Multiple approval groups with "Default" as name are added to projects

While attempting to create an MR against https://gitlab.com/gitlab-com/people-ops/employment, I noticed the following in the MR creation widget.

The same name Default was reused again. This prevented from the MR being created because it violated the uniqueness property of Name. and I had no way to change that from MR widget.

@nick suggested looking in the project settings for approval rules, and I see the same table as above screenshot there also. Few interesting facts

1. Somehow three GitLabbers, who in the past were "eligible approvers", were now added as approval groups.
2. Same name, Default was reused for all these approval groups.

I think this is a bug/regression.
