Fix custom scoring system by adding maximum scores facility.

Previously, items with custom scoring in feedback were not able to pass a
maximum score for each item to the feedback engine. This oversight has been
corrected and proper methods have been added to affected item classes.

Fix Bug when no test was selected by the filter.

Fixed error regarding ticket 1974532; also fixed this error in testmaker3 installation.

Display conditions and more than one item per page now can go together when the conditions are tied to items that are NOT in the current item block.
