Allow a user to unlink their GitLab.com account from their SAML identity

Description

This issue is related to the ongoing SSO work for GitLab.com 🚀

Users should have control over their GitLab.com accounts; if a user is no longer affiliated with an organization or feels uncomfortable having their account affiliated with an identity provider, a user should be able to undo this link.

Proposal

• A user should be able to unlink their account from an affiliated identity provider.
  • If a user takes this action, she/he should be removed from the affiliated group. The user should be aware of this before they take this step.
