Throughout the opening scenes, the white ninja moves around with a katana on his back. Later on, he shoots an arrow using his bow but where was the bow?
Over the course of three different scenes, Mary Anne's gag starts off covering her mouth completely, then later is pulled between her teeth, and finally ends up spreading her lips apart but covering her teeth.
Over the course of the opening sequence the white ninja's sword changes position from shot to shot. At one point, the sword keeps jumping between his back and belt with every camera angle change.
During the opening sequence, Cole, the white ninja, shoots an arrow at a maroon ninja standing by a tree. When he draws back the arrow and fires it, the arrow is white, but when the arrow hits the tree narrowly missing the ninja's face, the arrow is brown.
During the opening scenes, the white ninja fights two red ninjas then engages a third red ninja by impaling the red ninja's hand to a tree with a throwing knife then "killing" him with a shuriken to the forehead. If you look closely, the white ninja's swords have mysteriously disappeared from his back and waist sash.

Factual errors

When Cole goes to the penthouse in ninja garb, he throws a star and hits a guard in the chest killing him. But the rifle fires in the same area as the pool with girls and other henchmen nearby and no one acts like they heard a shot.

Revealing mistakes

At the beginning of the movie during the Fight with the red Ninjas, A few minutes after he shoots his bow at the black Ninja Cole the white Ninja throws three exploding egg smoke bombs. If you look closely you can see the explosion before the eggs hit the ground.
During the fight when Cole and Mary Ann return from town, Cole kicks the second thug, but the double for Cole was topless.
Towards the end of the opening scenes, the white ninja's master is shown, multiple times, to be watching from outside the house. After defeating the black ninja, the white ninja approaches his master, who is clearly a live human being because of head movement and blinking eyes. When the ninja bows in respect to his master and the camera pans back, the "master" is now a mannequin and is beheaded by the white ninja.
When Cole, the white ninja, at the beginning of the movie is walking by a tree and a maroon ninja is slowly making his way down. you can barely see the bamboo steps added to give the ninja some foothold as he descends.

Crew or equipment visible

During the opening sequence Cole does a back flip to get down from a tree and lands behind some tall grass, as he pushes the grass aside and steps through you can see the stuntman who did the back flip trying to hide behind him.
