In 2000 cobalt-60 was unwittingly exposed in a scrap metal dump in Thailand. Several of the workers became ill and subsequently died. This was the only recorded incident involving the radioactive substance.

Towards the end there is a uniformed policeman in front of a door. On his right hip is a "clam shell" holster. The holster is cut in half and held together with a spring. It is released by putting the finger into the holster and into the trigger guard and pushing a button; the holster then springs open, releasing the revolver. The danger is that failing to push the button, prevents getting the revolver out. These are no longer used.
