Important Information

When you start make sure to play using the Blood: Refreshed Supply version as that will give achievements. The MS-DOS version is the original release of the game and won't give achievements.

Blood: Refreshed Supply consists of the original game and several add-ons. Completing each episode will unlock a corresponding achievement and there are also secret level exits in each episode of the game that also each give an achievement when found.

There is only one achievement related to difficulty and it is for the last level in Epsiode 5: "Post Mortem".

Occasionally you'll find yourself low on health and dying repeatedly. When this happens it is useful to backtrack to look for secrets to replenish your health and armor. Refer to this additional secrets guide to find extra weapons and items, particularly Life Seed and Super Armor, which can really turn around a dire situation.

Find a super secret

Super Secrets are very rare secrets that don't count towards the normal secret count. There are several hidden throughout the game but the first one you can find is at the end of the first level: E1M1: Cradle to Grave.

At the end of the level you will look through a broken part of the wall and see some coffins, Cultists and the exit button below. Before dropping down in to this area make a running jump off the raised edge of the cracked wall () and move straight in to the stone wall ahead of you ().

It is a tricky jump to make but eventually you will pass straight through the fake stone wall to find a super secret containing a Napalm Launcher.
‎‎‏‏‎ ‎‏‏‎ ‎Secret Exit
Digestive Irritant
Find the secret level in The Way of All Flesh

The secret exit in Episode 1: The Way of All Flesh is located in the level E1M4: Dark Carnival.

Proceed through the level as normal until you open the Moon Key Door. Shortly after opening it you will come across a broken bridge over some water. Instead of swimming in the water to find the normal exit you should jump on the nearest wooden pier () that is sticking out of the water and carefully jump across them until you reach the one at the end ().

You'll now be facing a wall with a crack (). Blow it up using Dynamite or the Napalm Launcher to reveal a small cavern with more water.

Swim through this small underwater cavern and you will surface in a small cave with some Super Armor and Life Seed items. There is also a combination lock featuring the key icons (). Using each key icon will cycle through the various keys. You need to change them to the order in which you picked up the keys for this level: Dagger, Eye, Moon.

The message "You're going to the secret level!" will appear and you will automatically be taken to the secret level: E1M8: House of Horrors.

‎‎‏‏‎ ‎‏‏‎ ‎Secret Exit
Should Have Gone to the Beach
Find the secret level in Even Death May Die

The secret exit in Episode 2: Even Death May Die is located in the level E2M4: The Overlooked Hotel.

This complex secret requires you to collect five Tomes that are scattered throughout the hotel to activate a secret teleporter.

Proceed through the level as normal until you reach the hotel itself. Once inside the entrance hall head up the stairs and pick up the Skull Key from the balcony () but watch out for the fireball trap. Now drop back down and open the Skull Key Door () and pick up the Fire Key which you'll need later.

Head back up to the balcony and look to the left of hole created by the fireball trap earlier. The red part of the wall () is a secret entrance to the library. Open it and head inside.

Inside the library look for the end of a bookshelf which is a slightly different colour (). Use it to reveal it's an elevator that will take you up to the location of Tome A.

Whilst still in the library drop back down and look for a painting of a woman (). You can blow it up with Dynamite to reveal a secret hole behind it.

Jump and crouch in mid-air to squeeze your way through this small hole and pick up Tome B.

Head back through the hole, back through the library and back to the corridor. Take a right turn and look for a ticking grandfather clock. Enter the wooden door () that is nearby.

Inside this room use the nearby bookshelf () to slide it out of the way and reveal Tome C.

Return to the corridor again and continue on and you will see the Fire Key Door ().

Once inside look next to the bed to find Tome D and also pick up the Dagger Key () as you will need it shortly.

Exit the room back to the corridor and look over the edge. You'll see two staircases. Drop down and ascend the one on the left () and you'll soon see the Dagger Key Door. Open it and head inside.

The final Tome E will be sitting on the desk near the Spider Key.

