Ask to provide template values when adding a file template

Description

We have nice templates which help people create licenses and .gitlab-ci.yml files, but after adding those templates, there's sometimes required configuration required. Rather than depending on the user editing the files before committing, how about presenting a form with one or more fields that the user needs to provide before we instantiate the template?
