Process reference to attachment in nav

If a nav entry references an attachment, only the content (the text) of the nav entry is shown in the aggregate document. If the site URL is specified (either in the playbook or using the site-url attribute in the Assembler configuration), the content should link to the attachment in the published site. (It should be handled much like a nav entry that's an external link).
