Shot in 4:3 (1.3:1) ratio which is very close to Academy ratio 1.37:1, so comes across on-screen as a Square picture. 4:3 ratio was used for the old square-boxed-screen style standard television sets as 4:3 ratio was, also, the aspect ratio used for 35mm films in the silent era.
The opening scene in which the man stands on stage to warn the audience about what they're about to watch is based in reality. It was common before the age of television and the Internet for theatre owners to build up tension and hype for horror films by having someone stand up on stage and "prepare" the audience for what they were about to witness. In some cases, theatre owners even rented ambulances and actors in medical uniforms to sit outside the theatre, giving the impression that the film being shown was so scary that emergency medical services were there in case they were needed.
