Notes: The copyright page states 'Reprinted 1976 by Special Arrangement'. Apparently a small print run as could only find one other copy for sale on the internet. May not have had a dust jacket as this copy does not nor does the one copy for sale could find. Binding material is different on the two copies (tan cloth with red titles on spine, red cloth with gold lettering and front board art).
