- Guts, a swordsman formerly belonging to The Band of the Hawk, stumbles upon a disturbed town while wandering the wilderness with his companions Puck the elf and Casca, a traumatized woman whom he is trying to cure. He discovers that the town is victim to a deadly disease called the "Mandragora". He makes a deal with the ruler of the town to retrieve a cure, so his doctors can cure the people, and possibly Casca as well. But can Guts trust the Baron?

Both Wallace and Rakjit eventually gather a party, which roams the world, visiting various locations and advancing the story in a predominantly linear fashion. The appearance of random enemies triggers a battle screen, which is viewed from an isometric perspective. The battle system is rather old-fashioned, with physical and magical attacks; level progression is automatic, as in most games of the genre. Sometimes the game prompts the player to choose an action or a response; while the flow of the story is rarely influenced by the player's decision, the game keeps track of these decisions, evaluating the player's attitude between chapters.
- The player's spaceship has a ball for a weapon, which can be fired in eight directions; each direction corresponds to a different musical note. The note plays when the player presses the fire button, and is also quantized in time so that it matches the beat playing in the background. By using the weapon selectively the player can improvise music while playing.
