I've attached my Commands.ini file which demonstrates the problem.

When loading on my PC the "Powershell" item appears after the "Admin
Powershell" item when it should appear between the "Console" and "Git Bash"
items.

Also a separator is displayed between "Admin Console" and "Admin
Powershell" rather than after "Admin Powershell".

Attempting to re-arrange the items in the options menu does not work, after
accepting the changes the items are in a completely different order, e.g. I
might move the "Powershell" link to the correct location and after
accepting it is somewhere else and several other items are now also in
alternate locations.

I also have had this issue - for a while now actually, but have just tried to learn
to live with it.  It is rather annoying though.

I tried your commands.ini file, and it works as expected on my machine. Also
reordering of commands works just fine.

    I'll do some more testing...
