Implemented two selects in FileTypeDAO. One overrides the base class and returns a list of FileUpdateResponses that contain a temporary file along with the FileTypes and codes.

The other returns a single FileUpdateResponse with the FileType and a temp file for a given app_id, file_id.

Ipmlemented getFile() method that returns the contents of the file from the database in a temporary file.

Assume we should close up shop if the login dialog closed and the user is not valid. Don't worry about checking to see if the user canceled. He may have closed the dialog with the 'x' widget on the window bar.

Implemented select for a single application id that returns the populated application item.

Updated application table and DAO to include proper ids and dates

Fixed the UserTypeDAO class and the DB creation script to include schema-required fields.

++ Added a call to VersionedItemTypeHelper.markAsNew() to take care of some required fields (this may be unnecessary)
