- It's 2006: Major Rebecca Gordon, a highly-trained trauma surgeon, and Captain Bobby Trang, a brand-new doctor, report for duty at the NATO Role 3 Multinational Medical Unit at Kandahar Airfield. Perched at the edge of the airfield and built out of plywood and canvas held together with duct tape, the temporary building that houses the Role 3 shakes every time a plane takes off. The hospital's commander, Colonel Xavier Marks, tells the newcomers to expect an experience unlike any they've had before. Rebecca thinks she can handle anything, while Bobby is acutely conscious of his inexperience. They both get off to a rocky start, as Colonel Marks points out Rebecca's mistakes and tries to build Bobby's confidence. But over the next 48 hours, as they deal with rocket attacks, flying shrapnel and wave after wave of wounded U.S. Marines, Canadian soldiers, Afghan civilians and even a Taliban prisoner, they manage to rise to the challenge, even as they grow more and more exhausted. Welcome to Kandahar, indeed.
- When both Dr. Simon Hill and Chaplain Marjorie Plottel are called in to treat and comfort Sgt. Geoffry Daums, a severely injured patient, their worlds threaten to collapse. Simon is on the verge from dealing with guilt and anger over the past that haunts him. And Plottel has lost her faith. Daums, who has suffered a major spinal trauma and may be left quadriplegic, calls on Chaplain Plottel to help him grapple with his own faith. Simon, trying to redeem himself in his own eyes, petitions Col. Marks to allow him to do a risky surgery to help Daums regain use of his arms. Because she promised Daums she wouldn't leave him, Plottel finds herself in the O.R. as Simon tries to work his magic. A friendly repartee turns into deadly barbs as the surgeon and the Padre go toe to toe. As things heat up in the O.R., Majors Rebecca Gordon and Grace Pedersen have to respond quickly at the Women's Clinic when a young Afghan teacher brings in her young charges who have been poisoned and are near death.

But when the mysterious Special Forces Agent, "Snake Eater" returns, Col. Marks and Pedersen find out that not all is as it seems. Knowing that further questioning will increase the emotional trauma to the children, Pedersen is caught between her professional and personal ethics and the need to find out what the children know regarding an imminent Taliban attack. As Pedersen grapples with her conscience, the Canadian and American hockey teams play a game of one upsmanship - each trying to psyche the other team out. Captain Bobby Trang and Major Rebecca Gordon get caught up in the escalating rivalry which explodes in the rink as things get out of control. Meanwhile, Rebecca resumes her flirtation with Snake Eater, much to Simon's chagrin. And Will is reunited with a young friend.

Fitz and Col. Marks are rushed to the Role 3 where Major Rebecca Gordon has been in charge of triage in Col. Marks' absence. Because of a previous group of injured, their supplies are low, and the operation to save Fitz will use their last resources. It seems possible, until an ambushed Afghan family is brought to their Trauma Bay. Following the rules of combat triage, Rebecca has to choose - the family or Fitz -- who is to go to the O.R. and live and who has to wait, which means certain death. But being true to her profession, she immediately begins saving his life. But not before the bullet intended for Samizay has mortally wounded one of our own. The Role 3 is left shocked that one of their own is killed and both Rebecca and Bobby have to grapple with their feelings when they're called upon to save the life of the man who killed their friend. Dr. Simon Hill is on his own journey when an urgent call in the middle of the night breaks up his tryst with Rebecca and sends him on a drinking binge that threatens his medical license.
- Rebecca is nearly run over by a pick-up truck full of Afghan National Army soldiers heading for the hospital. Their leader -- who turns out to be an American Special Forces operative -- carries in one of his men, delirious with a serious infection, one that neither Rebecca nor Bobby have encountered before. When another patient comes down with the same infection, the team realize they're dealing with a highly contagious and resistant bacterial strain, with no idea what it is, where it came from or how to stop it from spreading. Colonel Marks is left with no choice but to quarantine the OR, ICU and the recovery room, so that the Role 3 medics are forced to do surgery in the dining hall. Meanwhile, Rebecca accompanies Pedersen off base to the Women's Clinic, where they find a local in dire need of emergency life-saving surgery.
