Director Antonia Bird's script originally addressed darker subject matter, such as mental illness and teen suicide. However, due to controversy surrounding Bird's previous film, O Padre (1994), which was released while Mad Love was in production, Touchstone Pictures had her leave out the darker material. Casey's behavior is also relegated to mere depression, when her symptoms indicate she suffers from bipolar disorder.
Drew Barrymore's character's name is Casey, which is also the same name as her character in Scream which was released a year later.
