Tested 3 times, twice on different USB flash drives, and once on the SD card. Encrypted volume will fail before it is mounted / dismounted 10 times with Windows 11 Pro.
Steps to failure:
1. Auto mount devices, enter password.
2. Double click highlighted drive letter to open.
3. Volume opens in File Explorer, double click any folder, content is displayed. Click on any item and Explorer crashes.
4. Dismount the volume.
From now on, when mounted again the volume is visible in VeraCrypt

So it looks that VeraCrypt sees the volume, but it is unable to open using Windows Explorer.
But when double clicked to open, Windows message says,

and Explorer comes with a message to format drive.

The external storage shows as in RAW format that can’t be undone in Windows. Fdisk does not work with RAW format drives, encrypted files are not recoverable according to hundreds of Internet inquiries I run on this question.

I think this is something you’ll need to look at, at one point after volume is mounted and files are visible, a single click on any file wipes out the drive formatting.
