Slack App Home should link to GitLab settings

Once the Slack account is linked with GitLab, there isn't a clear way for the Developer or Admin to see or manage your connected + authorized GitLab projects.

Proposal

Once the user accounts are connected, include a link in the App Home that links to the Chat profile screen.
