fixed issue #2993800: adding a notice to an application will no longer result in invalid xccdf

Added Yes, No, Cancel option dialog for adding rules or groups to profiles that are dependent upon other groups and or rules.

+ fixed an issue with the howto panel that would cause the autosave manager to throw a validation exception and bring down the RT.

+ Made sure that output documents are written in UTF8 encoding

Make groups added to applications selected by default.
Don't show groups or rules that are required by other groups or rules in profiles.
Don't show non-selected sub groups or rules of group in a profile.
Reworked how items are added to table model list.

+ fixed issue #2991100: edits to a newly created reference persist
+ changed return types in ApplicationListViewPanel from ArrayList<...> to List<...>

- Removed debugging System.out.println() statements

- Replaced LoginDialog with LoginWindow; a JFrame so it appears in the task bar.
