At the end of each episode, they reveal which stories are "fact" or "fiction".

While the factual stories are described as "real" or "actually happened", it would be more accurate to say they are based on documented incidents, and not invented by screenwriters. Whether or not they have been thoroughly investigated and validated, is never revealed.
