There is a lot of graphic violence, but there is no explicitly sexual content and relatively little cursing for a king book.(less)

The first refers back to puberty starting the growth of the unabsorbed pieces of Thad's twin, which started growing again causing his headaches and visions. They don't detail it much but in the few sentences at the time and the discussion by Prichard, we get the picture of the tumor appears to be an eye, teeth, and something else. I would assume that it was preserved for study or as a curiosity somewhere.
The second explanation has to do with the duality ability of a twin, even if the other is not alive, they are always connected. So Thad in his ability to write in two distinct writing styles was using his creative connection residue from his lost twin. His imagination gave life to George.(less)
