Elected to the House of Representatives as a Democrat from California, representing San Francisco, in a special election following the death of Representative Sala Burton, and has served June 2, 1987 - present.

Her father was Thomas D'Alesandro Jr., Baltimore's first Italian-American mayor. Her mother was Annunciata D'Alesandro. Her brother Thomas J. D'Alesandro III was also elected Mayor of Baltimore.

Achieved the highest political rank for a female in American history when she attained the position of 60th "Speaker of the United States House of Representatives" (4 January 2007).

She was honored by the American Conservatory Theatre in San Francisco, California for her efforts on behalf of the company in securing federal support for the rebuilding of the theater.

(October 20, 2019) Her older brother, Thomas J. D'Alesandro III, died of stroke complications at the age of 90.

In 2020, she became the oldest-sitting Speaker of the House of Representatives, surpassing Sam Rayburn.
