- When a hitman is diagnosed with a terminal illness, he decides to take a hit out on himself. But when the very hitmen he hired also target his ex-girlfriend, he must fend off an army of assassin colleagues.
- Follows veteran assassin Joe Flood, who is diagnosed with a life-threatening illness and authorizes a kill on himself to avoid the pain that is destined to follow. After ordering the kill, he finds out that he was misdiagnosed and must then fend off the army of former colleagues trying to kill him.
- At a dance performance in Budapest, hit-man Joe Flood (Dave Bautista) carries out an assassination, but is spotted by guards. The target's security entourage and the opera house's security engage each other, not knowing who had carried out the assassination. A gunfight ensues which spills over the to the main stage of the opera house, scattering all the dancers. In the ensuing chaos, Joe spots a fallen dancer, Maize (Sofia Boutella), and helps get her out safely. Maize had to be carried out as she had hurt her ankle and wrist in a stampede caused by the shootout. She tries to thank him, but Joe starts to suffer a blinding headache and leaves before Maize can help him. Joe retreats to his hideout, which is extremely well protected with the latest security gear.
After a complete medical examination for his headaches, Joe meets his handler Zvi who informs him that the target was a human trafficker, and Joe did a favor by taking him out. Joe made $750,000 on the contract, minus 30% for the cleanup, minus expenses, minus Avi's commission, and it leaves little for Joe. Joe wants another target, but Avi encourages him to take a vacation and live his life.
Joe finds Maize at her dance studio and returns her phone, which she lost during the previous mayhem. Maize and Joe soon fall for each other, with him lying about his profession to her. Maize reveals her struggle as a dancer, which has resulted in multiple injuries to her in the past and says that she is looking forward to her retirement.
Joe continues his missions all over Europe as he dates Maize. Joe tells Zvi about Maize and says that he wants out. Zvi respects the fact that Joe always adhered to the rules for assassins and says that Joe's exit from the business can be arranged. Joe asks Zvi to get him a life insurance policy.
Maize throws a birthday party for Joe, and he requests Zvi and his wife Sharon to attend. Sharon and Maize hit it off and Zvi also approves of Maize. Zvi tells Joe that he understands why he wants to quit. Zvi tells Joe to tell Maize the truth about himself as he says that a life cannot be built on a lie.
Joe is soon diagnosed with Creutzfeldt-Jakob disease (a rare, fatal brain disorder that causes dementia and muscle control issues), with his physician, Dr. Kagan (Raffaello Degruttola), giving him roughly three months to live. Kagan asks Joe to get into palliative care at the earliest.
Joe tries to get his friend and handler Zvi (Ben Kingsley) to put a contract on him for $2 million (that Joe would pay Zvi for the contract), which would allow Maize to collect life insurance off of Joe's death, but Zvi refuses. Joe turns to Marianna Antoinette (Pom Klementieff), another assassin and handler, to handle the contract. Joe was the one who executed a contract on Antoinette's father. Joe says that he was a good assassin, but he broke the rules and hence deserved the death sentence.
Marianna, whose father also mentored Joe, accepts the contract, and soon sends the hit out, which takes effect at midnight. That night, Joe publicly breaks up with Maize, and later explains his diagnosis in a drunken voice message.
Just before midnight, Dr. Kagan calls Joe to inform him that he was misdiagnosed, as his test results were switched with another patient. At midnight, Joe is attacked by the first of Marianna's assassins, the Korean Goyang (Lee Hoon) and his gang, whom he quickly kills. The Hungarian Langos brothers, who attack together with their motorcycles, follow, and Joe kills them too.
Joe tries to cancel the contract, but Marianna refuses (as this was an opportunity to kill Joe that she did not want to lose, especially after Joe says that he enjoyed killing her father) and contacts every available hit-man in Europe as she doubles the contract from the initial $2 million to $4 million. Marianna decides to spend her own money to bring Joe down.
Lovedahl Gasevich (Terry Crews), who turned down the initial contract, joins the hunt, along with Scottish brothers Angus (Scott Adkins) and Rory Mackenzie (Drew Galloway), the British strippers Ginni (Lucy Cork) and Tonya (Shaina West), a.k.a. the Party Girls, and a dancing assassin called Botas (Marko Zaror). Joe calls Maize and asks her not to go back home and tells her to get out of Budapest. He asks her not to tell him where she is going and promises to find her wherever she is.
Joe visits Zvi and his wife, Sharon (Alex Kingston), the latter of whom fixes his headaches (as she used to be a massage therapist) and bad vision. Joe sets up traps at a nearby castle to lure the other assassins there. Most of the assassins arrive to attack Joe, but he quickly takes them out.
Meanwhile, Lovedahl arrives in Budapest, where he is forced to partner with an aspiring assassin, Money. Marianna says that Money is her cousin, whom she is trying to mentor into the business. Money is awkward, clumsy, and not trained for the assassin line of work. Marianna calls Lovedahl and tells him about Maize, based on photos shared by her bodyguard Max. The two of them track down Maize to her apartment and take her captive. Lovedahl calls Joe through Maize's phone and tells them to meet at a nearby cemetery.
When Joe arrives to confront Lovedahl, Maize escapes by making a fool out of Money, while Lovedahl was busy confronting Joe. She steals Lovedahl's car and rams into him before she takes Joe with her. A hit squad, led by Marianna's right hand man Max (Daniel Bernhardt), shows up and fires on Joe. A furious Lovedahl has Max call Marianna and demands that she increase the contract to $6 million before he leaves Money behind. Meanwhile, Joe tells Maize that he is an assassin, and of his misdiagnosis, but that he is willing to retire to be with her. She then tells Joe that she is pregnant and would not want to raise a child with a murderer. Maize starts to pass out because one of Max's bullets hit her in the shoulder.
Joe brings Maize to a church, where the Priest, Father O'Brien (Dylan Moran) gives them sanctuary as Joe tends to Maize. Joe proposes to Maize shortly after she wakes up, and O'Brien reluctantly agrees to marry them. The ceremony is interrupted by the arrival of Max and his men, who injure the priest shortly before Lovedahl shows up.
Joe kills most of Max's squad before he pins Lovedahl under some scaffolding. Max tries to kill Joe, but Maize kills him first with a head-shot, to her horror. Joe takes Maize out of the church and leaves an injured but alive Lovedahl there. Meanwhile, Zvi confronts Marianna at her compound and executes her for breaking the assassins' code. Marianna hired Max's mercenaries on a contract job, which was not allowed.
Joe and Maize later get married, and while he promises to retire from the assassin life, Maize tells him to consider the costs of raising a child, and suggests he not give it up entirely.
