Black attacks and kills the father Steven with a magic spell. However, the magic does not work on Roland and hence the Man in Black is not able to kill him.
At his apartment home in New York City, a group of workers from an alleged psychiatric facility offered to rehabilitate Jake. Recognizing them from his visions as monsters wearing human skin, he tries to warn Laurie, but Laurie thinks that Jake is just acting up at the prospect of being taken away. Jake enters the bathroom and escapes via the window. Jake tracks down an abandoned house from one of his visions, discovers a high-tech portal, and travels to a post-apocalyptic world called Mid-World.
Jake enters a desert with nothing around for miles. As he walks towards a hill in the distance, in the evening, he notices a small fire burning on it. In Mid-World, Jake encounters the last Gunslinger, Roland Deschain (Idris Elba) who emerged in his visions. Roland is pursuing Walter Padick, the Man in Black. A ruthless, ageless deceiver and sorcerer who seeks to reach the Tower and rule over its seemingly infinite kingdoms.
Walter had also appeared in Roland's dreams across a desert, seeking to kill Walter in revenge for the murder of his father, Steven (Dennis Haysbert) and other fallen Gunslingers. He explains that Walter has been abducting psychic children and is attempting to use their powers to destroy the Dark Tower, a fabled structure located at the center of the universe. This will allow monsters from the darkness outside to invade and destroy reality. Roland tells Jake that there are infinite worlds in the Universe, and the Dark Tower stands at the center of them all. It sends out positive energy that protects the Universe and shields it from what is outside. When the tower falls, Walter will be able to rule by unleashing his monsters on the Universe.
Roland takes Jake to a local village in order to have his visions interpreted by a seer. On the way, Jake is lured by a monster who approaches Jake in the form of his dead father. As Jake approaches him, the monster takes its true form and tries to abduct Jake, but he is saved by Roland who shoots the monster and forces it to retreat. Roland tells Jake that every time the tower is attacked, there is a small tear in the fabric of the Universe and a few monsters make their way through. Just then, Roland is attacked by a monster and Jake is forced to pick Roland's gun and shoot the monster to save Roland.
Having learned of Jake's escape by using a portal on Earth, Walter investigates and discovers from his minion Sayre that Jake has "pure Shine", enough psychic potential to destroy the Dark Tower all by himself. Walter kills Jake's stepfather, then he interrogates his mother about her son's visions.
Back in Mid-World, the seer determines that the machine is six months away on foot and portal access is restricted to Walter's bases. Jake realizes that Walter has a base in New York that they can use to reach the machine.
Suddenly, the Taheen, Walter's minions, attack the village, but Roland kills many of them. Roland and Jake return to Earth where Roland's injuries are treated at a hospital. Jake learns the location of Walter's base from a homeless man who helped him earlier. When Jake returns home to check in on his mother, he finds her charred remains and breaks down in tears. Seeing this, Roland vows to avenge her death. This leads to him teaching Jake the basics of gun fighting, as well as the Gunslinger's Creed, which he has not uttered since his own father's death.
As Roland replenishes his supply of ammunition at a gun store, he is attacked by Walter, who captures Jake and takes him through a portal at his base to a machine that will destroy the Dark Tower with Jake's powers. Jake uses these psychic powers to alert Roland to the portal code that he needs and Roland battles his way through Walter's henchmen, reopening the portal, which Jake forces to stay open. Walter is forced to return to New York to fight Roland and wounds him. When Jake reminds him of the Gunslinger's Creed, Roland recovers and kills Walter with a trick shot after a brief fight. Finally, he destroys the machine and saves the Dark Tower, Jake, and the other children.
Roland prepares to return to his own world and offers Jake a place by his side as his companion. Jake accepts the offer and the two head back to Mid-World together.
