💊 Pharmacy benefits strategy rarely makes the agenda at the executive level. The assumption: it's complex, it's handled by HR, and there are bigger competing priorities. With costs accelerating, that assumption is becoming one of the most expensive ones an organization can make. A few numbers worth knowing: 1️⃣ The average employer-sponsored health plan is expected to increase 6.7% in 2026, pushing the bill above $18,500 per employee. For a company of 55,000 people, that's well over a billion dollars a year. (Mercer, 2025 National Survey of Employer-Sponsored Health Plans) 2️⃣ Prescription drug costs are driving a significant share of that increase — and they're the fastest-rising category in employer health plans, projected to climb 11% in 2026. 💊 Same drug, same day. One pharmacy got reimbursed $8,000. The other got $50. That's a result of the predatory reimbursement structures legacy PBMs use to favor their own pharmacy networks, and it's a major driver of independent pharmacy closures across the country. Two audits put the scale of this in context: 1️⃣ The State of Tennessee's audit of Caremark (CVS) exposed same-day reimbursement disparities for identical products, with CVS's corporate pharmacy line receiving over 100x more reimbursement than independent competitors for the same drugs. 2️⃣ The 2024 FTC Interim Report on PBM Markups found that PBMs affiliated with subsidiary pharmacies showed preferential reimbursement rates thousands of percent higher than competitor pharmacies.