Now you have all the Tomes explore the rest of the level and eventually on the other side of the hotel you'll come to a very large room with a fireplace and the Moon Key. If you have found all five Tomes then a black pentagram symbol will have appeared on the floor ().

Enter it to be teleported outside near the exit to the secret level: E2M9: Thin Ice.
‎‏‏‎‎‎‏‏‎ ‎‏‏‎ ‎‎I'm Not Staying in This Freaking House Another Second
I'm Not Staying in This Freaking House Another Second
Complete The Haunting (E2M5) in under 40 seconds

Normally this level is quite large and requires a lot of keys and exploring. To beat it in this strict time limit you will need to skip large amounts of the level as well as move quickly and avoid fighting enemies.

The recommend way to skip parts of the level involves using the Boots of Jumping item taken from a previous level in this episode. This lets you easily jump over large hedges and skip most of the level.

If you don't have that item then you can also throw some Dynamite and jump on top of the explosion to boost yourself up. This is of course a lot harder to get right and has a high risk of killing you.

From the start of the level move forward and up a staircase and you will be outside facing a building with the Skull Key Door. Go to the right of this, activate your Boots of Jumping and jump (or Dynamite boost) on to the hedge (). From here then jump in to the window ().

Inside the room you will immediately see the Moon Key (). Grab it and jump back out the window.

This time go to the hedge on the left side of the Skull Key Door. Once again use your Boots of Jumping or Dynamite to jump over the hedge (). On the other side continue forward through a narrow hedge corridor.

You'll come to a large open outside area so look for the Moon Key Door in the corner (), open it and head through.

Inside you'll soon find a large swimming pool. Look for a crack () under the water and blow it up. There are plenty of explosives nearby if you need them.

Jump in the pool and swim through the crack. After swimming through the tunnel past all the monsters you will surface in a small stone cave area. Blow up the TNT Barrels () you find here.

Once the TNT Barrel explosion has stopped there will be two paths to take. Take the path to the left () and you will find the exit button for the level.

Hopefully you made it through the level in under 40 seconds. If you didn't, reload and try again.

‎‎‏‏‎ ‎‏‏‎ ‎Watch out for Pedestrians
Watch out for Pedestrians
Complete Ghost Town (E3M1) without any Innocents getting killed

If you want to make this achievement completely trivial then simply set the difficulty to "Made To Order" with "Monsters = Off". This completely empties the level of Innocents as well as enemies so all you have to do is complete the level to earn the achievement.

However if you want to attempt this achievement on a normal difficulty then you need to get to the end of the level with no Innocents getting killed, either by your hand, the enemy or explosions. For the method below to work you will need to select one of the lower difficulties: Still Kicking, Pink on the Inside or Lightly Broiled.

It is vital that you stay at 100 health and not take damage from enemies at the start as you will need all your health for Dynamite jumping. Use quick-save and quick-load a lot to make sure everything goes well.

At the start of the level scour the streets outside and pick up ammo for the Shotgun, the items in the dumpster, and most importantly a Bundle of TNT (). Once that's done head in to the building with a grey door ().

Be careful in here as there is an Innocent under attack from some Zombies so carefully kill them. Around the corner is one or more Fanatics with machine guns so make sure to kill them without losing any health. Pick up the Fire Armor as that will help you survive the TNT explosions. Finally, use the painting in the corner () to reveal a secret with two more Bundles of TNT.

Head back outside to the streets and head down the alley on the left side. You'll probably see a glass window opposite a large crack above you but ignore that window for now. Continue down the alley and you will see another glass window above you (). Shoot it out with your gun then throw your first TNT bundle in to that room. Listen carefully as this has to kill the Cultist in there or he will shred you with his shotgun.

Once he is dead you have to use your second TNT bundle to Dynamite jump up to the window. To do this throw a TNT bundle in front of you (but not too close) and then jump on the explosion to let it give you a massive height boost. It is very important to not lose more than 50 health doing this so keep trying until you get it right.

