Inmates would be cuffed to the bed, in a way that they were able to operate, from the start. If they needed both of his hands free, they would have cuffed his leg(s). They would not have his hands cuffed to each other across his abdomen like that, regardless.

There would also typically be at least two guards with an inmate, sometimes more for truly dangerous offenders (as he was reported to be).
