Compare the insert tabs in the .pdf with: http://en.logilogi.org/Logi_Logi=Admin_User_4/do/logi_inserts/new Thus it must be that more went wrong with the than you detected. (Also note that in order to update your production environment, and not just your local development-copy you need to run rake server:production:deploy_full, as described in the instruction- e-mail I sent to you for this on the 19th of September). I have now done this from a local copy configured to update the logilogi.lirmm.fr server. When updating (which confirmed that it was still an old version) I also noted that the newest rails gem was not yet installed, so I don't know what you did when I asked you to update them, but I hope it works now.