Inside the room grab the Dagger Key then carefully drop back down to the alley.

Return back the way you came down the alleyway and look for another glass window (), this time it is opposite a large crack in the wall. Shoot it out and then use your last TNT bundle to Dynamite jump up to the window.

This will leave you very low on health so immediately kill the enemies in here. Grab the Case of TNT from this room and blow up the crack () you saw opposite the window.

Jump over through the hole in that building and open the Dagger Key Door and you will be in the final room with the exit button.
‎‎‏‏‎ ‎‏‏‎ ‎Secret Exit
Better Left Buried
Find the secret level in Farewell to Arms

The secret exit in Episode 3: Farewell to Arms is located in the level E3M4: The Sick Ward.

Shortly after opening the Fire Key Door you will enter a small chapel. Head over to the Skull Key and then turn around to look at the doorway you came through. High above it is a very small skull switch (). Shoot it to activate a hidden door behind you.

Turn back around and you'll notice blue light coming out of the secret door ().

Head inside to find the exit to the secret level: E3M8: Catacombs.

Find the secret level in Dead Reckoning

The secret exit in Episode 4: Dead Reckoning is located in the level E4M4: Crystal Lake.

Proceed through the entire level right up to the final room where you surface from the green water in to a dark cave. Ignore the exit button and instead blow up the wall () to the right of it and you will reveal a small cave.

Inside the cave is a secret exit button that will take you to the secret level: E4M9: Mall of the Dead.
[Episode 6: "Post Mortem"]
Priest, Beast, Deceased
Complete Post Mortem

Episode 6? Wait... where's Episode 5?
The "Post Mortem" episode was originally added by an expansion pack called "Plasma Pak" created by Monolith and released about 6 months after the main game. When Monolith made the original game it contained 4 episodes however the multiplayer Bloodbath levels were internally considered episode 5. So when the "Post Mortem" episode was released it was named internally as episode 6. Even more confusing is that the Cryptic Passage expansion pack (which wasn't made by Monolith) was released before Plasma Pak and is sometimes considered as episode 5.
For Blood: Refreshed Supply the "Post Mortem" episode is grouped with the original Monolith episodes while Cryptic Passage is separated in to the Add-Ons menu.

"Post Mortem" consists of 9 levels which includes 1 optional secret level.

NOTE: The final level in this episode has an achievement that requires playing on "Well Done" difficulty. If you want that one make sure to select that difficulty when playing this episode.

Find the secret level in Post Mortem

The secret exit in "Post Mortem" can be found in the level E6M6: Forbidden Rituals.

Proceed through the level as normal until near the end. You will be facing the Moon Key Door which takes you down to the normal exit. Don't go through it and instead shoot the stone eye symbol () above the door to trigger a nearby hidden wall ().

Inside is the hidden exit to the secret level: E6M9: Forgotten Catacombs.
‎‎‏‏‎ ‎‏‏‎ ‎Chosen-in-Regress
Chosen-in-Regress
Defeat the 3 Beasts of Beauty and the Beast (E6M8) in under 3 minutes on Well Done difficulty or above

NOTE: This achievement has been fixed in the version 2.2 patch and now should be obtainable. If you load an old save from the previous version you will have to restart the map by loading in from the previous level or dying and restarting.

This achievement requires that you are playing on the Well Done difficulty mode. In the level previous to this one try to acquire as much ammunition for your weapons as possible, particularly the Tesla Cannon, Napalm Launcher & Life Leech.

Once you reach E6M8: Beauty and the Beast there are three Priests in the level (they look like Cultists with dark orange robes) that morph in to the Beasts. It's best to focus on them one at a time and deal with each as quickly as possible.
  • Make sure to save at the start of the level so you can retry from the beginning if you're too slow.
  • There is a Guns Akimbo power up that works very well with the Tesla Cannon. Unleashing your entire ammo supply while dual-wielding the Tesla Cannon will kill a Beast very quickly.
  • The stomp attack the Beast uses can be avoided by jumping. You can avoid it almost every time by continually jumping while fighting them.
  • Using the alt-fire of the Life Leech will drop the staff on the ground and it will start attacking nearby enemies. This can add some extra damage while you attack with another weapon.
  • If a Beast runs in to a corner then unleash the Napalm Launcher as the knock back will probably keep them stuck in the corner while you keeping firing.

