The "two words and comma" form - in plain block letters - is on the page 2 before the title page, and also on the page 2 after the title page; plus it's 2 words in the story text. The latter form (2 words and comma) used for consistency and clarity. ("etcetera" is one word in the block letter versions, so one word form maintained for "et cetera".)
- The cover artist isn't specifically credited, but the clock on the cover is also included in the interior, and the mice are same style as in the interior.