Kill 100 Cultists

Cultists are the most common enemy and you will easily encounter more than 100 throughout the game. There are many variations of Cultist and all of them count towards the achievement:

The brown robed Cultists use a Shotgun and occasionally throw a stick of Dynamite.
The black robed Fanatics shoot Machine Guns.
The blue robed Zealots wield Tesla Cannons.
The green robed Ackolyte exclusively throws Dynamite.

‎‎‏‏‎ ‎‏‏‎ ‎Swallow This

Kill a Choking Hand

Choking Hands are a fairly common enemy throughout the game. If they grab you and start choking you to death press the "Use" key and you will throw them off.

The first Choking Hand in the game can be found in E1M4: Dark Carnival behind some bars but they regularly appear throughout the game.

Oh My God... You're Rabid!?
Exploit the Hellhound's greatest weakness

This achievement requires you to kill a Hellhound creature using it's weakness: water. If you can lure or blast a Hellhound in to a body of water it will drown and die. Hellhounds will start to appear in the game from Episode 3 onwards.

For a quick way to get this achievement simply start a new game and select Episode 4: Dead Reckoning to play the level E4M1: Butchery Loves Company. From the start of the level drop down and go through the wooden door () at the top of the stairs.

In here look for a colourful stained glass window (). Smash it and jump outside.

Head past the pool of liquid and around the corner (). You'll soon encounter one or two Hellhounds (). Quickly retreat and jump in to the pool of water ().

While in the water simply hold down jump to float at the surface of the water. This causes the Hellhounds to aggressively attempt to reach you () and subsequently jump in to the water and die.

Kill yourself 10 times in single-player

The Chosen-In-Regress patch is live since yesterday, so you can do it without the beta branch. I just did it :)

"Chosen-in-Regress", damn I just finished that on Extra Crispy and got no achievement... I quickly speed runned the whole Post Mortem again on Well Done, did this again and again no achievement. Now I'm reading it's bugged... I already saved in the last map so I'll have to replay the whole episode again on beta branch...

Chosen-In-Regress:
"NOTE: This achievement is still bugged as of version 2.1"

So you say that the in game timer counts, and you can save as much as you want if the bug will be fixed at some point. I thought that the real time counts and that is why I did not receive it. BTW, it looks like if you survive, the 3 minutes is very much doable.

I was not aware of the announcement of Hotfix #2 announced on Dec. 18. so I didn't op in to the beta, thanks.
Xazuki  [author] 5 Jan @ 5:42pm
The fix isn't in a regular update yet, did you opt in to the separate beta like the news update described?
Al Capone 5 Jan @ 4:55pm
It was after when the update released. I completed Post Mortem last night.
Xazuki  [author] 5 Jan @ 4:11pm
Did you swap over to the Beta version for that Al Capone? If you did, did you load an old save of the level in progress or did you try the level fresh from the start?
Al Capone 5 Jan @ 4:05pm
Chosen-in-Regress still bugged for me. Kill the beasts under 2 minutes at least 5 times on Well Done.
Mandingo Warrior 26 Dec, 2025 @ 4:39pm
The first level in the 4th episode has 2 hell hounds on Lightly Broiled. After you go through the door where the first key is jump out the window and run up the hill. Have them chase you down then jump in the water pool there. One should follow you in.

Pro Tip for Watch out for Pedestrians, use Made to Order difficulty and turn off monsters, that will remove all off them from the level and beating it will still drop the achievement

I used some TNT jumping tricks to then get the key to leave the level so idk if it's still beatable with no monsters
